The transition from treatment into a life of recovery is a rewarding challenge. The TreeHaven is designed to assist the resident with that transition by providing sober peer support and by offering accountability in their first months in recovery. As a member of the TreeHaven program, your loved one will be required to participate in an active recovery program including: Intensive Outpatient Treatment 12-Step participation Sponsorship support Maintain a clean and healthy living space And better their circumstances through education, employment or both. Just as their addiction did not progress to its full manifestation over a short period of time, their recovery will take some time to develop. Recovery is not simply about abstaining from alcohol and other drugs. It is about living a life that is meaningful and participating in relationships in a way that benefits them and the people around them. For these reasons, the resident will be expected to achieve certain goals and follow house rules while they are here. Program Description We understand the need to bridge the gap between leaving primary care treatment and entering the real world. The Tree House offers transitional living for women to support them in a safe environment as they put into action their recovery program, gain employment and/or return to

The TreeHaven transitional program provides residents with the necessary tools to move forward in their lives and continue the mission of the Ranch at Dove Tree by preparing young adults and their families for a lifetime of recovery. The newly renovated residence offers a nurturing, home-like atmosphere where individuals can focus on recovery in comfort. The accommodations include prepared meals, wireless Internet access and complimentary health club membership. The TreeHaven is a place to practice using the tools of recovery and to focus on applying the 12- Steps to solve every day problems while living with other recovering individuals. Programming at The TreeHaven supports individuals as they fully reclaim their independence from addictive substances, learn to manage the stress of an active life without returning to drug and alcohol dependence, and attain personal, career and recovery goals. The program offers many benefits including: Transition into a collegiate recovery community Focused direction and personal accountability 24 hour on-site house managers Individualized transitional plans, recovery protection planning and goal setting Life skills planning including learning to prioritize daily responsibilities, house cleaning, meal planning and preparation, budgeting, and finances Resume and job placement assistance Medication management Random alcohol and drug testing Program Outline The TreeHaven is a Transitional Living Program for females between the ages of 17 and 25. It is designed for successful completion within a 90-day time period. The TreeHaven recognizes that long-term treatment is the preferred option for young adults with addictive disorders and prescribes to the idea that a comprehensive discharge plan will include significant recommendations for aftercare services. Residents in the TreeHaven progress according to a three-tiered phase system. Residents receive additional privileges as they complete each phase of the program. Movement from one level to the next is determined by the TreeHaven team which judges each resident according to his individual goals in the transition process. Successful completion of the program requires that a resident progress through each phase of his transition program which includes recovery, relational, academic/vocational, and wellness goals. The TreeHaven provides residents with the necessary tools to move forward in their lives and continue the mission of The Ranch at Dove Tree by preparing young adults and their families for a lifetime of recovery. Each resident s personalized plan includes: Intensive Outpatient Therapy Recovery Coaching at least 1 hour each week Assistance with and coordination of psychiatric appointments/follow-ups Three meals per day provided with a fully stocked kitchen and pantry Transition into a collegiate recovery community Focused direction and personal accountability 24 hour on-site house managers Individualized transitional plans, recovery protection planning and goal setting Life skills planning including learning to prioritize daily responsibilities, house cleaning, meal planning and preparation, budgeting, and finances Resume and job placement assistance Educational advising (college placement/high school diploma or equivalent) Gym membership at Texas Tech University s state of the art recreation facility Transportation for meetings, school, job interviews, and employment Cable and WIFI Medication management Random alcohol and drug testing Curfew Phase 1 9:00 pm Monday Sunday Phase 2 11:00 pm Monday Sunday Phase 3 11:00 pm Sunday Thursday/ 12:00 am Friday Saturday Guidelines The facility is strictly alcohol and drug free Residents may have their cell phones and computers upon admission

4 TreeHaven Program Overview Residents may have their vehicles after they progress to Phase 2 No overnight guests TreeHaven Phase System While the resident is living in the TreeHaven, they will move through phases. These phases are designed to support the development of their recovery. They are outlined below. If we become concerned about the residents recovery, if the resident has a problem with rule infractions or if the resident fails to maintain employment, or status as a student, they may be stepped down to Phase 1 or receive further restrictions. All Residents will be reviewed on a case by case basis. Requirements may be adjusted by staff depending on progress. Phase 1: Residents are expected to attend and participate in RDT s Intensive Outpatient Program (IOP) from 6-9pm Monday Friday for at least 10 sessions. Residents are expected to attend individual counseling sessions as scheduled by the IOP treatment team and actively participate in their individual treatment plan. Residents must attend a minimum of seven 12-step meetings per week. You will be required to turn in meeting sheets at the Monday house meeting each week. Residents are expected to attend weekly house meeting Mondays at 11:00 am. Residents are expected to actively work a 12-step program. Residents are expected to obtain and maintain a sponsor that can be verified by a member of the TreeHaven staff. Residents are expected to work towards job readiness. Create a resume, identify possible job leads, and work with staff to prepare for job seeking interactions and interviews. Complete all assigned chores at the TreeHaven as well as maintain the cleanliness of your room. Residents must be up and out of bed by 9:00 am and remain awake throughout the day. Residents are expected to participate in life skills planning including house cleaning and planning and preparation of meals twice weekly. All of your reasonable transportation needs should be accommodated by a member of the TreeHaven team. During Phase 1, personal vehicle privileges are subject to staff approval and legal documentation (license insurance registration). Residents cannot leave the premises with anyone other than a member of the TreeHaven team. No non-family visitors are allowed for the first week of phase 1.

5 TreeHaven Program Overview Phase 1 can be extended if the resident does not make significant progress in the areas listed above. Their progress will continually be evaluated by the TreeHaven team. Each house member will be assessed on an individual basis and moved to the next Phase based on their progress/compliance. Phase 2: Residents are expected to attend and participate in RDT s Intensive Outpatient Program (IOP) - 8 remaining sessions days are determined by you and your individual counselor. Residents are expected to attend individual counseling sessions as scheduled by the IOP treatment team and actively participate in their individual treatment plan. Residents are expected to attend weekly house meeting Mondays at 11:00 am. Residents are expected to attend a minimum of seven 12-Step meetings each week (in addition to the IOP meetings). You will be required to turn in meeting sheets at the Monday house meeting. Residents are required to turn in their work schedules for the week at the Monday house meeting. Residents are expected to actively work a 12-step program. Residents are expected to obtain and maintain a sponsor that can be verified by a member of the TreeHaven staff. Residents must be up and out of bed by 9:00 am and remain awake throughout the day. Residents are expected to participate in life skills planning including house cleaning and planning and preparation of meals twice weekly. Complete all assigned chores at the TreeHaven as well as maintain the cleanliness of your room. Residents are expected to obtain and maintain employment or attend school. Residents are expected to complete at least 20 job applications per week until a job is secured. Proof of application is to be provided to TreeHaven coordinator daily. Residents are expected to be a leader in the house and to demonstrate recovery behavior in your interactions inside and outside of the TreeHaven. Residents in this phase get a curfew extension of 11:00 pm - Monday Sunday Phase 3: Residents are expected to attend weekly house meeting Mondays at 11:00 am. Residents are expected to attend a minimum of seven 12-Step meetings each week (in addition to the IOP meetings). You will be required to turn in meeting sheets at the Monday house meeting. Residents are required to turn in their work schedules for the week at the Monday house meeting. Residents are expected to actively work a 12-step program. Residents are expected to obtain and maintain a sponsor that can be verified by a member of the TreeHaven staff. Residents must be up and out of bed by 9:00 am and remain awake throughout the day.

6 TreeHaven Program Overview Residents are expected to participate in life skills planning including house cleaning and planning and preparation of meals twice weekly. Complete all assigned chores at the TreeHaven as well as maintain the cleanliness of your room. Residents are expected to obtain and maintain employment or attend school. Residents are expected to be a leader in the house and to demonstrate recovery behavior in your interactions inside and outside of the TreeHaven. Residents in this phase get a curfew extension of 11:00 pm Sunday Thursday and 12:00 am Friday Saturday. Residents are eligible for overnight pass (2 nights maximum). You must submit a written pass request at least 3 days in advance. Residents are expected to facilitate a Big Book Study at least twice weekly at the direction of the Tree House Coordinator. The TreeHaven Visitation Guidelines Visitation Visitors must sign in and are subject to staff approval and all other rules of the house. No overnight visitors. Male visitors are not allowed in the house, with the exception of female family members. Residents on Phase 3 are eligible for a pass. Pass requests are to be submitted by noon on Monday. Passes will be granted at the discretion of staff, according to resident progress, behaviors, and risk factors. All visitors need to be introduced and identifiable to all staff members. No uninvited guests are allowed on the premises. The maximum house visitation limit is 6 visitors total. Anyone over that limit may be asked to leave. Visitors must leave the premises by 9:30 pm on weekdays/10:30 pm on weekends. Visitors need to vacate the premises during any house meetings or house functions. Visitors must be in the presence of a resident or staff member at all times. Any visitor in violation of these guidelines may be asked to leave the house. Visitors may be asked to leave if house duties are not being completed by residents. Any of these guidelines are subject to revision at any time.

10 The TreeHaven Program Overview Al-Anon/Alateen Resources for Families Al-Anon: Has only one requirement for membership; members have family that has been affected by someone s drinking. Al-Anon is an anonymous fellowship of: relatives and friends of alcoholics, adult children of alcoholics, parents, partners, spouses, co-workers, etc., who meet to share their experience, strength, and hope in order to solve their common problems. This is a separate fellowship from Alcoholics Anonymous (AA); Al-Anon is based on the Twelve Steps and Twelve Traditions adopted from AA. The fellowship is: non-professional, self-reporting, non-denominational, multiracial, apolitical, and is available almost everywhere. The Ranch at Dove Tree hosts an Al-Anon meeting every Saturday at 1:00pm and encourage families to attend. Alateen: Alateen is an Al-Anon program designed for the younger relatives and friends who are 18 years old or younger. Members conduct their own meeting with the guidance of an Al-Anon sponsor. Meetings follow the same Twelve Steps, Twelve Traditions, and Principles as Al-Anon. Al-Anon/Alateen: Fellowship meetings teach the facts about alcoholism as an illness; how it has impacted their lives physically and emotionally; and improve attitudes and behaviors the studying the Twelve Steps and Twelve Traditions. Al-Anon and Alateen introduce contacts to other members who have similar problems.

11 The TreeHaven Program Overview Al-Anon/Alateen Members Receive Help: When they attend meetings regularly; Read literature about the12-steps and 12 Traditions, and apply it to their lives; Make contacts with other members; Have a sponsor; and Become involved in Al-Anon volunteer work. Al-Anon and Alateen Groups: Give advice. Members DO NOT: endorse or oppose, therapy, treatment, or religious beliefs. Members DO NOT: gossip or criticize. Al-Anon/Alateen is Self-Supporting: Voluntary contributions of its members subsidize group costs, and there are no dues or fees for membership. Outside funds, grants, or donations are not accepted. Al-Anon/Alateen is Anonymous: The identity of all Al-Anon and Alateen members is protected. Confidentiality is allowed to develop a sense of trust and honesty. The focus is on spiritual principles, not spiritual personalities. 19 TreeHaven Program Overview Resident Rules & Guidelines All staff members at The TreeHaven seek to provide you the best possible care while you are with us. The staff is responsible for maintaining the rules, which are designed to ensure safety and growth while in our transitional program. The staff will always seek to administer the rules and guidelines fairly with courtesy, respect, and the highest degree of professionalism. We ask that you follow the guidelines in this handout and respond, in turn with the same courtesy and respect with which they are administered. Any of these rules & guideline infractions may result in a loss of privileges, level adjustment and possible discharge from the facility. Rules are subject to modification based upon the discretion of TreeHaven Staff. Responsibilities of the Resident: 1) To remain free from all addictive or potentially addictive mood or mind-altering substances, including alcohol and other behaviors (e.g., self injury, eating disorders) for the duration of your stay. 2) To voluntarily submit to a urinalysis test upon request of the staff. 3) To be present and arrive on time for all individual and group meetings and remain awake and alert. 4) To respect completely the privacy and anonymity of all TreeHaven residents. 5) To not become involved romantically or sexually with other residents. 6) To be committed to becoming more honest with yourself. 7) To report to staff any problems that you are experiencing that could indicate a potential relapse. 8) To be considerate and respectful of the rights of other residents and staff. 9) To take an active part in group activities. 10)To complete all issued assignments. 11)To keep personal living area neat and clean and to participate in cleaning the facility on the designated day. Searches Upon arrival at The TreeHaven, staff will search all resident s belongings with the tenant present. If at any time treatment staff determines there is a need to conduct room searches, staff will do these with residents who are staying in the room present. At no time will body cavity searches by conducted; however, residents may be asked to empty their pockets. General This facility is strictly alcohol and drug free. Residents may not have romantic or sexual relationships with other residents. Residents will not be in any type of physical fight or loud verbal altercation. Residents may invite family members only during visiting hours. Any exceptions must first be cleared with the house manager. Residents are allowed to play video games within a reasonable amount of time determined by individual s schedule and staff. If they are becoming a problem, staff reserves the right to restrict the client from playing. Residents are not allowed to gamble. Energy drinks are prohibited. No drugs or alcohol allowed. Anyone found to be under the influence must vacate immediately. 9

20 TreeHaven Program Overview Any information regarding substance use must be reported to staff. Urine alcohol and drug testing be given randomly. Visiting head shops is prohibited. Any accident or property damage must be reported to staff immediately. Residents are permitted to drive their personal vehicle in Phase 2 and proof of insurance and current drivers license is photocopied in office. Residents are encouraged to park in the rear lot or driveway for security reasons. In order to promote self sufficiency and exercise, weather permitting, residents could be required to walk or bike to any destination less than a mile away. Permitting weather requirements are : 1) 50 degrees or more 2) Winds less than 30mph 3) No hail, rain, snow, tornado, or haboob Medications All medications must be turned in to the office upon arrival. This includes all vitamins and over the counter medication. All personal vitamins and over-the-counter medications must be in new sealed containers. All medications are to be self-administered with staff observation. All medications must be signed for in the medication log when taken. Medications must be taken before leaving the office. Medications will be given at scheduled dosage times. The medication times are as follows: Morning - 9-9:30 am Lunch :30 pm Afternoon - 3-3:30 pm Dinner - 5-5:30pm Bedtime - 9-9:30 pm If you miss a medication time, you will have to wait until the next medication time to take your medicine. Failure to take medication consistently may result in disciplinary action. All new prescriptions are to be checked in immediately after pickup, in unopened condition. Bedrooms No food or open drinks in bedrooms. Residents may keep a bottle of water in their room. All bedrooms are to be kept clean throughout your stay. Beds are to be made every morning prior to starting daily schedule. Bathrooms must be kept clean and toiletries and other personal items must be put up after every use. Rooms will be deep cleaned by residents at least one time per week, per cleaning schedule. Rooms and belongings are subject to inspection. Deep cleaning includes the toilet, sink, tub/shower, vacuuming, and mopping. Tobacco Use NO USE OF TOBACCO INSIDE THE FACILITY OR FACILITY VEHICLES. This includes but is not limited to cigarettes, snuff, and electronic nicotine devices. 10

21 TreeHaven Program Overview Please dispose of cigarette butts in designate containers. Smoking is only allowed in designated smoking areas and must be at least 20 feet from all doors. Please be considerate to non-smokers when smoking in shared areas. Dipping or chewing tobacco must be used in designated smoking areas only. Please spit in a cup and dispose of it in the trash. Do not dip or chew tobacco in the house. Resident Food Residents must alert the house manager during admission of any food or drug allergies. Please label all personal food items and/or drinks. Visitation Visitors must sign in and are subject to staff approval and all other rules of the house. No overnight visitors. Female visitors are not allowed in the house, with the exception of female family members. Visitors do not have meal privileges unless prior approval is given by staff. Residents on Phase 3 are eligible for a pass. Pass requests are to be submitted by noon on Wednesday. Passes will be granted at the discretion of staff, according to resident progress, behaviors, and risk factors. All visitors must be compliant and in good standing with IOP, CSAR, or whichever sober living facility they live in. All visitors need to be introduced and identifiable to all staff members. Any visitor who has been identified to have had an inappropriate relationship with any resident may be banned from the house at staff s discretion. All visitors are subject to approval of staff and fellow house members. No uninvited guests are allowed on the premises. Residents are permitted to have no more than two guests at a time; any visitor that exceeds limit needs approval. The maximum house visitation limit is 6 visitors total. Anyone over that limit may be asked to leave. Visitors must leave the premises by 9:30 pm on weekdays/10:30 pm on weekends. Visitors need to vacate the premises during any house meetings or house functions. Visitors must be in the presence of a resident or staff member at all times. Any visitor in violation of these guidelines may be asked to leave the house. Visitors may be asked to leave if house duties are not being completed by residents. Any of these guidelines are subject to revision at any time. Daily Routine Residents have to be up and out of bed by 9:00 am every weekday and remain awake throughout the day. Breakfast is served at 8:30. Rooms must be clean and beds made by 9:30 am. Morning Meditation group is at 10:00 am. Lunch is served at 11:00 am. Each resident in Phase 1 is expected to attend a Noon meeting DAILY! 11

22 TreeHaven Program Overview Evening Routine Residents will attend RDT s Intensive Outpatient Program (IOP) from 6-9pm Monday Friday for two weeks (10 sessions) in Phase 1. Residents will attend IOP in Phase 2 two three nights per week to complete your 18 sessions. Residents are expected to keep their individual appointments with their primary IOP counselor. Noon meetings can be substituted for evening meetings; however IOP meetings do not count toward 12 step meeting attendance. Curfews will be strictly observed. Phase 2 11:00 pm/phase 3 11:00 pm Sun-Thurs, 12:00 am Fri - Sat Residents are not to exit the house after alarm system is armed except for designated door allowed for smoking. Resident Consequences to Rule Infractions Residents will receive consequences, should they be found not in compliance with the rules and responsibilities. The specific consequence will be determined by the TreeHaven team. This includes, but not limited to: 1) Getting up on time; 2) Keeping respective rooms clean; 3) Completing assigned chores and house duties on time; 4) Attending and participating in IOP; 5) Complying with phase requirements (obtaining employment in a timely manner); 6) Taking medication during designated times Grounds for Immediate Discharge: Below are rule infractions that will result in an immediate discharge from The TreeHaven: 1) Physical aggression or threat of harm to another person; 2) Sexual conduct including but not limited to kissing and touching; 3) Possession of drugs or alcohol on person or found in possession after search; 4) Dispensing of sharing prescribed medication with another resident; and 5) Drug or alcohol consumption while in our care. 6) Non-compliant discharge from the Intensive Outpatient Program. The TreeHaven staff holds the authority to immediately discharge a resident for any additional rules that are not listed above, if it is deemed inappropriate and threatening to the other residents and/or staff. 12

23 TreeHaven Program Overview Agreement for Residency By signing below, I,, affirm that I have read and received a copy of the TreeHaven conditions and expectations for residency. I agree to abide by these rules and understand that failure to comply can result in additional chores, loss of privileges, and/or removal from the house. Additionally, I agree to keep my living quarters and personal belongings in order and presentable for visitors and prospective residents. I will complete my daily house chores and assist with shopping and meal preparation when assigned. I will sign out whenever I am off the premises and return according to curfew and pass schedules. I will attend at least 7 support group meetings weekly, and keep all my appointments with TreeHaven staff and IOP counselor. I agree to avoid people who use drugs and places where alcohol is served. I commit to put forth the effort to accomplish my stated goals and ensure my sustained recovery. I understand and agree to the TreeHaven relapse policy including: If a relapse occurs, the resident must leave the facility with their belongings as soon as possible the same day The resident can be considered for re-admission into the TreeHaven after they have 30 days sober Going back to residential treatment at The Ranch at Dove Tree is not the default, although it may be considered based on the individual circumstance.
