In addition, we are a certified American Heart Association Training Center, as well as a California and Nationally accredited Continuing Education Provider. From children to adults, our courses are for those who want to save a life or find a new one in the medical field. The institute offers courses in CPR, AED, First Aid, Baby Sitting, Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Pharmacology, ECG Interpretation, Medical Technology and Paramedic Preparation. 6 Faculty Members Medical Director: Dr. Julie Cameron Dr. Cameron was born in England and moved to this country in After attending both Syracuse University, a three year nursing program, and, Virginia Commonwealth University, she emerged as an RN and a Bachelor of Science in Psychology. After spending time in the Virgin Islands, she moved to Colorado in She climbed the corporate ladder at the city hospital and decided to work and teach in the field. Her community activity included commissions that brought with it one of the first 911 systems in the country. A desire to go to medical school emerged, and after 4 years she graduated in 1988 and moved to the west coast for her residency. After completing four years at L.A. County USC in emergency medicine, with both BLS and ALS instructor certifications, she moved out into the community to practice Emergency Medicine. Dr. Cameron elected to do Sheriff and Police education, both in the classroom and on their helicopters. She joined the Baker to Vegas staff in Retired now from active Emergency Medicine, she continues to work towards educating those around her. Primary Instructors: All Primary Instructors are currently licensed Paramedics having completed at least 40 hours of additional education in adult teaching principles. All instructors also go through an approval process with the State of California and the Local EMS Agency. Mark Komins: BSEMS, Paramedic: Our Program Director has been in the field of Emergency Medical Services (EMS) for more than 23 5

7 years. He began as an Emergency Medical Technician on a volunteer Search and Rescue (SAR) Team in Los Angeles, California. Mr. Komins then became a California Licensed Paramedic in Since then, Mr. Komins has worked as a ground and flight Paramedic with private ambulance companies, as well as with a local fire department. He has also been in education for more than 20 years as well. Mr. Komins has an undergraduate degree in Fire Technology, Emergency Medical Services and a California Teaching Credential, as well as a Post Graduate Certificate in Instructional Design Systems and a Graduate Degree in Emergency Health Services. He is also certified as an American Heart Association BLS/ACLS/PALS instructor, BLS Regional Faculty, PHTLS Instructor/Course Director, ECSI Instructor, EMS Instructor and a National EMS Educator. Genia Crews: EMT and EMT instructor, SARTech3, NCPT-1 (Phlebotomy), PHTLS certified instructor, Medical Reserve Corps of Ventura County; training lead, NAEMSE, HazMatFro, American Heart Association Instructor Janet Lester: EMT, LVN: Ms Lester attended Pasadena City College and obtained her EMT certification in She worked for Medevac Ambulance Company in Los Angeles County until she returned to teach in the EMT program at Pasadena City College. She went onto teach CPR and EMT at Simi Valley Adult School. Ms Lester worked for Ventura County Fire Protection District as a civilian contract employee establishing Public Access Defibrillation programs for contracted municipalities and conducted recurrent training. She owned and operated Tri-County Training, an American Heart Association training center, and served as Regional Faculty, as well as Chair of the AHA Ventura County Task Force. Ms Lester returned to 6

8 school to earn her AA from Moorpark College and also became a licensed LVN in March of She is currently working on obtaining her BSN. Skills Instructors: All Skills instructors are experienced EMT s and Paramedics who have been recognized as instructional assistants in the field of EMS and have been approved by the State of California. Equipment Utilized EMS Training Institute, Inc. maintains a large inventory of state-of-theart education and training equipment designed specifically for the training of Emergency Medical Services (EMS) personnel. This equipment simulates equipment currently used in the field by EMS providers on a state and national level. The equipment includes specialized anatomical models and human-like manikins. Defibrillators are available for hands-on training. Equipment is also utilized for learning to manage airways and ventilate patients, treat shock and administer medications on a limited basis. 7

9 Description of Courses Classes in the classrooms at Institute location EMERGENCY MEDICAL TECHNICIAN (EMT Initial) (160 hours) (maximum student enrollment- 26) This course fulfills all of the training requirements set forth by the State of California for Emergency Medical Technicians (EMT). All students are expected to participate in both skills and lecture. The class prepares the student to render pre-hospital basic life support at the scene of an emergency, during transport of the sick and injured, or during inter-facility transfers. Students will cover selected topics and anatomy, physiology, patient assessment and emergency treatment. Theoretical learning is followed by corresponding skills training. This course provides clinical experience in actual patient care settings. There is a hospital clinical phase (12 hour shift) and a field internship phase of a pre-scheduled shift (12 hour shift). Upon successful completion of the course, students will be eligible to take the National Registry exam that is the current certifying exam for the State of California and its counties. 8

10 EMERGENCY MEDICAL TECHNICIAN (EMT) SKILLS TESTING California State Skills Certification: This course is designed to provide recertification to California Certified and National Registered Emergency Medical Technicians who are currently certified or have expired not more than 2 years prior. Upon demonstrating competency in the following skills, the student will receive a completed EMT Skills Competency Verification Form, EMSA-SCV to be submitted for recertification. The skills requiring verification of competency are: 1) Patient examination- Trauma patient 2) Patient examination- Medical patient 3) Airway emergencies 4) Automated external defibrillation 5) Circulation emergencies 6) Neurological emergencies 7) Soft tissue injuries 8) Musculoskeletal injuries 9) Obstetrical emergencies 9

11 EMERGENCY SKILLS PRACTICE- NREMT and CALIFORNIA STATE This course provides Emergency Medical Technicians with an opportunity to revisit their skills with a certified instructor before challenging their actual skills testing. PREHOSPITAL TRAUMA LIFE SUPPORT (PHTLS) 16 hours 4 year certification The Pre-hospital Trauma Life Support (PHTLS) course was created in recognition of the real need in EMS education for addition training in the handling of trauma patients. The course promotes critical thinking and shares trauma care techniques in an innovative format. This 16 hour course is designed to enhance the knowledge and skills of first responders, EMT s, Paramedics, Nurses, Physicians and other licensed health care professionals. The course provides the latest standards in pre-hospital trauma care using didactic lectures and dynamic hands-on skills sessions such as rapid extrication, pediatric immobilization, needle decompression and more. 10

12 MEDICAL TERMINOLOGY CLASS 50 hours This class prepares participants for a career in the health care industry to understand and communicate medical information. It teaches the language of medicine, focuses on the building of medical terminology through the use of word components. Students will acquire basic words, prefixes and suffixes upon which medical words are built. Learning includes medical terms related to anatomy, physiology, diseases, diagnoses, pharmacology, therapeutics and common abbreviations. PARAMEDIC PREPARATION AND REVIEW COURSE 8 weeks/ one day weekly 64 hours This intensive program is designed to prepare the EMT (Basic) for a Paramedic training program and to provide currently licensed Paramedics with a comprehensive review. The course provides instruction in those areas found to be most difficult in a Paramedic training program including patient assessment, pathophysiology, electrocardiography (ECG), and pre-hospital pharmacology. 11

13 BABY SITTER LESSONS AND SAFETY TRAINING (BLAST) 4 hours This course is an interactive program that prepares teens to interview for a babysitting job, select safe and suitable games and activities, prevent accidents, perform first aid and begin babysitting safely and competently. AMERICAN HEART ASSOCIATION CPR INSTRUCTOR- HEALTHCARE PROVIDER (BLS) This course enables an instructor to teach the following courses: BLS Healthcare Provider CPR Heartsaver CPR/AED Heartsaver Pediatric First Aid/ CPR/ AED Heartsaver First Aid All Friend and Family Courses 12

14 FRIENDS AND FAMILY CPR This is a video based course that teaches adult Hands Only CPR, AED use, child CPR and AED use and infant CPR. It also teaches how to relieve choking in an adult, child or infant. This Family and Friends CPR course is for members of the general public who want to learn CPR. HEALTHCARE PROVIDER (BLS) TRAINING This course is taught using the new science in the 2010 American Heart Association Guidelines for Cardiopulmonary Resuscitation and Emergency Cardiovascular Care. With this training, a two year Healthcare Provider Initial Training certification is issued. HEARTSAVER ADULT/ CHILD/ INFANT CPR WITH AED This is a classroom-based, video driven course led by an American Heart Association Heartsaver or Basic Life Support (BLS) Instructor. Upon successful completion of all course requirements, students receive a Heartsaver course completion card in CPR and AED, valid for 2 years. 13

15 HEARTSAVER FIRST AID This course teaches how to manage illness and injuries in the first few minutes until professional help arrives. The course includes General Principles, Medical Emergencies, Injury Emergencies and Environment Emergencies. HEARTSAVER FIRST AID/ CPR/ AED This course is designed for employee training at companies, corporations, businesses or other entities. It is designed for employees or designated first responders who have a duty to respond to a first aid or cardiac emergency because of job responsibilities or regulatory requirements. HEARTSAVER PEDIATRIC FIRST AID/ CPR/ AED The course is designed for those involved in child care who have a duty to respond to a first aid emergency because of job responsibilities or regulatory requirements, such as child care workers, teachers, foster care providers, camp counselors, youth organizations and coaches. 14

16 HEARTSAVER/ FRIEND AND FAMILY INSTRUCTOR This course gives the instructor the ability to teach the following courses: Heartsaver CPR/ AED Heartsaver Pediatric First Aid/ CPR/ AED Heartsaver First Aid All Friends and Family courses ACLS INITIAL TRAINING- 2 DAY COURSE ACLS is based on simulated clinical scenarios with hands-on participation through learning stations where students will practice essential skills individually, as part of a team, and as a team leader. Realistic simulations reinforce the key concepts: proficiency in Basic Life Support care, recognizing and initiating early management of periarrest conditions, managing cardiac arrest, identifying and treating ischemic chest pain and acute coronary syndromes, recognizing other life threatening clinical situations (such as stroke) and providing initial care, ACLS algorithms and effective resuscitation team dynamics. 15

17 ACLS INSTRUCTOR COURSE 6 hours on-line 4 to 5 hours in the classroom Students must have a current ACLS Provider card. This course includes core instructor instruction, ECC Guidelines, ACLS Instructor materials (instructor book, DVD s, stop watch and posters). ACLS REFRESHER TRAINING 1 DAY COURSE This is a one day renewal course. ACLS is based on simulated clinical scenarios with hands-on participation through learning stations where students will practice essential skills individually, as part of a team, and as a team leader. Realistic simulations reinforce the key concepts: proficiency in Basic Life Support care, recognizing and initiating early management of peri-arrest conditions, managing cardiac arrest, identifying and treating ischemic chest pain and acute coronary syndromes, recognizing other life threatening clinical situations (such as a stroke) and providing initial care, ACLS algorithms and effective resuscitation team dynamics. 16

18 BASIC ECG AND PHARMACOLOGY COURSE This class is an instructor-led, classroom based ECG and Pharmacology course. It is an essential resource for training healthcare professionals who participate in the management of cardiopulmonary arrest or other cardiovascular emergencies. This includes personnel in emergency response, emergency medicine, intensive care and critical care units, such as physicians, nurses, paramedics and others who require this specialized knowledge for their job. This course is a pre-requisite course for ACLS and for PALS. The scope of the class includes: Basic electrophysiology Normal ECG measurements Basic arrhythmias Administration of appropriate drugs within the ACLS algorithms This course contains two modules, ECG and Pharmacology, which may be taught separately or combined into one course. Course length is approximately 3.5 hours for the ECG component and approximately 2.75 hours for the Pharmacology module. Together, the course length is 6.25 hours. The course is intended for credentialed healthcare providers. 17

19 ECG- PHARMACOLOGY- ACLS- PALS COMBO CLASS This is a 5 day course series containing American Heart Association ECG/ Pharmacology courses, ACLS and PALS courses. The only stipulation in the course sequence is that ECG/ Pharmacology needs to be completed prior to ACLS. PALS INITIAL TRAINING 2 DAY COURSE The PALS course is to aid pediatric healthcare providers in developing the knowledge and skills necessary to efficiently and effectively manage critically ill infants and children. The skills taught include recognition and treatment of infant and children at risk for cardiopulmonary arrest; the systematic approach to pediatric assessment; effective respiratory management; defibrillation and synchronized cardioversion; intraosseous access and fluid bolus administration and effective resuscitation team dynamics. 18

20 PALS INSTRUCTOR COURSE This course instructs current PALS Provider card holders to become instructors. The course provides ECG Guidelines, Core Instructor course and all PALS Instructor materials. PALS REFRESHER TRAINING 1 DAY COURSE This is a one day refresher course only. The Refresher PALS course is to aid pediatric healthcare providers in developing the knowledge and skills necessary to efficiently and effectively manage critically ill infants and children. The skills taught include recognition and treatment of infants and children at risk for cardiopulmonary arrest; the systematic approach to pediatric assessment; effective respiratory management; defibrillation and synchronized cardioversion; intraosseous access and fluid bolus administration and effective resuscitation team dynamics. 21 ONLINE COURSES AND TRAINING These courses are available 24/ 7. AMERICAN HEART ASSOCIATION CLASSES ACUTE STROKE ONLINE This course is for all healthcare providers who need to recognize the signs and symptoms of stroke. The interactive course covers: Stroke chain of survival 7 D s of stroke survival Definition of ischemic and hemorrhagic stroke Pathophysiology Stroke risk factors, recognition, management transition to critical care and rehabilitation 20

22 HEART CODE ACLS PART 1 ONLINE This is a web-based. Self paced instructional program that allows students to assess and treat patients in virtual healthcare settings following the ACLS algorithms. This allows students to apply their knowledge to real-time decision making and skill development. Debriefing and coaching are provided immediately after each simulation to facilitate learning. The course is a convenient alternative to taking the ACLS course. The online course allows students to train at their own pace and schedule. Heartcode ACLS may be used for initial or renewal certification. Features of this course include: the importance of continuous, high quality CPR, team dynamics and communication, immediate post cardiac arrest care, airway management, related pharmacology. Students will have access to all information in the online course for 24 months following activation of the course key. AMERICAN HEART ASSOCIATION SKILLS PORTION OF E-LEARNING COURSE This course is required for the American Heart Association Heartcode ACL s E-Learning course. Students must have American Heart Association certificate of completion for online training present at the time of the skills session with a completion date within the last 60 21

23 days. Skill sessions will be scheduled upon completion of the online course. PALS HEARTCODE PART 1 Heartcode Part 1 is a self-directed interactive course intended for healthcare professionals who require a PALS Provider course completion card, as well as for nurses and emergency medical services personnel seeking continuing education credit. Students have access to the course for one year from initial license activation. PALS HEARTCODE SKILLS SESSION This course is required for the American Heart Association PALS Heartcode Part 1. It may be used for initial or renewal certification. An American Heart Association certificate of completion must be presented at the time of the skills session with a completion date within the last 60 days. Skills sessions will be scheduled upon completion of the online course. 22

24 AMERICAN HEART ASSOCIATION SKILLS PORTION FOR ONLINE COURSE Required skills sessions for all American Heart Association e-learning courses. Online e-learning courses include: BLS for Healthcare Provider Heartsaver First Aid Heartsaver CPR and AED Heartsaver First Aid with CPR and AED Students must have an American Heart Association certification of completion for online training present at the time of skills session with completion date within the last 60 days. BLS FOR HEALTHCARE PROVIDERS ONLINE PART 1 This web-based, self directed course uses case-based scenarios and interactive lessons to teach students about both single-rescuer and basic life support. This includes promptly recognizing several life threatening emergencies, provide high quality chest compressions, 23

25 delivering appropriate ventilations and providing early use of an AED. The course may be used for initial or renewal BLS for Healthcare Provider course completion. Students must complete all three (3) parts of the course: Part 1- online course. This includes a written exam after which students will receive a Part 1 certificate of completion, which must be presented for Parts 2 and Part 3. Part 2- Hands-on skills session with an authorized American Heart Association BLS instructor. Part 3- A skills test with an authorized American Heart Association BLS instructor. This course is designed for healthcare professionals and other personnel seeking an alternative method for completing an initial or renewal BLS for Healthcare Providers course. HEARTSAVER BLOODBORNE PATHOGENS This course is designed for persons with a chance for occupational exposure to blood or blood-containing material. 24

26 HEARTSAVER CPR AND AED ONLINE PART 1 This course teaches the basics in CPR and AED use. It is a two (2) hour on-line course and a 30 minute skills session at the training center. Skill session is sold separately. HEARTSAVER FIRST AID ONLINE PART 1 This course is designed for anyone who needs basic first aid training. It includes: First aid basics Medical emergencies (including adult chocking) Injury emergencies Environmental emergencies This course is 1 to 2 hours with a 30 minute skills session at the training center. Skills session sold separately. The certification is valid for 2 years. 25

27 HEARTSAVER FIRST AID WITH CPR AND AED ONLINE PART 1 This course is a comprehensive safety training for the workplace. It includes: First aid basics Medical emergencies (including adult choking) Adult CPR and AED use Injury emergencies Environmental emergencies Optional topics include child/infant CPR with AED, Child/infant choking. Duration of online course (2.5 hours); skills session at the training center (45 minutes). The skills session is sold separately. Certification is valid for 2 years. STROKE PREHOSPITAL CARE- ONLINE This course teaches about stroke pathophysiology, risk factors, differential diagnosis, recognition, assessment and management. CEH credit is given for EMTs and Paramedics. 26

29 ONLINE US DOT 24 HOUR EMT REFRESHER COURSE Approved EMT Refresher Program #20598 School Code # The National Standard EMT Basic Refresher course adheres to the 2011 EMT Basic Curriculum. The EMT is able to complete the required 24 hour refresher training online and at anytime. This course meets local and state requirements for California. The course qualifies for instructor based training. It also qualifies under code F3 for CECBEMS and NREMT. This course is not approved for Arizona Certified EMT-B. Check your state EMS Authority for any possible additional requirements. PARAMEDIC REFRESHER The National Standard Paramedic Refresher course adheres to the 2011 Paramedic Curriculum. The Paramedic is able to complete the required 48 hour refresher training online and at anytime. This course meets local and state requirements for California. The course qualifies for instructor based training. It also qualifies under code F3 for CECBEMS and NREMT. Check your State EMS Authority for any possible additional requirements. 28

30 EMT TRANSITION COURSE During the next 4 years everyone in EMS will be going through a transition from levels of certification designated in the 1990 s to new titles with new interventions and responsibilities. The focus of these changes were to increase the knowledge of future EMS providers. The NREMT is requiring this education as part of the continued certification process. All Emergency Medical Technician Basic (EMT-B) must take the transition course to transition to Emergency Medical Technician (EMT) in order to be eligible for National EMS Certification as an Emergency Technician (NREMT). If the transition class is not taken by the expiration dates of March 31, 2015 or 2016, the EMT-Basic will be issued National EMS Certification as an Emergency Medical Responder (NREMR). New guidelines may include: pulse oximetry, auto transport ventilator, humidified oxygen, partial rebreather mask, simple face mask, venturi mask, tracheostomy mask, mechanical CPR device, hemorrhage control (direct/ tourniquet), oral aspirin, assisting patients with self medication. 29

31 ALS/ BLS CONTINUING EDUCATION ONLINE Continuing education shall be in any topics contained in the United States Department of Transportation EMS Education Standards from All approved CE shall contain a written and/or skills competency based evaluation related to the course, class or activity objectives. 32 INSTITUTE POLICIES OPERATING SCHEDULE EMS Training Institute, Inc. is open Monday through Friday from 8:00am until 4:30pm and Saturdays 8:00am until 12:00pm. This schedule is in addition to scheduled course times as well. Staff members are available to answer questions and help with enrollment needs. STUDENT CONDUCT At the discretion of the school administration, a student may be dismissed from school for any of the following: -Possession of drugs or alcohol on school premises, or in a drugged or intoxicated state. - Possession of a weapon on school premises. - Behavior creating a safety hazard to other persons at school. -Dishonesty pertaining to documentation, examinations, quizzes or testing. -Disobedient or disrespectful behavior to other students or faculty members. 34

No text messaging of any kind. STUDENT DRESS CODE Initial EMT students are required to wear a complete uniform to class and on scheduled internships. Patches must be properly placed and uniforms should be clean and neat. ENTRANCE REQUIREMENTS There are no pre-entrance examinations in order to qualify for EMS Training Institute, Inc. courses. All students are required to have access to a computer with internet access, sound and a color monitor. For the Initial Emergency Medical Technician class, all students must be 17 ½ years old at the time of registration. 35

37 GRADING Completion of the Emergency Medical Technician (EMT) program at the EMS Training Institute, Inc. is subject to the following criteria: -Maintaining a 75% grade point average (GPA) 90%-100% A 80%- 89% B 70%- 79% C 60%- 69% D < 60% F -Completion of the course Final Exam with a minimum grade of 75%. -Active participation in practice sessions and successful completion of all Skills exams based on National Standards. -Compliance with all attendance requirements. ATTENDANCE Students are expected to attend classes as scheduled as they would be expected to be present during employment. Students must attend at least 150 of the 160 hours of classroom time during the didactic phase of training. There will be assigned make-up work for all classroom hours missed; make-up work will not be accepted or 36

38 counted as hours or class attendance. Instructional hours will need to be made up with attendance in another class. Tardiness is a disruption of the learning environment and is discouraged. Any student arriving to class more than 5 minutes tardy will be docked ¼ hour of class attendance. Leave of Absence- Written requests for leaves of absence will be considered and such leaves may be granted to students at the discretion of the school. For online courses, Learning Management System is available 24/7 from any internet access computer. All online courses are completed at the students convenience. DISMISSAL AND SUSPENSION If students do not maintain passing grades, students will be dismissed from the current course. If students do not maintain professional conduct, they will be dismissed from the coursework. TUITION All tuition for courses is due at the time of registration. Initial Emergency Medical Technician (EMT) registration can be paid in full or divided into 3 payments, which are all due by the first day of class. A $100 non refundable registration fee will be charged for the EMT class; a $25 non refundable fee for the EMT Refresher class. California residents are charged a $0.50 fee for the State Tuition Reimbursement Assessment (STRF). 37

39 The State of California created the Student Tuition Recovery Fund (STRF) to relieve or mitigate economic losses suffered by California residents who were students attending schools approved by, or registered to offer Short-term Career Training with the Bureau for Private Postsecondary and Vocational Education (Bureau). You may be eligible for STRF if you are a California resident, prepaid tuition, paid the STRF fee, and suffered an economic loss as a result of the following: - The school closed before the course of instruction was completed. - The school s failure to pay refunds or charges on behalf of a student to a third party for license fees or any other purpose, or to provide equipment or materials for which a charge was collected within 180 days before the closure of the school. - The school s failure to pay or reimburse load proceeds under a federally guaranteed student loan program as required by law or to pay or reimburse proceeds received by the school prior to closure in excess of tuition and 38

40 other costs. - The school s breach or anticipatory breach of the agreement for the course of instruction. - There was a decline in the quality of the course of instruction within 30 days before the school closed, or if the decline began earlier than 30 days prior to closure, a time period of decline determined by the Bureau. -The school committed fraud during the recruitment or enrollment or program participation of the student. You may also be eligible for STRF if you were a student that was unable to collect a court judgment rendered against the school for violation of the Private Postsecondary and Vocational Education Reform Act of You must pay the state imposed fee for the Student Tuition Recovery Fund (STRF) if all of the following apply to you: You are a student, who is a California resident and prepays all or part of your tuition either by cash, guaranteed student loans, or personal loans, and your total charges are not paid by any third-party 39

CATALOG DISCLOSURE- STUDENT TUITION RECOVERY FUND FORM 4 The Student Tuition Recovery Fund (STRF) was established by the Legislature to protect any California resident who attends a private postsecondary institution from losing money if you prepaid tuition and suffered a financial loss as a result of the school closing, failing to live up to its enrollment agreement, or refusing to pay a court judgment. To be eligible for STRF, you must be a California resident and reside in California at the time the enrollment agreement is signed or when you receive lessons at a California mailing address from an approved institution offering correspondence instruction. Students who are temporarily residing in California for the sole purpose of pursuing an education, specifically those who hold student visas, are not considered a California resident. To qualify 40

42 for STRF reimbursement, you must file a STRF application within one year of receiving notice from the Bureau that the school is closed. If you do not receive notice from the Bureau, you have four years from the date of closure to file a STRF application. If a judgment is obtained, you must file a STRF application within two years of the final judgment. It is important that you keep copies of the enrollment agreement, financial aid papers, receipts or any other information that documents the monies paid to the school. Entrance Requirements Entrance in our training program requires prospective students possess a high school diploma or its equivalent. Prospective students also need a current and valid Healthcare Provider CPR card per the California State EMS Regulations. The CPR card must have proof of inclusion of the ECC Guidelines 2010 information and be dated after April 1, Only one of the following cards will be accepted: - American Heart Association Healthcare Provider - American Red Cross CPR for the Professional Rescuer -American Safety and Health Institute CPR for the Professional Rescuer -EMS Safety Services CPR/ AED for the Professional Rescuer If the American Heart Association is chosen, the Training Solutions or EZCPR Training Center located in Woodland Hills, California, WILL NOT be accepted, nor will cards from any of their instructors. 43

If by the end of the probation period the student s grade is not raised to 75%, or overall attendance is not raised to at least 90% of scheduled hours, the Veteran Administration will be notified and benefits will be interrupted. Conditions for Reentrance If the director determines that the conditions, which the interruption, have been rectified, the student will be eligible to receive benefits. Conduct Policy Students shall at all times when on the school premises, conduct themselves in an orderly and considerate manner, and shall appear in a sober and receptive condition. Violation of this condition is a just cause for dismissal. 45

50 STUDENT S RIGHT TO CANCEL: The student has a right to cancel the enrollment agreement and obtain a refund. The institute shall, for all students, without penalty or obligation, refund 100% of the amount paid for the instructional charges, less a reasonable deposit or application fee not to exceed $100, if the notice of cancellation is made prior to or on the first day of instruction,or 7 days before class starts for American Heart Association classes. Further, the student has the right to withdraw from a course of instruction at any time. A student who has completed 60% or less of the course of instruction shall be granted a pro-rated refund of tuition. If the student withdraws from the program of instruction after instruction has begun, the student is obligated to pay only for educational services rendered, plus the non-refundable registration fee of $25. If the student has received federal student aid funds, the student is entitled to a refund of moneys not paid from federal student financial aid program funds.
