Brief Description of Site Activity (e.g. auto repair, storage warehouse, tank farm, etc.) EMERGENCY CONTACT 6. Name 7. Street Address 8. City State Zip Code 9. Phone hour Phone (if applicable) PROCEDURES (develop and attach each of the following procedures) 11. First Response Procedure Describe the responsibilities of the first person(s) to recognize a hazardous materials spill. 12. Employee Notification Procedure Describe the procedures (and alarm signals as appropriate) for notifying other site employees of the spill. 13. Employee Response Procedure Describe the responses expected of employees (evacuation, spill response, etc.). 14. Evacuation Routes, Emergency Exits, Staging Areas Show clearly on a site map the primary and alternate evacuation routes, exits, and employee staging and checkoff areas.

2 15. Spill Mitigation Procedure Explain the procedures for mitigating a release or threatened release. Include equipment and resources for use in emergency situations such as communications equipment, containment materials, monitoring devices, etc. Identify the spill response personnel (on-site team or prearranged contracts with a spill response company). If responses are to be different for spills or emergencies of different magnitudes, describe the management procedures to be used for each. Identify the person(s) who will coordinate with and assist emergency response teams or agencies. Describe the procedures for proper management and disposal of the recovered hazardous materials. 16. Emergency Equipment Record Prepare a chart listing of emergency equipment by location. Include an inspection schedule that assures items are present and in working condition. 3 17. Preparedness Checklist Prepare an Emergency Preparedness Checklist that is adapted for your facility. The following list of items is suggested for your consideration. It does not necessarily include everything appropriate for your facility. EMERGENCY PREPAREDNESS CHECKLIST LEGAL REQUIREMENTS Owner and/or operator is familiar with his/her legal responsibilities should an emergency involving a hazardous material occur at the facility Copies of applicable State ans Federal Hazardous Materials Regulations are available at the facility Emergency reporting and notification of appropriate agencies are posted in case of an emergency HAZARDOUS MATERIALS REFERENCE INFORMATION Material Safety Data Sheets (MSDS) are available for hazardous materials used and stored at the facility Employees and supervisors have access to the MSDS file for routine information or use during an emergency. The MSDS file is reviewed for completeness, and is kept up-to-date Employees are trained in the use of MSDSs and other hazardous materials information The emergency coordinator or the person responsible for maintaining the Safety and Hazard data on materials and equipment is available during emergencies. REQUIRED A list of all emergency equipment, including required inspection frequencies and repair records, is maintained for the facility. Internal communications and alarm systems capable of notifying facility staff are in place at the facility and in working order. A telephone or other communications equipment is available near hazardous materials to notify fire, police or health agencies in the event of an emergency. Portable fire extinguishers, (including special extinguishing equipment such as fire control equipment, inert gas, dry chemicals, and foam) are available for emergencies. Spill control equipment is available and in place to control hazardous materials spills or releases should they occur. Decontamination equipment to cleanse plant staff and equipment is available should a spill or release occur. A private cleanup company or hazardous materials cleanup policy is available for handling release emergencies should they occur. An adequate water supply with sufficient water pressure and volume to control fires and other emergencies is available. MAINTENANCE AND TESTING

4 A regular program of emergency equipment maintenance and testing is in place at your facility. There is adequate aisle space to allow the unobstructed movement of personnel and of fire protection, spill control, and decontamination equipment throughout the facility. EMPLOYEE TRAINING The owner or operator of the facility has as employee training program which ensures that the facility complies with all elements of hazardous materials/waste laws. The training program is directed by a person familiar with hazardous materials and waste management procedures, including emergency response plan management. Employees receive instruction on hazardous materials and waste management practices which relate to their duties at the facility. Training includes instruction in emergency procedures, and the use of emergency equipment and emergency systems. There are procedures for using, inspecting, repairing, and replacing emergency and monitoring equipment at your facility. The training program must include hazardous materials training for new employees within 90 days of employment or job function change. Refresher training must me provided annually. The training program will also include as appropriate: (1) Instructions on the use of automatic cutoff systems; (2) Response methods for fires or explosions; (3) Response methods for releases to storm drains, sewers, or surface waters; (4)Procedures for operations shut-down. RECORDS AND DOCUMENTS THAT SHOULD BE MAINTAINED AT FACILITY Job title and name of each employee holding a position related to hazardous materials/waste management. Written job descriptions for each position related to hazardous materials/waste management. Written description (including dates and hours) or training given to each person managing hazardous materials/wastes at the facility. Records that show each employee holding a position related to hazardous nmaterials/waste management has the required training and refresher courses.

5 18. Agency Notification Procedures and Telephone Numbers Outline emergency notification procedures for each potential emergency. Provide the name/position of the person to make the report, the persons or organizations to be notified, the telephone numbers, the information to be reported, and the time within which the report must be made. National Response Center or Same as above At earliest practicable moment National Response Center Carrier (Employer) Written Report Carrier s # DOT Form F as prescribed Within 30 days Spill of Haz.
