1 Call for Application For The Edith P. Vincent Healthy Schools Nutrition Award Deadline for Submission: April 30, 2011 Nemours Health & Prevention Services (NHPS) in partnership with the Delaware Department of Education (DDOE), Delaware School Boards Association (DSBA), the School Health Advisory Council (SHAC), the Delaware Charter School Network, the Delaware Association of School Administrators (DASA) and Delaware Coalition for Healthy Eating & Active Living (DE HEAL) is pleased to announce a call for nominations for the 2011 Edith P. Vincent Healthy Schools Award. The awards will recognize the work of public schools championing children s health in the area of nutrition. Award recipients will consist of those public schools that have demonstrated effective implementation of best and/or creative and visionary programs that address policy and changes related to improved nutrition/healthy eating in the school setting in an effort to combat childhood obesity. Award Description The Edith P. Vincent Healthy Schools Award will be given to public school that has displayed success in the implementation of programs that address policy and change in the areas of improved nutrition/healthy eating. Each winning school will receive a commemorative plaque recognizing its achievement at the Delaware School Boards Association Fall Clinic held annually in October, a banner suitable for public display and formal recognition at their local Board of Education meeting. Award Eligibility All nominees must be a public school within the State of Delaware and focus on policy and changes related to healthy eating in an effort to combat childhood obesity. The awards are intended for Delaware public and charter schools that: Demonstrate child health promotion as an integral part of their infrastructure; Address policy and changes related to nutrition/healthy eating activity in an effort to combat childhood obesity; That promote and implement recognized best s and/or innovative, visionary approaches; Are replicable and sustainable. Award Criteria Program Design (worth 20% of the score): o Goals and Objectives (10%): Clearly articulates the goals and objectives of the program. o Evidenced-based (10%): The extent to which the program design is based on the best available evidence. Effectiveness (worth 70% of score): o Evaluation criteria (20%): Describe the evaluation criteria of the program and how the program objectives are evaluated.

2 o Impact (20%): Describe the impact (or potential impact) of the program and its success (or progress) in meeting its goals and objectives. Describe the program s overall impact (or potential impact) on helping the children it serves through the provision of a healthy environment in the school setting. o Replication (15%): Describe the extent to which the program can be successfully replicated and implemented in other schools and/or districts. o Sustainability (15%): Describe the potential of the program to be sustained over time. Completion of the Scoring Rubric (worth 10% of score) How to Apply for the Edith P. Vincent Healthy Schools Award To apply, simply identify a program that fits the criteria outlined above; follow the instructions outlined on the Edith P. Vincent Award Application Form. All application materials must be postmarked by April 30, Note that there may be additional information requested after the submission. Awards Program Process Upon submission, all applications will be reviewed by the Edith P. Vincent Healthy Schools Award Committee. The Awards Committee will be comprised of representatives from the following sponsoring agencies: Nemours Health & Prevention Services, the Delaware Department of Education, Delaware School Boards Association, the School Health Advisory Council, the Delaware Charter School Network, the Delaware Association of School Administrators, and Delaware Coalition for Healthy Eating & Active Living. The Committee will review and score all nominations based on the award criteria outlined above. Once the scoring has been completed, a pool of the highest scoring finalist will be identified and on site evaluations of finalists will be conducted. Winners will be notified in writing by June 1, 2011, with formal recognition taking place at the recipient district or schools July - September 2011 Board meeting and the awards ceremony will take place during the 2011 Delaware School Board Association Fall Clinic in October 2011.

5 Initiating Partial implementation Full implemention *Note: Documentation will only be requested from Finalist schools - Do not submit any documentation until requested Elementary schools limit the sale of individual beverages during of school hours to 100%. fruit/vegetable juices with no added sweeteners (4oz), 1% or fat free milk (unflavored/flavored) and water. Middle/High Schools offer only beverages approved by the food service program and limited to: Water or flavored water, 100% fruit and vegetable juices with no added sweeteners (up to 4 oz. for middle and up to 8 oz. for high school), 1% or fat free milk (unflavored/flavored) (up to 8 oz.) and nutritionally-equivalent and nondairy beverages including lactose-free and soy beverages. Primary snacks offered are fruits and vegetables. Primary beverage is water or 1% or fat free milk. Fundraising activities that involve food/beverages are limited to items that meet the adopted food standards are permitted. Schools encourage fundraising activities that promote non-food items and/or physical activity Schools provide a list of ideas for fundraising activities Schools provide and promote a list of healthy party ideas to parents and teachers. School celebrations include no more than one food or beverage that does not meet the adopted nutrition standards. If Elementary/Middle schools allow sale of food individually, items limited to fruit and vegetables. 6 A choice of at least two fruits and/or vegetables is offered for sale at any location on the school site where foods are sold. Foods and beverages offered or sold at school-sponsored events operating outside of the school day meet the adopted food standards.
