Failure (such as seizure) to any of the other switches to the other three capstans. Cracks occur in this weld. Cracks occur in the welds between the centre pole and the vertical webs between the two plates. 58 Ramba Zamba, Trebant, Twist Restraint failure Restraints may be operated from inside the cars. The threaded section of the adjusters has failed. Ensure operator has manufacturers tensioning technique advice. Ultrasonic NDT on threaded parts. See detailed report. 66 Quasar ARM Fatigue cracks Cracks have been found on the H frame upper pivots, radiating from the hinge pin holes. The region is inaccessible without stripping down. 71 Ski Jump Pollard Bearing failure Careful examination and maintenance of the arm bushes is vital. 75 Balloon Race Zamperla Fatigue cracks Cracks found in the lifting trunnions which attach the upper end of the hydraulic ram rods to the main centre of the ride. Lack of air results in the brake releasing. Brakes should fail to safe. Three incidents have occurred where children hve placed their feet outside the walls of the galleon and have struck fixed features. Annual inspection to check if it is possible to trail a leg or make contact with a fixed feature. Ensure the loading platform retracts Fit entrance gates in the side of the galleon Provide suitable signs. Containment systems should protect against Falls as result of reasonably foreseeable passenger behaviour Inadvertent opening of restraints Passengers thrown forward during deceleration in the water trough and sustaining injury. Look for footrests, handrails, padding at the front of the log, depth of water etc. 109 Miami trip Emmett Fatigue Failure Need for 6 monthly NDT by MPI of car shafts. 111 Barriers All Falls Barriers should normally be 1m high.
