1 Healthy Workplace Award Nomination Packet Every year, Michigan worksites large and small take that extra step, spend the extra dollar, develop that innovative concept which helps create a healthier workplace and helps make their employees healthy, fit and more productive. To honor that commitment, the Governor s Council on Physical Fitness, Health and Sports and the Michigan Fitness Foundation bestows the Healthy Workplace Award on organizations with outstanding programs, and help to create best practices for organizations throughout Michigan to replicate. Organizations honored in the Healthy Workplace Award category will be evaluated using the following criteria: Policies Procedures Resources Utilized (physical, financial, incentives, etc.) Executive Leadership Involvement Culture Evaluation and Results Influence on Family/Community Organizations nominated for these awards must have shown outstanding commitment to the health of their employees by establishing creative worksite health promotion programs with demonstrated success. Worksites can apply for recognition at three levels: Silver, Gold, and Diamond. Any business meeting the criteria can receive an award at the first three levels. However, of the organizations that apply for the Diamond level, the Governor s Fitness Award will be bestowed on one organization from each of four levels: very large (more than 1,000 employees), large ( employees), medium business ( employees), and small business (less than 50 employees). Category Very Large (1000+ employees) Silver Gold Diamond Governor s Fitness Award 3 chosen from Diamond level to be interviewed, one chosen as winner Large ( employees) 3 chosen from Diamond level to be interviewed, one chosen as winner Medium ( employees) 3 chosen from Diamond level to be interviewed, one chosen as winner Small (less than 50 employees) Unlimited # of Unlimited # of Unlimited # of 3 chosen from Diamond level to be interviewed, one chosen as winner Recognition Invited to both GFA events, recognized, but not presented Invited to both GFA events, recognized, but not presented Invited to both GFA events, recognized, but not presented Award winner honored on stage at both GFA events.
2 The Application Process Will include general information about the organization, a set of questions designed to give applicants an opportunity to talk in detail about their programs, and a set of categories that employers programs may fall within that will help in determining into which award level the organization fits. There will also be a section that will allow applicants to explain their commitment to health and fitness. This can describe programs, management support, incentives, success, goals and overall involvement. Finally, applicants will be asked how they measure results and what data they might collect in their wellness initiatives. A panel of workplace wellness experts will evaluate the applications and may call or conduct interviews with an applicant s wellness committee to determine Governor s Fitness Award winners in each category. Each awardee will receive recognition at the Governor s Fitness Awards, and will also receive a commemorative logo to be used through social media and their website to promote their award. 3 Award LevelsSilver: Recognizes workplaces that have begun program(s) providing health and wellness opportunities for employees. Silver award recipients must have implemented their program within the past two (2) years. Gold: Recognizes workplaces that have sustained participation or comprehensive program(s) providing health and fitness opportunities for employees and are clearly aligned with the organization s priorities. Gold award recipients must have had programs in place for 2-4 years, and have had at least 20 percent employee participation in one wellness activity during the past year. Diamond: Recognizes workplaces that have sustained comprehensive programs providing health and fitness opportunities for employees, and are strategic and integral parts of the organization. Diamond award recipients must have had programs in place for at least five (5) years, provide an incentive to employees to participate in wellness activities, and have had at least 40 percent employee participation in at least two wellness activities during the past year. Governor s Fitness Award: Recognizes workplaces that are excelling at the highest level in providing health and fitness opportunities for employees, and are fully integrated into the organization s mission, vision and goals moving forward. Governor s Fitness Awards will be presented to one very large employer (more than 1,000 employees), one large employer ( employees), one medium employer ( employees), and one small employer (less than 50 employees). To be eligible, organizations must have had programs in place for at least five years, provide an incentive to employees to participate in wellness activities, have had at least 75 percent employee participation in at least three wellness activities in the past year, sponsor and/or have had employee participation in an event promoting community wellness, and have family and/or community participation in wellness activities. Up to three organizations in each category can be selected from the Diamond Award winners to complete an interview process in order to be designated a Governor s Fitness Award winner.
4 SELF- ASSESSMENT SURVEY This survey is intended to compile basic information about each organization s workplace wellness programs and is not intended to get into be a deep dive into each applicant s ability to put together effective wellness programs. Organizations of all sizes have different resources available for use in these settings, and the goal of this award category is to recognize innovative and effective programs from organizations with diverse backgrounds and resources. For organizations selected to compete for the Governor s Fitness Award in each size category, a more in- depth interview will be conducted to evaluate programs. Please answer these questions to the best of your ability, and feel free to use additional paper for your answers. 5 The following questions WILL NOT be used in determining which category an organization might fall into, but rather be used to tell a collective story of what organizations in Michigan are doing to impact the health of their employees. 6 Please fill out the following section with as much detail as possible. You are welcome to answer the questions on a separate sheet of paper should you need more space. What are the mission, goals and objectives related to the wellness program your organization is currently implementing? Why did you implement a wellness program and why are you applying for this award? How did you integrate your wellness programs into your mission? What goals have you set for the program and have you branded your wellness initiative in any unique ways to engage your employees?
7 Please describe the wellness programs and policies you have implemented for your employees (this can include but is not limited to incentive programs, classes, resources, health coaching, etc.). Please describe the input and buy- in from Executive Management surrounding your wellness programs, including specific examples of senior leadership engagement and support for the program. How has the culture of your organization been affected by your wellness initiatives? How are employees engaged in the culture of health promoted by these programs?
8 Please describe how you promote wellness events and opportunities to your employees. Does your company utilize incentives or other recognitions (i.e. announcements, awards, etc.) in your wellness programs? If so, please describe the incentives and the outcomes those incentives have achieved. Describe any innovative or unique programs that your organization has implemented as a part of the wellness initiatives you have undergone. What changes to the physical environment have been made at your organization to create a culture of health and wellness?
9 Did your organization have to overcome any obstacles in implementing your wellness programs? If so, please describe. How did you evaluate the programs and what results have you seen from those evaluations of your wellness programs? Please provide specific metrics and outcomes. Does your program currently track Return on Investment (ROI)? If so, how? Are you currently seeing a positive return on investment for your wellness program? Describe the critical benefits your organization has achieved through your wellness programs.
10 Describe any impactful or inspirational stories that have come out of your wellness programs for your employees. Did you achieve the goals you had set out for your programs, and what is your next step in creating a healthier workforce? How have your programs internally made an impact on families? Have your programs extended out into the community to engage others to lead healthier lives? If so, give specific examples.
