2 WOMEN AND HEART HEALTH Vanderbilt Heart is proud to offer free heart health screenings to women to help identify risk factors for heart disease. For your convenience, we offer free screenings every Friday from 9 a.m. - noon and 1 p.m. - 3 p.m. Or, you may call and make an appointment. Women are just as at risk for heart attacks, coronary artery disease and strokes as men. In fact, cardiovascular disease is the leading cause of death in both women and men. Our women s heart and vascular program focuses on prevention, diagnosis and treatment of heart disease. Heart health screenings help a woman and her physician know what steps need to be taken to best prevent heart disease and manage heart health. SIGNS OF A HEART ATTACK IN WOMEN The most common sign in women: Chest discomfort in the center of the chest that lasts a few minutes or more, or that goes away and comes back Other symptoms (may or may not include chest pain): Shortness of breath Back, arm, neck or jaw pain Nausea/vomiting Unexplained fatigue Breaking out in cold sweat Unusual lightheadedness If you have any of these symptoms, call your doctor immediately.

4 HEART SCREENING RESULTS The screening you took part in today has helped determine whether or not you have any risk factors for heart disease so that you can best manage your heart health. Thank you for participating. Diabetes, high cholesterol, high blood pressure, being overweight, smoking and physical inactivity are all risk factors for heart disease that you can control. If your test results are above ideal levels, we highly recommend that you consult your primary care physician or cardiologist to discuss your screening results. Please take this form to share with them. If you do not have a primary care physician or a cardiologist, you may request consultation with a cardiologist in one of our Nashville locations. We can also provide you with contact information for the community heart program of your choice. 5 Name: Date: BLOOD PRESSURE: There are several actions you can take to control, or possibly even prevent, high blood pressure. Because high blood pressure is a common disorder, we recommend that you have your blood pressure checked regularly. Please talk with your health care provider about how often to schedule a blood pressure check. Your blood pressure (mm Hg) results: Ideal Levels PreHypertension Hypertension Less than 120/80 Systolic: Greater than or equal to 140/90 Diastolic: CHOLESTEROL: According to the American Heart Association, knowing your total cholesterol level is an important first step in determining your risk for heart disease. In addition, knowing your individual blood cholesterol levels can give you insight into what health habits you may need to change to lower your risk of heart disease. Your HDL cholesterol is referred to as good cholesterol because higher levels of HDL are associated with a lower risk for heart disease. Regular aerobic exercise is the best way to raise the HDL level. Your LDL cholesterol is referred to as bad cholesterol because higher levels of LDL are associated with increased risk for heart disease. Exercise and a heart healthy diet can lower LDL cholesterol; however, depending on your LDL level, medication may also be recommended. Your cholesterol levels (mg/dl) results: Total HDL LDL Triglycerides Ideal Levels Borderline High At Risk Risk Total Cholesterol less than 200 HDL Cholesterol greater than 50 LDL Cholesterol less than Greater than 190 Triglycerides less than 150

6 BLOOD GLUCOSE (DIABETES SCREENING): Diabetes is a chronic disease that makes the body unable to use carbohydrates properly, causing it to rely too much on protein and fat for fuel. Cardiovascular risk in diabetic patients is equivalent to persons who are known to have coronary artery disease. Metabolic syndrome significantly increases the risk of heart disease and requires three of the following five criteria: abdominal obesity (defined by waist circumference), high triglycerides (greater than 150), low HDL (less than 40 for men, less than 50 for women), high blood pressure (greater than or equal to 130/85), and/or impaired fasting glucose (greater than or equal to 110). Your fasting blood glucose* (mg/dl) results: Ideal Levels At Risk for Diabetes/Pre-Diabetes Diabetes ( Impaired Fasting Glucose ) (Provisional Diagnosis) Less than Greater than or equal to 126 * Accurate results require fasting levels - no food or drink, except water or medications, in last 9-12 hours. WEIGHT MANAGEMENT: Waist Circumference (inches) Body Mass Index High Risk Women: Greater than 35 Normal Weight: Men: Greater than 40 Overweight: Obesity: 30 or Greater SMOKING HISTORY: Current Former Never Yes No Yes No Yes No EXERCISE: Current Former Never Yes No Yes No Yes No Thirty minutes or more of moderate intensity exercise (such as brisk walking), 4-5 days per week, is recommended. RECOMMENDATIONS ARE BASED ON: American Heart Association Evidence-Based Guidelines for Cardiovascular Disease Prevention in Women: 2007 Update; American Diabetes Association (2009) Guidelines; The Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure (JNC 7); National Cholesterol Education Program (NCEP) Third Report of the Expert Panel on Detection, Evaluation, and Treatment of High Blood Cholesterol in Adults (Adult Treatment Panel III).
