2 St. Martin s in the Pines Our mission at St. Martin s in the Pines is to serve God through a ministry that provides a continuum of care and quality of life to aging individuals and their families. Our vision is to be a caring community reflecting God s love through service to the aging. Fifty years ago, the Rev. David Cady Wright, rector of St. Mary s-on-the- Highlands Episcopal Church, was concerned that many of his parishioners would not have a dignified, comfortable retirement. It was Rev. Wright s pastoral concern that gave St. Martin s its beginning. This commitment has been one reason for our continued growth. In 1957, St. Martin s began as a retirement residence for 80 residents. Today, we re a comprehensive retirement community, with a nursing home, an assistedliving facility, a secure Alzheimer s community (Evergreen at St. Martin s), and a four-story retirement apartment building. But as we ve grown, one thing remains constant: our mission to meet the physical, social and spiritual needs of each resident. Because St. Martin s physical and spiritual growth is still guided by the Episcopal Foundation of Jefferson County, our residents and their families can count on a sense of comfort, security and personal worth. Although the religious orientation of St. Martin s is Episcopalian, residents of all denominations are welcome. We invite you to read the information in this packet and take a tour of our retirement community. You ll quickly agree that St. Martin s is more than a place to live. It s a place to begin life anew.

3 Five distinct levels of care. One place to call home. The Arbors We offer independent-living residents a variety of apartments from which to choose, including one and two bedrooms along with large studios and suites. All of our apartments have full kitchens, balconies and an emergency call system. One meal a day is provided, and residents are invited to entertain family and friends in our lovely private dining room. Woodside Place St. Martin s assisted-living facility, Woodside Place, gives residents the feeling of home while providing 24-hour assistance with dressing, bathing and other personal-care needs. Our comprehensive wellness program includes medication administration, three nutritious meals served daily and programs ranging from the recreational to the spiritual. The Pines Nursing & Rehab Center Our skilled nursing facility, The Pines Nursing & Rehab Center, provides care 24 hours a day under the supervision of a professional management team and a highly qualified licensed nursing staff. Living in rooms that preserve a homelike atmosphere, residents are encouraged to reach their maximum physical potential through St. Martin s full-time on-site therapy staff including physical, occupational and speech. Evergreen Evergreen at St. Martin s promotes the safety and well-being of persons in all stages of Alzheimer s disease or related memory disorders. Receiving extra supervision and assistance from our specially trained Evergreen staff, elders experience a nurturing and secure setting while also having opportunities to participate in individual and group activities that encourage mental and physical stimulation. The Cottages Our unique and revolutionary care model for those requiring long-term skilled nursing, The Cottages are the first residential-style nursing homes to be built in Alabama and part of a national movement known as THE GREEN HOUSE Project. Each house is served by a certified nursing assistant and a clinical support team of nurses, medical directors, social workers, physical therapists and others.

4 The Arbors Rental Plan Options & Fees The Arbors offers a selection of studios, one and two bedrooms, and spacious deluxe one bedrooms and suites. All apartments have a full kitchen and private balcony, and they may be decorated to individual tastes. With both plans, a non-refundable application fee is required together with a physician s report and completed application form for each person. For fees and pricing information, please call and speak to a St. Martin s representative. The Arbors offers a second option, where residents may choose to move into St. Martin s apartments through an admission plan that requires an initial apartment payment fee, which serves to substantially reduce the monthly occupancy fee. A percentage of the apartment fee will be refunded when the resident vacates the apartment. The refund amount varies based on the years of residency. The precise terms of this option are included in a separate agreement, which will be provided upon request. The plan is designed to be comparable to rental rates for residents who Additional services available at extra charge: A Medication Reminder Program is available for residents that require support with medication management. Respite (short-term) stay is offered based on availability with a minimum four-day stay. Furniture is provided with the respite option. A non-refundable Pet Fee is assessed for pets that meet community guidelines.

7 The Pines Nursing & Rehab Center Services & Daily Fees The Pines Nursing & Rehab Center is a skilled care facility of 138 beds licensed by the State of Alabama. The rooms are spacious and attractive, preserving a homelike atmosphere for residents. Care is provided 24 hours a day under the supervision of a professional management team and a highly qualified licensed nursing staff. To help residents reach their maximum physical potential, St. Martin s has a full-time, on-site therapy staff, which includes physical, occupational and speech therapy. Ancillary medical services such as podiatry, dentistry, optometry and geriatric psychiatry are also available. Services provided by the Social Work, Pastoral Care, and Activities departments ensure that the social, spiritual and recreational needs of the residents are met. Located within the nursing home is our 20-bed Evergreen Alzheimer s Program, which was established to offer a highly specialized program in a secure environment for residents in the middle to late stages of Alzheimer s disease and other related memory disorders. Our program includes a custom-designed community with dining, living and activity areas. All staff receive specialized Alzheimer s training in communication and providing care to memory-impaired residents. For those residents who are diagnosed with a terminal illness, St. Martin s offers complete hospice care in the comfort of our Rose Suite. The Rose Suite is completely furnished to provide peace and privacy for the resident, family and visitors. The resident will receive the full range of nursing care, including pain management, as prescribed by the attending physician. Accommodations for overnight stays by the family are available. The admission process requires a nursing assessment conducted by the Director of Nursing and recommendations from a personal attending physician. An Admissions Coordinator, who will assist families in the completion of all necessary forms, is available. St. Martin s accepts private pay, Medicare and Medicaid.

8 Woodside Place Monthly Fees Our assisted-living facility is licensed by the State of Alabama to provide assistance with the activities of daily living for persons who do not require 24-hour nursing care. Residents experience a feeling of home in the comfortable rooms decorated with their own furniture. Three nutritious meals per day are served in the attractive dining room. Our comprehensive wellness program includes medications administered by licensed nurses. A variety of recreational, social and spiritual programs are led by trained staff and held in common areas. Monthly fees are based on individual resident assessments. The care level required will be determined by a Service Assessment completed by the Administrator and a medical team prior to the resident s admission. The submission of an application, including a medical history and examination signed by the applicant s personal physician, and the results of a chest x-ray are required. Areas to be assessed include bathing, dressing, grooming, toileting, feeding, ambulation and prompting for medication. The resident s care level will change as necessary to accommodate changes in the resident s condition. For fees and pricing information, please call and speak to a St. Martin s representative. Additional services available at extra charge: The Medication Service Program includes management, ordering and administration of all medications performed by a professional licensed nurse. This service includes the administration of daily injections, eye drops, breathing treatments and diabetes management. Unfortunately, we cannot order medications from any source other than the pharmacy with which we have a contractual arrangement. Medications from the Veterans Affairs and other mail order programs cannot be ordered by our staff. Respite (Short-Term) Care is offered, based on availability, with a minimum four-day stay. Furniture is provided with the respite option. A wireless emergency pendant is available for a nominal monthly fee. A non-refundable Pet Fee is assessed for pets that meet community guidelines.

10 Comprehensive Alzheimer s Care Evergreen at St. Martin s is designed to promote the safety and well-being of persons with early to middle stage Alzheimer s disease or related memory disorders that do not require skilled nursing care. Residents living in this secure, nurturing setting receive extra supervision and assistance with personal care needs. All medications are administered by a professional licensed nurse. Specialized individual and group activities are also provided to encourage mental and physical stimulation. The Evergreen staff, which includes a fulltime registered nurse, medical social workers, a consultant geriatric psychiatrist, and resident assistants, is specifically trained in the care of Alzheimer s residents. Acceptance into Evergreen is based on the degree of care deemed necessary for the applicant, which will be determined by a Service Assessment completed by the Administrator and a medical team prior to each resident s admission. Areas that will be assessed include bathing, dressing, grooming, toileting, feeding, ambulation, and the supervision and administering of medication. The submission of an application must also include a medical history and examination signed by the applicant s personal physician and the results of a chest x-ray. 12 It s Simply Home With an ongoing commitment to the enhancement of quality of life for elders, St. Martin s in the Pines has launched a unique and revolutionary care model for those requiring long-term skilled nursing. The Cottages at St. Martin s is the first residential-style nursing home to be built in Alabama and part of a national movement known as the GREEN HOUSE Project. As a pioneer in the state, St. Martin s serves as a beacon of hope for a culture change in long-term care. The GREEN HOUSE model creates a small, intimate community for a group of ten elders. A radical departure from traditional skilled nursing homes, the GREEN HOUSE home alters size, interior design, staffing patterns, and methods of delivering skilled nursing services. Its primary purpose is to serve as a place where elders can receive assistance and support with activities of daily living as well as clinical care, but without the assistance and care becoming the focus of their existence. The physical transformation is only one element of the GREEN HOUSE model. Staff responsibilities are dramatically different from those in a conventional nursing home. The traditional certified nursing assistant receives a new role of a universal care worker called a Shahbaz. Each is given additional training in ways to protect, sustain, and nurture the elders living in The Cottages and to take on a broader array of responsibilities, including the facilitation of meaningful activities, cooking, light housekeeping and laundry. Each house is also served by a clinical support team of nurses, medical directors, social workers, physical therapists and others. These professionals are not stationed within the houses, but visit according to a schedule, much like a home health model. Please contact the Admissions Director for Skilled Nursing Care to obtain admission information. The Cottages accept private pay, Medicare and Medicaid.
