3 3 ASSOCIATE IN SCIENCE DEGREE IN NURSING TRADITIONAL GENERIC Florida Gateway College is accredited by the Commission on Colleges of the Southern Association of Colleges and Schools, 1866 Southern Lane, Decatur, Georgia , , to award the Associate Degree through The Florida Gateway College Associate in Science Degree in Nursing (ASDN) Program is approved on a continuing basis by the Florida Board of Nursing (1964), 4052 Bald Cypress Way, BIN C02, Tallahassee, FL , (850) The ASDN program is accredited through Spring 2017, by the National League for Nursing Accrediting Commission (1993), 3343 Peachtree Rd NE, Suite 500, Atlanta, GA 30326, (404) , The ASDN program is highly ranked in excellence as evidenced by the state board passage rate above state and national averages. The nursing program was established in The program has grown to include the following components: Traditional Generic, Traditional LPN-to-RN Bridge, and the LPN-to-RN Fast-Track Bridge. The experienced nursing faculty is well prepared both academically and clinically. The faculty is committed to providing a nursing curriculum that offers the student the opportunity to develop knowledge and skills that are necessary to promote better health care for all people in the community. Students receive individual guidance in a simulated lab along with small group supervision in clinical facilities. Upon successful completion of the program, the student receives an Associate in Science Degree in Nursing and is qualified to apply for the State Board of Nursing NCLEX examination. If successful on this exam, the graduate becomes a registered nurse (RN). Graduates are widely recruited and consistently demonstrate a strong clinical competence. Graduates of the program are qualified to enter a variety of careers such as staff nurses in acute care settings, home health agencies or long term care facilities. Many of the area hospitals are predominately staffed by these graduates. Advancement in nursing education is encouraged through referral to one of several universities RN-to-BSN programs. Florida Gateway College graduates have demonstrated consistent success with advancement through these programs. 4 4 TRADITIONAL GENERIC APPLICATION CHECKLIST Completion of this checklist alone does not guarantee acceptance to the Associate in Science Degree in Nursing Program. Completion of the checklist is the applicant s responsibility. Students should complete the checklist in order. 1. A. Complete a Florida Gateway College (FGC) Admission Application and return to the Admissions Office. B. Have official high school transcript, GED, or documentation of Home Schooling forwarded to Admissions Office at FGC. C. Provide scores for ACT or SAT standardized testing OR register for and take the CPT prior to advisement. (For help with testing, check with the College-wide Learning Lab for test reviews in Building 59 ( ) or go to the college website to Administrative Departments, click on Testing Department, click on Study Resources, and click on CPT. If deficiencies are found in Math, Reading, or Writing, the student will be required to complete college prep courses in the specific area before attempting most college courses. Test scores must be dated within two years of beginning coursework.) 2. Attend the next available Nursing Group Information Session after declaring nursing as a major. Sessions are held the first Monday of each month from 12:00 noon to 1:00 p.m. in the Medical Technology Building (103) 3. Complete the nursing entrance exam (RNET) available at the FGC Testing Center ( ). Exam scores must be submitted to the nursing department no later than March 1, Submit the Associate in Science Degree in Nursing Program supplemental application by March 1, Attach the following to your application: Two (2) letters of reference. Note: Career Pathway applicants need to submit one of the two required references from the high school Career Pathway instructor. Brief resume Unofficial copy of transcripts from colleges attended, including FGC Documentation of health related experience 5. For pre-requisites and/or co-requisites taken at a college other than FGC during the Spring 2014 term, the student must provide official transcripts to the Nursing Department and the Registrar s Office immediately following the end of Spring term The transcripts must be received no later than May 15, 2014, to receive credit for the courses completed in Spring term Acceptance letters will be mailed May 19 through June 5, Applicants are responsible for providing the above documentation if they wish to be considered for admission to the Nursing Program. Make copies of all documentation for your personal records. Failure to present proper documentation will result in non-acceptance into the program.

5 5 ADMISSION CRITERIA TRADITIONAL GENERIC SUMMER 2014 The following criteria will be required for all students admitted into the ASDN program: Complete all pre-requisite classes (BSC 2085; BSCL2085; ENC 1101; DEP 2004; MGF 1106 or MAC 1105 or a higher math course) with a minimum GPA of 2.5 in those classes and no grade below C; Maintain a minimum GPA of 2.5 in all pre-requisite classes and any completed corequisite classes (BSC 2086; BSCL2086; MCB 2010; MCBL 2010; SPC 2608; HUN 1201; PHI 2600, HUM 2020, HUM 2210, HUM 2230, HUM 2310, HUM 2472, HUM 2511,or HUM 2551) with no grade below C; Score of 75 or higher on the RNET admission test. Scores must be submitted to the nursing department no later than March 1, Guaranteed Admission Any applicant who has earned a minimum grade of B+ in all pre-requisite classes and any corequisite classes taken will be guaranteed admission to the program, providing all of the above requirements are met. All pre- and co-requisite classes must be taken at Florida Gateway College to qualify under this criterion. Career Pathway Admission A number of positions will be reserved for Career Pathway students, under a Career Pathway agreement among FGC and the high schools in the service area. To be eligible for one of these positions, a Career Pathway student must (1) attend FGC for all pre- and co-requisite courses, (2) have a minimum 2.75 GPA in all pre-requisite and any co-requisite classes taken, and (3) apply to the Nursing Program within two years of high school graduation. In addition, Career Pathway applicants need to submit a letter of reference from the high school Career Pathway instructor. Other Admission Students who do not qualify under the Guaranteed Admission or Career Pathway Admission criteria will be admitted if space is available, with places being awarded to those individuals with the highest points, using the ASDN Point System for Competitive Admission. 8 8 PRE- & CO-REQUISITE COURSES FOR THE TRADITIONAL GENERIC PRE-REQUISITE COURSES The Associate in Science Degree in Nursing is a limited access program, which includes 30 hours of general education courses and 42 hours of nursing courses. The following 13 semester hours of courses must be completed prior to admission into the Nursing Program. The pre-requisite courses are listed below. Students who do not complete the co-requisite courses within the specified time frame with a C or better will not be permitted to continue in the nursing program. Prerequisite and co-requisite courses completed by the end of the spring term in which you are applying are included in the required 2.5 GPA for admission to the program. For credit toward admission to the program, Anatomy and Physiology 1 & 2 coursework must be completed with a C or higher within the previous five (5) years. If coursework was completed more than five years ago, you must pass the college required competency exam no later than March 1, 2014 OR you may successfully complete Essentials of Anatomy and Physiology 1 (BSC 2084) prior to admission. For credit toward admissions to the program, Microbiology coursework must be completed with a C or higher within the previous five (5) years. If coursework was completed more than five years ago, you must pass the college required competency exam no later than March 1, All science courses must have a lab. Permission may be granted by the Vice President for Occupational Programs to repeat a microbiology course that was completed more than five years ago provided the coursework was completed with a grade of C or higher. Any repeat course must be completed with a grade of C or higher.

10 10 Nursing Program applicants should: NURSING CORE PERFORMANCE STANDARDS 1. Possess sufficient physical, motor, intellectual, emotional, and social/communication skills to provide for patient care and safety, and the utilization of equipment in a variety of health care settings. Examples of performance standards for Nursing are outlined below. 2. Tour an appropriate health care facility, career shadow and/or otherwise ascertain if she/he can perform the professional duties of their chosen profession without becoming injurious to themselves or the patient. ISSUE STANDARD EXAMPLES Critical Thinking Critical thinking ability sufficient for clinical judgment. Interpersonal Interpersonal abilities sufficient to interact with individuals, families, and groups from a variety of social, emotional, cultural, and intellectual backgrounds. Communication Communication abilities sufficient for interaction with others in verbal and written form. Mobility Motor Skills Hearing Visual Tactile Physical abilities sufficient to move from room to room, through hallways, and maneuver in small spaces. Includes the strength necessary to lift patients as needed. Gross and fine motor abilities sufficient to provide safe and effective nursing care. Auditory ability sufficient to monitor and assess health needs. Visual ability sufficient for observation and assessment necessary in nursing care. Tactile ability sufficient for physical assessment and to provide nursing intervention. Complete competent assessment in a timely manner, make correct interpretations, devise a plan of care, and implement appropriate interventions. Progression during program to reflect independent decision with appropriate supervision. Be able to cope with life and death situations and deal with families stressed by their loved one s critical condition while working with other health care providers. Follow verbal and/or written instructions. Must communicate patient response to therapy with others, document therapeutic procedures performed on patient, consult with other health care providers in a professional manner. Move to and from departments to patient rooms, from room to room to take care of all patients on a team, and assist in patient transport. Perform vital signs, CPR, patient transport, physical assessment, equipment utilization. Auscultate BP, breath sounds, heart sounds, and bowel sounds. Hear alarms in units, call bells, and telephones. Must converse with patients, family, and staff. Read patient charts/flow sheet/monitors, draw up and administer medications, assess patient skin color, and read thermometers and other equipment. Perform palpation, give injections, start IV s, do sterile and non-sterile dressing changes, urinary catheterization, and assist patients with daily activities. Latex Advisory: The use of latex/latex based products may exist in health care universal precautions and in environments such as, but not limited to, Health Sciences classrooms and training labs, hospitals, nursing care facilities, laboratories, clinical areas, and medical/dental offices. Individuals with latex allergies should seek expert advice from their health care provider so that they may receive information to make an informed decision regarding their exposure to latex in the health care field.

11 11 TRADITIONAL GENERIC APPLICANTS NURSING ENTRANCE TEST (RNET) 1. The RNET is the HESI entrance exam. All applicants must take this exam. A score of 75 or higher is required. Scores must be submitted to the nursing department no later than March 1, The HESI is a computerized examination that is administered electronically. It consists of six exams that are academically oriented: Math, Reading, Comprehension, Grammar, Vocabulary, Anatomy and Physiology, and Science. Results from this test are good for 2 years. The HESI cumulative score will be used for the RNET score in the scoring grid. Please see Pages 6 and 7 for calculation of points on the Point System for Competitive Entrance in the ASDN Program. 2. The RNET exam is offered according to the FGC Testing Center. The schedule is available on the FGC website. The testing center is closed on holidays and weekends, as determined by the FGC academic calendar. Call the Testing Center at for assistance. 3. The RNET is administered on the computer. Complete instructions will be given just prior to administration of the examination. You will receive an explanation of your score following the test. (If you have a disability you may request special accommodations. You must request this at the time you sign up for the test. 4. The cost of the test is $ You may pay for the test with a credit card, cash or money order made out to FGC. Payment should be made at the cashier s office prior to the time of the test administration. Bring only your receipt and photo ID to the testing center.

12 12 FOLLOWING ACCEPTANCE INTO PROGRAM Acceptance letters will be mailed between May 19 and June 5, Acceptance information cannot be provided over the phone. Please do not call to request this information. FOLLOWING ACCEPTANCE INTO THE PROGRAM, THE STUDENT MUST: 1. Complete a basic CPR (Cardio-Pulmonary Resuscitation) course and submit a copy of certificate by August 17, 2014 the last day of open registration as determined by the FGC critical dates calendar. This must be a Basic Life Support for Health Care Providers (American Heart Association) or Professional Rescuer for American Red Cross (ARC). Contact Community Education at for available classes. You must have a valid card, throughout the nursing program. 2. Have a current PPD and physical examination verifying acceptable health status. Appropriate boosters must be completed. Proof of Immunity must be submitted from official documentation only. Official shot records from your doctor, the health department, or public school are acceptable. If official documentation is not available, then blood titers are necessary to determine immunity for hepatitis B, varicella (chickenpox), mumps, rubella, and rubeola (measles). Vaccinations may be necessary if immunity cannot be confirmed with the blood titer. All health records (physical exam, immunizations, PPD) are due by dates provided by program coordinator. 3. A current flu vaccination is required of all nursing students. Timeframe for obtaining this vaccination will be provided by program coordinator. 4. Provide a copy of his/her health insurance card by first day of class (fall semester). Student must be covered by personal health insurance following acceptance and throughout the program. 5. Attend Nursing Student Orientation on the specified date.* 6. Be free of offenses as listed in Florida Statutes S as verified by an in-depth background screen. Information on the background screen will be mailed in your acceptance letter. Students are responsible for the cost.* 67 Establish being drug free by testing negative on urinalysis for controlled substances of amphetamines, cannabinoids, cocaine, opiates and PCP (5-panel drug screen). Information on the Drug Test will be included in your acceptance letter. Students are responsible for the cost.* *This information will be included with your acceptance letter. Students who fail to provide required documentation by the due date will have their admission rescinded. Please make a copy of all records prior to turning them in to the Nursing Department. You will need them after you graduate for your job. You are responsible for maintaining these records. Copies will not be available following graduation from the program.

13 13 PHYSICAL HEALTH, DRUG SCREEN AND BACKGROUND SCREEN After acceptance into the program, all screening must be completed by date specified by program coordinator. Final acceptance into the program is contingent upon satisfactory results from your physical examination, drug screen, and in-depth background screen. The drug and background screen must be completed by an agency as designated by the Director of Nursing Programs. Information on obtaining these tests/reports will be provided in the provisional acceptance notification package. These reports should not be submitted with initial application materials. You must agree to authorize FGC to share the results of the background screen with any clinical site to which you may be assigned, if required by that site. Student Drug Screen and Background Check When submitted, these records will become the property of the College, and will not be available for copying or for use to meet the requirements of outside employers. Drug Screening Area health care agencies are drug free places of employment. All health care agencies require a negative drug screen before hiring a new employee. The Florida Gateway College Allied Health Division has been asked by area health care affiliates to require a drug screen for program applicants who will be placed in area health care agencies for clinical rotations. Program applicants are required to obtain a substance abuse drug screen and are subject to random drug screens upon admission to the program. These results are important for clinical rotations, and subsequent completion of the Nursing Program. Applicants with a positive result on the drug screen will be given a reasonable opportunity to explain the confirmed positive test result. If the explanation is unacceptable and/or cannot be satisfactorily documented by the applicant s physician or the Medical Review Office (MRO), the applicant will not be accepted into the program and, if enrolled, shall be dismissed from the program. Drug screen results will be kept in a confidential file. The applicant may reapply and be considered for application to a Health Science program after a positive reference from a treatment team who has followed the applicant for at least two (2) years, and has dealt specifically with the applicant s problem of chemical dependence. Results from the drug screen are returned to the Director of Nursing Programs and will be kept in a strictly confidential file, separate from the student s record. No drug screen test results are to be placed in individual student files. In-depth Background Screen Allied Health students at Florida Gateway College involved in direct patient care during a clinical rotation in a hospital or related medical facility are required to complete an in-depth background screen. Information will be included in your acceptance letter. This is a requirement mandated under the guidelines cited in the current Florida Statutes: Chapter 435, and by health care agencies with which FGC has clinical, internship and externship agreements. Information will be included in your acceptance letter. Any student who has been arrested regardless of adjudication, plea of nolo contendere, or pleas of guilty to, any offense under the provisions of Florida Statutes or under a similar statute of another jurisdiction may be disqualified from admission. According to the Florida Board of Nursing Policy, students must self disclose any charges and/or arrest information to the Director of Nursing Programs prior to admission to the program. For more information, contact the Allied Health Department at The Florida Board of Nursing requires additional criminal checks on all applicants for licensure and any nursing licensure applicant who has an arrest record to present those records. The Florida Board of Nursing is the state agency authorized to issue nursing licenses. As part of the licensure application process, graduates will be required to submit a signed statement verifying compliance with the Agency for Health Care Administration Florida Board of Nursing Rule 64 B which states: Prior to application for examination, convicted felons must obtain a restoration of their civil rights or they are ineligible to sit for the examination. If an applicant has been convicted of any offense other than minor traffic violation, the applicant shall submit arrest and court BACKGROUND records stating CHECKS the nature of the offense and final disposition of the case so that a determination can be made by the Board whether the offense relates to the practice of nursing or the ability to practice nursing. Any convicted felons must submit Last Updated proof April of 18, restoration 2013 BB of civil rights or they are ineligible for licensure.

14 14 Applicants for Allied Health Programs must be free of offenses that would disqualify one for employment as a Certified Nursing Assistant (CNA) in a nursing home (Florida Statutes s ). This policy is the result of requirements by clinical affiliates which must comply with state and federal regulations. Judgments of guilty or pleas of nolo contendere (no contest) to the following crimes may disqualify applicants from entering into clinical rotations associated with a Florida Gateway College Allied Health Program: a. Murder b. Manslaughter c. Vehicular homicide d. Killing of an unborn child by injury to the mother e. Assault, if the victim of the offense was a minor f. Aggravated assault g. Battery, if the victim of the offense was a minor h. Aggravated battery i. Kidnapping j. False imprisonment k. Sexual battery l. Prohibited acts of persons in familial or custody authority m. Prostitution n. Lewd and lascivious behavior o. Lewdness and indecent exposure p. Arson q. Theft, robbery, and related crimes, if the offense is a felony r. Fraudulent sale of controlled substances, only if the offense was a felony s. Incest t. Abuse or neglect of a disabled adult or elderly person u. Exploitation of disabled adult or elderly person v. Aggravated child abuse w. Child abuse x. Negligent treatment of children y. Sexual performance by a child z. Obscene literature aa. While you are enrolled in the Nursing Program it is recommended that employment be on a part-time basis, no more than 20 hours per week.
