2 Document Number: EP Version: 5.0 Prepared by: S.Healy Type: Specification Document Title : Procedure for Handling Hazardous spills Page 1 of OBJECTIVE The purpose of this procedure is to provide guidelines to personnel on the actions to be taken following the discovery or occurrence of a chemical spill. 2.0 SCOPE This procedure applies to all personnel at Stryker. 3.0 APPLICABLE DOCUMENTS IPC license No: - 23 Emergency Response Procedure Associated Forms on E-Doc 4.0 OVERVIEW SP-400 Many of the materials used on the Stryker site are hazardous. They have the potential, if handled incorrectly, to endanger life or health, and to cause serious damage to buildings, equipment and the environment. It is for these reasons that all chemical spills, no matter how small, must be taken seriously and personnel should be trained to act quickly, safely and effectively. It is therefore necessary to be prepared with a well-planned emergency procedure before a chemical spill occurs or is detected. A specially trained Emergency Response Team (ERT) has been established and trained at Stryker to provide for the rapid and safe clean up of large chemical spills, and for the spill clean up of corrosive materials such as concentrated acids, caustics etc. For minor and less hazardous spills, spill response kits have been located at various locations throughout the Stryker facility, they are not to be used in cleaning up water. The location of the spill kits is listed in EP 907 A. These spill kits contain various types of spill clean up materials and disposal bags. Drainblockers are also located at various key locations outside the facility and are specially designed to be placed over drains in the event of a chemical spill, thus preventing hazardous material entering the drainage network. This procedure sets out to define the types of chemical spills that personnel have a responsibility to clean up, the clean up methods to be followed, and the methods to be used to dispose of the clean up material. It also sets out to define the types of chemical spill that require an ERT response as set out in the Emergency Response Procedure SP-400.

3 Doc No. EP 907 Ver.5.0 Page 2 of DEFINITIONS Spill Any unplanned release of chemical ERT Emergency Response Team PPE Personal Protective Equipment Small Spill - Less than 5 litres or 1 metre in diameter Large Spill - Greater than 5 litres or 1 metre in diameter. 6.0 RESPONSIBILITIES 6.1 For small spills responsibility rests with appropriately trained personnel in the respective department. 6.2 For large spills the Emergency Response Team will assume responsibility for spill clean-up. 6.3 EHS Monitor the incident and advise handling procedures Inform Local Authority, Environmental Protection Agency and of the incident if necessary Provide training for the users of spill equipment Advise where necessary on measures to prevent a recurrence Maintain records of the incident Keep stocks of spillage absorbent material replenished as necessary. 7.0 PERSONAL PROTECTIVE EQUIPMENT Small Spill: Large Spill: Safety Glasses/Goggles, Safety Shoes, Gloves Coveralls or chemical-resistant suit, Respiratory Protection, Safety Glasses/Goggles, Boots, Gloves Note: It may be necessary to use a respirator and filter when dealing with Small Spills involving materials such as Ammonium Hydroxide, Nitric Acid etc. Respiratory Protection may require using a respirator and filter, or in some cases Self Contained Breathing Apparatus (SCBA).

4 Doc No. EP 907 Ver.5.0 Page 3 of SPILL RESPONSE - GENERAL 8.1 The employee who first encounters a spill should ensure that the following steps are taken: Turn off any machines involved Try to switch off or isolate the source of the spill e.g. closing a valve, shutting down a pump etc. WITHOUT endangering themselves in the process Contact the Facilitator or local trained spill response person(s) in the area 8.2 The Facilitator or local trained spill response person(s) in the area should assess the situation by gathering the following information where possible regarding the spill: Where is the spill? When did the spill happen? What chemical is involved? How much chemical has spilled? What surface area has the chemical covered? Has the source of the spill been isolated? Are there any injuries? On the basis of the information available and the size of the spill involved, the Facilitator or local trained spill response person(s) will determine whether the spill can be cleaned-up locally, or whether the ERT should be called. If the ERT are required then SP 400 Emergency preparedness and response procedure should be referred to.

5 Doc No. EP 907 Ver.5.0 Page 4 of SMALL SPILLS LOCAL AREA RESPONSE 9.1 Only appropriately trained personnel within the area should carry out a chemical spill clean-up. 9.2 Ensure appropriate personal protective equipment is worn before dealing with the spill. 9.3 Ensure information is available concerning the identity and hazards of the chemical(s) involved before proceeding; if in doubt, consult with the Facilitator or seek the help of the ERT. 9.4 Seal off the area of the spill and warn others of the dangers. 9.5 In the case of solvent spills avoid all sources of ignition. 9.6 Contain the spill and prevent it spreading. This can be done by stopping the source of the spill e.g. shutting a valve, plugging a leaking drum etc. and using spill materials provided such as socks, mats, pillows to soak up the material concerned. 9.7 Collect all absorbent materials in a disposal bag. Safely dispose of all coveralls, gloves etc and decontaminate all non-disposable items such as protective clothing, breathing apparatus etc. 9.8 A Chemical Spill Report, Ep 907 B, must be completed for all significant spills on site. The completed forms are given to the EHS Co-ordinator and are logged onto the spillage log Ep 907 C and are maintained on file in the EHS Office. A Waste Authorisation Form EP 806 A, needs to accompany the Spill report, the EHS department will identify the storage location of the material on the form. The form will be returned to the business unit. This form is then given to Plant Services who will be responsible for ensuring that the material is removed and brought to the proper storage location. 9.9 It is important EHS receive the Chemical Spill Report so that: All absorbent materials can be disposed of safely, The spill kit can be replenished. A log can be kept, so that a Cause is identified and Corrective actions taken so the spill will not reoccur RECORDS The EHS Department will maintain records of any spill incidents on site and notifications made to regulatory authorities. Records will be maintained for at least seven years.
