4 5 Rights of Delegation The right task Can this nurse be delegated in accordance with the Nurse Practice Act? Is the nurse allowed to delegate this task? Is this task within the nursing assistant s scope of practice? Is this task within the nursing assistant s job description?

5 5 Rights of Delegation The Right Circumstance Can this task be delegated based on the resident s current condition or need? Does the nursing assistant feel that the resident s condition will allow them to perform the task safely?

6 5 Rights of Delegation The Right Person Can this task be delegated to this particular nursing assistant? Do they have the training and preparation to perform this task? Will the nursing assistant be able to perform this task safely? Can the nursing assistant confidently perform the task?

7 5 Rights of Delegation The Right Direction Can this task be delegated by providing clear, concise directions, which can be easily followed by the nursing assistant? Does the nursing assistant understand the directions?

8 5 Rights of Delegation The right Supervision If this task is delegated, will the nurse be available if questions arise or if supervision is needed?

9 Keep In Mind... Nursing assistant should not refuse to perform a task that s within their scope Nursing assistant and delegating nurse should both be confident that the nursing assistant can perform the task Nursing assistant is responsible for knowing which skills are within their scope The nursing assistant has a responsibility to perform the task safely, to communicate with the nurse appropriately and to observe and report back to the nurse regarding the performance of the task and the resident's condition and response to the task

10 Nursing Assistant Student Are in training and not yet certified Must perform skills in lab prior to in clinical In clinical, you may only perform skills that you have proved to be proficient at Students are to work side by side with facility staff and/ or their instructor while performing skills Nursing tasks should not be delegated unless they re supervised or in an assistant role Hygiene, grooming, and feeding can be performed without assistance Students should not perform commonly delegated nursing skills

12 Relationship of Nurse Aide to Health Care Team Provide care according to the resident s comprehensive care plan. Actively listen and communicate with the resident, the family and the health care team. Observe and report any changes in the resident s appearance, behavior or mood to the nurse. 13 RELATIONSHIP OF THE NURSE AIDE TO RESIDENTS The role of the Certified Nurse Aide is to provide for the daily care, comfort, safety and health needs of the residents. The CNA is an important member of the health care team and the nursing team. Appropriate professional relationship. Inappropriate relationships.

14 RELATIONSHIP OF THE NURSE AIDE TO RESIDENTS To provide for the resident s daily care, the CNA assists the resident with: 1) Bathing, dressing and grooming 2) Eating 3) Elimination 4) Transferring and ambulating 5) Communication To provide for the resident s comfort, the CNA should: 1) Create a positive home-like environment 2) Help the resident develop trust 3) Offer emotional support and work with the family 4) Communicate and actively listen

15 RELATIONSHIP OF THE NURSE AIDE TO RESIDENTS To provide for the resident s safety, the CNA must: 1) Follow infection control practices 2) Maintain a clean, safe environment and report safety problems immediately 3) Perform procedures correctly To provide for the resident s health needs, the CNA should: 1) Observe and report any changes in the resident s appearance, behavior, or mood to the nurse 2) Determine measurements accurately (temperature, pulse, respirations, blood pressure, weight, intake, output and food consumption) 3) Assist the nurse during treatments, medical procedures and emergencies 4) Respond to any call light immediately.
