2 The new Health and Wellness tab on myuhc.com inspiring healthy behaviors The Health and Wellness tab provides an interactive member experience where best intentions can become healthy actions. It is designed to captivate members in continuous health-enabling actions that are fun and meaningful. The Health and Wellness tab is all about getting members engaged and living healthier lives. The Engagement Opportunity Business leaders in charge of promoting wellness initiatives have their work cut out for them. It s not that individuals don t want to get healthy and stay healthy. It s just that people rarely follow through on their resolutions unless they are actively engaged every step of the way. Many organizations leverage health websites to engage their population and build a culture of health. But far too often, today s websites don t rise to meet the challenge. In general, they prioritize health research over action and tend to be one-size-fits-all affairs that don t engage the individual. 2

3 The Health and Wellness tab contains powerful active ingredients The value of the Health and Wellness tab is achieved through a unique blend of clinical health information along with powerful personal health management tools, health trackers, incentive rewards, and personal action plans. The Health and Wellness tab s information and tools are designed to be: All told, the objective is to activate members. And it is reflected in every aspect of the Health and Wellness tab s design. The dynamic design instantly welcomes and delights members. Getting engaged is as intuitive and simple as I AM, I DO and I GET. } Engaging inspiring action through an intuitive, dynamic design focused on building member loyalty. It may actively tap into members desires to make positive changes in their lives and rewards them along the way. The clean visual experience, combined with sticky site features and points-based rewards, is designed to pull members in and keep them coming back for more. } Personalized cultivating a life-long commitment to health through precise customization of the individual user s experience. The more a member interacts with the site, the more personalized the experience becomes which may lead to reinforcement, continued engagement and action. } Integrated offering a fully synchronized experience across all UnitedHealthcare wellness solutions. Our goal is to keep the member focused on a prioritized set of health-enabling activities. So, whether they are online or working with a coach on the telephone, their experience is consistent and consistently rewarding. I AM I DO I GET Collects health information to continuously personalize the online experience Guides members through action steps which may lead to long-term behavior change Rewards members for getting and staying engaged 3

4 I AM Personalization begins and a member s story of health is told. The more information the member enters, the more personalized and engaging their online experience becomes. Health Assessment Developed in partnership with the University of Michigan s Health Management Research Center, this quick and easy questionnaire uncovers biometrics, healthrelated behaviors, quality-of-life indicators, medical history and more. Health Assessment results are instantly shared with the individual and are used to stratify members into telephonic wellness coaching and condition management programs. Aggregate results provide meaningful insights into the overall health of the employer s workforce. And lastly, Health Assessment data is deeply integrated online and offline, fueling the member s Personal Health Record, Health Trackers, Health Content, I DO action plan, and UnitedHealthcare wellness and condition management programs. Personal Health Record The Personal Health Record captures and organizes a vast array of health history information in one convenient location, while empowering members to manage their well-being with integrated health tools and trackers. Medical claims, lab values and pharmacy data are continuously compiled and updated to create a comprehensive, up-to-the-minute health history. Health Trackers The Health and Wellness tab is focused on engaging members, and sticky site features like Health Trackers deliver on that promise. Health trackers measuring blood pressure, weight loss, physical activity and more are tightly integrated into the I AM and I DO sections of the site. Members (and their UnitedHealthcare coaches) can plug health data into our Health Trackers as they work together toward health goal attainment. The Health Assessment, Personal Health Record and Health Trackers are instrumental in personalizing and continuously updating the user s online experience. That s important because a personalized website that is constantly refreshed may bring members back. 4

5 I DO This is where personalization meets action. Members are supported through real-time action plans that are tailored just for them. Action plans include clinical reminders, health trackers, videos and reading suggestions that provide a clear roadmap to behavior change success. Telephonic nurses and coaches can view and interact with a member s online action plan and Health Trackers, providing a fully integrated wellness experience. Members can choose from a variety of wellness and condition-focused programs devoted to weight loss, exercise, diabetes management and much more. I DO Action Plan Because behavior change is a lifelong process, I DO is designed to create an ongoing relationship with each member. Action plan activities are refreshed and can be completed based on the member s readiness to change. And because everyone needs reinforcement along the way, the program is reward-ready, and organizations can provide incentives for completing action plan activities. The Health and Wellness tab has many online programs designed to help members get healthy, stay healthy or live with a chronic condition. Also, the clinical information presented here is the same information conveyed in our telephonic programs, creating consistency across all learning opportunities. 5

It s where members can capitalize on their actions in I AM and I DO by discovering rewards in My Rewards and new ways to stay motivated in My Inspiration. I GET is designed to help members stay motivated as they pursue their individual health goals. The My Rewards detailed views show earnings potential and successes, both for the individual and family. My Rewards can support unique incentive programs that include online and telephonic programs, worksite wellness events and other company-sponsored events. My Rewards Redemption options include debit cards, gift cards and merchandise. With complete rewards information available within the Health and Wellness tab, your employee population will have the information to help them stay on the path to good health. In addition to the optional incentive program capability, I GET features My Inspiration for all members. My Inspiration can provide intrinsic motivation.
