1 DOVE RIDGE CLEAN AND SOBER/TRANSITIONAL LIVING TENANCY AGREEMENT This agreement is made and entered this day of 2013, By and between Dove Ridge (Clean & Sober Transitional Living, and Joan Maynord and or agent hereafter referred to as landlord, And resident: SS#: Hereafter referred to as tenant. Landlord does hereby rent to the tenant and tenant does herby rent from landlord, from week to week or month-to-month, a room at the premises designated as Cherokee Rd, Tuolumne, Ca and paying as rent the sum of $ per month, or $ per 7-day week. A $25.00 Intake is required upon residence. The sole purpose for a tenant to live at Dove Ridge Clean & Sober Transitional Living is to live in a clean & sober environment and tenant understands that they are giving up traditional rights available under normal landlord-tenancy laws because of the special circumstances for the protection of their sobriety and that of the other tenants. ( ) Tenant agrees to submit to drug and alcohol testing, random or in the event of any suspicious circumstances. The use of alcohol or illegal drugs by resident, either on site or off site is strictly prohibited and will cause for immediate termination from Dove Ridge Clean and Sober Transitional Living. Medication prescribed by a physician and over the counter medications must be reported and signed in to the house manager for safe storage. James Berry, Manager retains the right to establish a method by which the use of medicine may be monitored by the house manager no Narcotics or Sleep aids allowed. Failure to comply will result in immediate termination from Dove Ridge Clean and Sober Transitional Living. No Personal Checks! 2 The undersigned acknowledges receipt and agrees to the following: Resident: Date: I agree to take personal responsibility for my financial obligations owed to Maynord s CDRC, Inc. upon pre-admittance to Dove Ridge Clean and Sober Transitional Living. I will set up a payment plan with the billing/financial department. I agree that if I am more than 7 days late from the due date that this is grounds for immediate termination. ( ) Tenant agrees that termination and removal may be enforced by local law enforcement officials in the event of violations of the above conditions. Resident: Date: This tenancy may be terminated by either party by giving 7 days written notice to the other party ( ) tenant understands and agrees that Rent and Deposits paid is non refundable due to; early move out or involuntary termination. Resident: Date: Tenant hereby agrees not to make any repairs, improvements or alterations unless consented to in writing by landlord or authorized agent. Landlord may enter at any time to inspect, repair or supply necessary and agreed services. Tenant shall not rent or board any persons without written consent of the landlord. Violations can result in termination of tenancy. ( ) Tenant agrees to quit and surrender the premises at the expiration of this tenancy in as good condition as reasonable wear thereof will permit, damage by the elements expected, and tenant especially agrees to remove any and all accumulation of rubbish. TENANT AGREES TO PAY THE MONTHLY or weekly RENT PROMPTLY WHEN DUE TO DOVE RIDGE CLEAN AND SOBER TRANSITIONAL LIVING, $25.00 late fee will be applied if more than 3 days late and if any action or suit shall be brought to enforce any rights hereby conferred for the recovery of possession of these premises, the nonprevailing party shall pay the prevailing parties attorney s fees and costs. 3 VISITORS, HOUSERULES AND CHORES Visitor s Rules Resident visitors are Responsible for being with children at ALL TIMES INSIDE or OUTSIDE the house. No Visitors are to be left in or outside of the house or property, unless approved by manager. No Visitors in the house after 11:00p.m. Weekdays. 12:30p.m. Weekends unless attending a during scheduled meetings. No Visitors allowed in the bedrooms. No Sexual activity in the house or on the Dove Ridge Property. Revised

4 House Rules Curfew: Sunday through Thursday 9:00pm Friday and Saturday Set by Manager. All residents shall be up and dressed by 9:am and all choirs shall be complete by 10:am For the first 30 days a client must check in every three hours at the house and stay within a 20 mile radious. ALL residents are assigned kitchen storage for food. A closet section and a dresser. NO personal Items to be left out, other than pictures or books. NO pictures or any other items are to be hung on walls at any time. There is plenty of storage area for all residents. Residents are responsible for their own meals. (Share meals are encouraged) Resident agrees not to use other s food and not to borrow from each other. Please turn off all lights, TV s, radios, etc. when not in use. Dove Ridge client s house is not responsible for any valuables (Jewelry, money, clothing, food, etc.) ALL residents must dress in a decent manner. No Smoking allowed on front Porch and only in designated area. NO eating or drinking allowed in bedrooms. NO direct profanity against Residents, Manager, House Manager, or Children. NO parking cars on grass. Designated parking only. NO washing vehicle on property. Mandatory attendance at the weekly house and continuing care meetings. Knowledge of another resident using alcohol, illegal drugs or any illegal activity MUST be reported to the House Manager or Management immediately. ALL emergencies must be reported to House Manager IMMEDIATELY. Participant agrees to obey other reasonable rules and regulations that Dove Ridge may establish from time to time to insure the effectiveness of this clean and sober Transitional Living Program. Dove Ridge has an open door policy to all management to insure the safety of residents and a Routine inspection of the home. Laundry Room hours are from 6:00a.m to 11:00p.m. Weekdays and 6:00a.m. To 12:00a.m. Weekends. Residents are to sign out on the white board where they are going what time they left, and expected time the plan on returning. Please check in if you are going to be late on expected time of return. No Resident shall be allowed to go or to work at the Casino at anytime. Revised

5 Chores Each of Dove Ridge residents will be assigned chores and chores must be completed thoroughly and in a timely manner. Residents are to be responsible for cleaning and maintaining of their own living space. Kitchen MUST BE CLEANED IMMEDIATELY after each use. Bathroom area MUST BE CLEANED IMMEDIATELY after each use. 6 DOVE RIDGE CLEAN AND SOBER LIVING HOME RECOVERY AND SUPPORT Group Rules RESIDENTS AT DOVE RIDGE AGREE TO ADHERE TO THE FOLLOWING: 1. Don t go to Maynord s except for 15 minutes before and 15 minutes after AA/NA meetings unless written approval is obtained. Written approval may be obtained by the following; complete request form from House Manager. Turn it in to House Manager; it takes 48 to 72 hours for a reply. Dove Ridge Management/owner has the right to restrict any person/persons from entering Maynord s CDRC inc. property which includes Dove Ridge homes for the safety of Dove Ridge residents and/or Maynord s CDRC inc. Clients. 2. In the first 30 days resident agrees not to leave the general area (20) mile radius. 3. No phone calls TO and FROM current Maynord s clients. Maynord s clients are not allowed to visit Dove Ridge Clean and Sober Transitional House until 72 hours from discharge date. 4. You must sleep at Dove Ridge Clean and Sober Transitional House for the first 30 days. Obey all rules and complete all chores in a timely manner. To take a pass, from 30 to 60 days, one night per 7-day week may be taken. From 60 to 90 days, two night passes and there after Passes must be 5 days apart. 5. Attend two mandatory support meetings and Continuing Care groups. Sponsor may attend support meeting (except for Wednesday Continuing Care). This is encouraged. 6. Attend 80 community 12-step meetings in 90 days That Is 5 Meeting per Week, and at least 3 meetings per week after 90 days. 7. Work the 12 steps of recovery under the direction of a sponsor. 8. Refrain from unacceptable fraternization with other Dove Ridge residents or any clients from Maynord s CDRC Inc. This is grounds for termination and a 6 month ban from all Maynords properties 9. Keep appointments with treatment centers and counselors. 10. Complete assigned chores according to house guidelines. 10. Pay your fee on time. Late fee $25.00 will be charged. 11. 7 DOVE RIDGE CLEAN AND SOBER TRANSITIONAL LIVING STANDARDS SUGGESTED FOR A GOOD RECOVERY PROGRAM Volunteer for service commitments at 12-step meetings Read recovery literature daily Attend 12-step conventions, retreats Contact and call recovering people daily Eat healthy and exercise Avoid new intimate relationships in early recovery DOVE RIDGE RESIDENTS AGREE THE FOLLOWING STANDARDS ARE IMPORTANT FOR A GOOD FAMILY ENVIRONMENT WHERE EACH RESIDENT FEELS SAFE, CARED FOR AND RESPECTED, AND THE HOME BUILDS SELF-ESTEEM AND PRIDE. Treat Dove Ridge as a home Make amends when necessary Be supportive to others Do things to enhance Dove Ridge home. Take direction from House Manager Support successes of others. Acceptance of failures in ourselves and others. Avoid power abuse everyone uses Standards as a guide. Help Dove Ridge be a service and resource center for others in recovery Be loyal and supportive to Dove Ridge Keep homes comfortable and neat Dove Ridge will not tolerate discrimination or derogatory language of any type. 8 DOVE RIDGE CLEAN AND SOBER/TRANSITIONAL LIVING 1/6/2009 This computer is owned by Joan Maynord and she will allow you to use it if you follow these simple rules. Respect the hardware and software. Have all necessary applications open and active. Read the instructions on the computer screen before asking for help. Do not surf the Internet or read when you're logged in on other users desktop. Log off and use your own desktop. No pornographic, Drug & alcohol material, any offensive sayings or objects allowed. You will loose the privilege of using Joan Maynords computer and with it the ability to use the magic jack phone long distance service. The management of Dove Ridge retains the right to make the decision on what is appropriate for use. Stay out of other people's files. Log off when you're finished. Log on using your own name and password if you have been blocked from using the computer and you try to use another s login you will be terminated from Dove Ridge per the Dove Ridge contract/application. Don't eat or drink near the computers. Respect your time and others' time. Take turns using programs. Ask for help if you don't understand something. Boot correctly and follow established log-on procedures. Respect yourself -- don't give out your log-on name and password. Get organized -- have your task at hand and use your time wisely. Respect others -- don't hog the machines. Report malfunctions and glitches to JamesBerry121@yahoo.com it saves time and energy. Take responsibility for your actions clean up your site. Logoff leave the station ready for the next person to use. Attempts to gain unauthorized access to network resources or attempts to hide one's true identity (except when such behavior is explicitly allowed). Attempts to disturb or interrupt the intended use of the network obviously wastes available resources (personnel and equipment) e.g. by sending chain letters.
