3 Southwestern Ohio Nurses Association (SONA) About The Southwestern Nurses Association (SONA) is one of 26 local districts of the Ohio Nurses Association (ONA). Membership in SONA is open to nurses who live or work in Brown, Clermont, Clinton, Hamilton and Warren counties. Membership in SONA offers Registered Nurses opportunities for involvement in issues of interest to nursing and healthcare as well as discounts and other benefits. SONA has a membership newsletter and holds general meetings at various locations in the greater Cincinnati area. Meetings provide opportunities to meet and network with other RNs. Educational programs are offered throughout the year to aid in the earning of continuing nursing education (CE) credit. Our Mission The mission of the Southwestern Ohio Nurses Association (SONA) is to advance professional nursing and quality health care in the district. This mission is accomplished by furthering SONA s leadership and advocacy roles in the community through education, communication, collaborating and mentoring. Our Vision The Southwestern Ohio Nurses Association (SONA), a constituent of the American Nurses Association, is a collaborative organization of registered nurses recognized as a proactive leader in the promotion of nursing and quality in health care. Community Health Nurse Managed Clinic at DIC SONA District is blessed with a strong Human Rights Committee that has faithfully served the local Drop Inn Center with a weekly nurse managed clinic since All RNs are welcome regardless of your expertise. 4 SONA Human Rights Committee Nurse Managed Clinic at the Drop Inn Center Philosophy The Human Rights Committee of the Southwestern Ohio Nurses Association conducts a nurse s clinic for men and women who visit the Drop Inn Center, a shelter in downtown Cincinnati for those who are homeless. Acceptance of and respect for all persons undergirds the care given at the clinic. The development of trusting relationships and nonjudgmental attitudes are fundamental. The g rowing problem of homelessness does not occur in a vacuum: no one thing causes it and no one thing will solve the problem. However, providing nursing care is an opportunity to demonstrate concern while simultaneously empowering persons to make changes in the status of their health. Nursing services provided include health assessment, education, counseling, health maintenance, and referral. There is an emphasis on assisting people to help themselves through teaching and caring. Approved September 8, 1997 Policies for Clients Seeking Services 1. Clients will have a selective screening assessment of their health status. 2. Health problems will be identified, cared for, or referred to area agencies. 3. Any emergent health problem will be immediately referred to Teaching about health behaviors will be done in response to a client s interest and need. 5. The self care behaviors of clients will be encouraged. The clinic is open on Tuesday afternoon from 4:00 p.m. to 6:00 p.m. November

5 The Drop Inn Center The Drop Inn Center is a facility that provides shelter to 250 homeless men and women each day. It was started in the early 70 s by Buddy Gray whose vision was to prevent homeless people from dying on the streets. It is located on the corner of West 12 th and Elm Street, and is run by volunteers and staff to provide basic needs to the homeless population in Cincinnati. For more information, please visit their website at: Our Mission Doing All We Can At the Drop Inn Center, our mission is simple as a community of residents, staff and volunteers, we work together to provide basic human services for the men and women of the Greater Cincinnati area who are experiencing homelessness. Our primary commitment is to shelter offering a place to stay for those in need now, and helping find and maintain a permanent place of residence for the future. For those who don't have to imagine what's it's like to be hungry, alone, frightened, homeless, people for whom this is a reality, the Drop Inn Center is a place they can turn to for hope, for help. 6 General Information Parking The Drop Inn Center is located at 217 West 12 th Street which is on the corner of 12 th and Elm Streets. Metered parking is available on the West 12 th Street and Elm Street side of the building. There is a charge until 5:00 p.m. Parking is also available across the street from the DIC on 12 th Street at no cost. DIC security guards are available to escort you to your car. Tour Prior to volunteering at the Drop Inn Center, you should take a tour of the facility to get a better understanding of the population of people you will be working with. To arrange for a tour, you will need to contact Fanny Johnson at , ext You may also come to the desk at the front and ask for the dorm manager if Fanny is unavailable. Keys/Name Tag Keys to the clinic and supply cabinets will be provided, along with a name tag that should be worn at all times while volunteering at the clinic. 9 Computer Training Computer Basics The computers are stored under the counter in the locked cabinet. (Use key provided to lock and unlock cabinets). The primary laptop is marked Main and must be turned on first and turned off last. If only one computer is being used, this is the laptop that must be used. The router is located in bag with the Main Computer. Computer setup 1. Unpack the main computer (#1) 2. Plug the router box into the electrical outlet. 3. Plug the computer electrical cord into the back of the computer & into the electrical outlet. 4. Once the computer is booted up, click on the icon for the Drop Inn Center (DIC) 5. You should now have a screen that looks like the picture on the next page. 6. Once the main computer is turned on, the second computer may be turned on and booted up. Click on the icon for DIC and continue with the following instructions to electronically chart on each patient. November

10 DIC Patient Documentation Notice the tabs across the top of the screen. Click on the tab Patient Center at the top to access the patient database. You should now have a screen that looks like the picture on the next page. November

11 Enter a new patient (or Find Existing Patient) Place cursor in the Last Name box Click Find Existing Patient box Type in the last name of the patient and select Find next (May need to scroll through a list of names) If patient found, skip to the last three steps of this process If not found, a message will appear search item was not found. Click ok. Close this box by selecting cancel Click Enter New Patient If a name is already in the box click save visit (The boxes need to be blank before proceeding with next step) November

12 Place the cursor in the Last Name box and type the patient s last name. Tab over to the First Name box and type the patient s first name Tab over and type the patient s date of birth Tab over and type the patient s age Tab over and select the patient s gender Tab over and complete the Patient Questionnaire when applicable Tab over and click in the blank space under Patient Allergy List and enter one allergy per blank space or NKA Tab over and click in the blank space under Patient Medication List and enter each of the medications the patient takes Tab over to the Patient History box and enter all pertinent past medical history Click Add/Review Clinic Visit The oldest clinic visit date will automatically appear. Click Enter New Visit and a clear screen will appear (Be sure to click Enter New Visit before typing so that a previous visit is not amended) Tab to Attending Practitioner and select your name from the drop down box. See screen shot on next page. November

13 Record the patient s vital signs by tabbing over or placing the cursor in each of the appropriate boxes Tab over or place the cursor in the Current Complaint box and enter the information for the reason the patient is visiting the clinic today Tab over or place the cursor on the RN Assessment box and type your assessment data here. (Click on Assessment Questionnaire for tips. Click on X to close box). Tab over or place the cursor on the Intervention box and type in what treatment was done. (Click on Patient Teaching for tips. Click on X to close the box). Tab over or place the cursor on the Additional Notes box to type in any additional information. This may include information like inappropriate behavior/comments, etc. November

14 Tab over or place the cursor on the Referral drop down box and select the referral service if appropriate. (See additional information at the end of this packet for proper forms to be completed) Tab over or place the cursor on the Medication Given Through Protocol box and select the appropriate medication given. You select this box for each medication given. Tab over to the Doses Given drop down box and select the total number of doses given. Tab over to the Units Given drop down box and select the total number of units given. (NOTE Doses given is equal to the number of actual DOSES given; UNITS given is the total number of pills given.) Click on the Save Visit box, the click on Back to Patient box. This will take to you back to the first screen. Click on the Save Patient Info to save everything you have entered on this patient. Click Enter new patient to advance to next screen to search and/or enter next client. November

15 Miscellaneous Cab tokens or slips may be provided for residents/clients who need to go to the hospital. When available, these may be found in the locked drawer. *For medical emergencies, please call 911 and stay with the residents/clients until EMT arrives. Continue to assess the patient and obtain vital signs and other pertinent information that may be requested when the call to 911 is placed. For help with assessment skills, a copy of Mosby s Health Nursing Pocket Resource Consultant is available and can be found on the counter. November

16 Forms See the following pages for copies of forms that will be utilized when seeing residents/clients. Sign up to see a nurse Summary Date form & place a copy on one of the tables outside of the clinic so that the DIC residents/clients may sign up to see the nurse. You will use this form to see residents/clients in the order they signed in. This form will be filled out at the end of each clinic session which summarizes the total number of residents/clients seen, types of conditions treated, and signatures of nurses working. Client Data Sheet Referral This form will be used if the computer is not working to manually record the residents/clients visit. These forms are then placed in alphabetical files which are located in the locked cabinet. All attempts should be made to use the electronic charting method. This is the form you will need to complete for the residents/clients to take to the referred health care provider, emergency room, the Cincinnati Health Department Homeless Medical Van, or dental services. The medical van is located at the DIC on Wednesday. Please see the bulletin board for other available times/locations for the medical van. (Bulletin board located next to bathroom) Medications and Infectious Diseases List of medications and total doses that may be administered. There is also a current copy taped to the door where medications are kept. Front Line Training on Infectious Diseases also provided (Page 5 excluded). Community Resources This is a list that can also be found on the SONA website which lists the various community resources available for services including pharmacy, medical, vision, dental, and various other services.
