DECLARATION OF INSURANCE ARRANGEMENT You are required to make your own arrangements for travel and health insurance cover the entire period of stay in Hong Kong. Insurance coverage should include (but not limited to) travel, personal accident, medical expenses (accident and non-accident), hospitalization and emergency evacuation/repatriation, etc. You are required to provide proof of coverage prior to arrival. I hereby declare that I will arrange my own insurance at my home country and provide details of the coverage to the International Office (IO) of HKIEd prior to my arrival in Hong Kong. I understand that it is my responsibility to maintain a valid insurance which cover my entire period of stay in Hong Kong. Should I fail to do the above, I am solely liable for any consequences that may arise. Please mark X where appropriate 6. Programme Fee (HK $4000) Application will be considered on first-come-first-served basis. The IO will confirm the offer and a total of HK $4000 programme fee will be charged. The programme fee shall include lectures, workshops, excursions, accommodation (twin bedroom), and most of the meals and local transportations as specified on the schedule. The payment shall be made by credit card and the fee is non-refundable. Students who failed to settle the full payment before the designated deadline will be considered as withdrawals. Payment procedures are to be made available in due course. I understand that if admitted, I am required to pay a non-refundable fee of an amount of HK $4000 to the International Office of the Hong Kong Institute of Education. Visa or Entry Permit for Visit to Hong Kong & China Visa According to Hong Kong Immigration Department, nationals of most foreign countries can visit Hong Kong visa-free. For list of visa-free countries, please visit the Hong Kong Immigration Department website at For students whom visa or entry permit is required, they are responsible for applying to the Hong Kong Immigration Department for the necessary visa or Entry Permit. If you plan to visit Mainland China before or after the programme, you are required to apply for a China visa. You will have to apply for one from the Embassy or Consulate-General of People s Republic of China in your resident country prior to your arrival to Hong Kong.
