Harald Gerny Born as the only child of an Austrian aesthetic dermatologist and a Swiss sales manager from the Swiss shoe factory Bally, I studied medicine and dermatology at the University of Zurich. I had the vision to combine cosmetics, dermatology and biochemistry very early on. In 1980 I opened the first aesthetic clinic with a medical cosmetic department in Zurich. Today, together with my team, I operate 25 clinics in Switzerland, Germany and Austria. My philosophy was always skin rejuvenation without a scalpel. One must improve the quality of the skin, train it, and replace the substances lost over time, and not simply cut a piece off. I attended many conferences in Brazil and the USA and gave many lectures. I was the pioneer, who introduced both fruit acids and their derivates, such as esterified fruit acids into German speaking Europe, as well as highly concentrated vitamins. I was the first one to use collagen for wrinkle filling and was part of the lead group for the use of Botox and hyaluronic acid. In 1980, I was the first one to introduce external medical acne cleansing, with antibiotic masks and scar treatments. I have been an established laser specialist for many years.

8 CV Dr. med. Harald Gerny I was one of the first members of the lipolysis community to dissolve fat pads using chemicals. In 1997, I founded the Schweizerische Gesellschaft für medizinische Kosmetik [Swiss Medical Cosmetics Company], which I have been president of since then, with the aim of providing beauticians with medical training. In 1991, I founded Med Beauty Swiss AG, and in 1994 the German subsidiary Med Beauty GmbH. In 1999, we introduced aesthetic franchising concepts for beauticians and hotel spas. This summer, the 3rd edition of my text book "Medizinische Kosmetik" [Medical cosmetics] comes out. In 2008, I developed the first plant stem cell range with many neutral medical studies, together with my good friend Fred Zülle. With this technology, the growth of callus cells (wound tissue) in selected plant tissue is induced under special conditions. 16 PhytoCellTec TM Stem cell properties Stem cells are one of the most fascinating fields in modern biology. They have important properties and serve as our body's repair kits. What are stem cells? Renewal and repair are ongoing processes in the human body. Adult stem cells supply the necessary cells for this replacement. They have been identified in over 20 organs or tissues. Two key features set them apart from ordinary cells: They can differentiate into a diverse range of specialized cells of the same tissue and they have the remarkable ability to renew and regenerate themselves through cell division throughout a person's life. This makes stem cells especially valuable and important in regenerative skin care research. Stem cells in the skin epidermal stem cells Adult stem cells are also present in the outer layer of human skin, the epidermis. These cells are called epidermal stem cells and are found in the innermost layer of the epidermis (basal layer). Every second to seventh cell in this layer is a stem cell. Although their appearance does not differ from that of the other cells in the basal

17 PhytoCellTec TM layer, stem cells have a unique function: They constantly renew and rejuvenate the epidermis by forming new keratinocytes and regenerate damaged tissue as only these cells have the ability to divide indefinitely. Epidermis with epidermal stem cell Epidermal stem cells are located in the basal layer of the epidermis. The stem cells divide to form new cells, which also divide and are slowly pushed up to the top of the epidermis. They are then shed from the surface of the stratum corneum, the outermost layer of the epidermis. In young skin, the epidermis fully renews itself approximately every 4 weeks. This process continuously slows as we age. The number of skin stem cells decreases and their vitality diminishes. Excessive UV exposure of the skin and an unhealthy lifestyle (smoking, alcohol) aggravate these effects considerably, resulting in thinner, more deeply wrinkled skin. The hydrolipid film is reduced, which leaves the skin dryer and less well protected. As no new epidermal stem cells can develop during a person's life, it is all the more important to maintain the vitality of these valuable cells and their ability to divide as long as possible.

18 PhytoCellTec TM Plant stem cells protect skin stem cells. How does this work? Every stem cell contains specific epigenetic factors whose task is to preserve the stem cells' multi-potency and its ability to self-renew. Mibelle Biochemistry researchers have discovered that plant stem cells contain epigenetic factors similar to those of adult human stem cells. Applied in the right way, they have a positive impact on the vitality of skin stem cells and their proper functioning. How can the vitality of epidermal stem cells be measured? Is it even possible to improve the vitality of stem cells, keeping the skin youthful longer? Proving this is particularly difficult since epidermal stem cells look exactly the same as the other cells in the basal layer. Only the development of a new test method, the so-called progenitor cell targeting, made it possible to isolate stem cells from the human epidermis for test purposes. These epidermal stem cells can be grown in Petri dishes using specific culture media. Using this cell culture test, one of the unique properties of stem cells, the colony-forming efficiency (CFE), can be assessed in the lab. First, very dilute dispersions of epidermal stem cells are plated on Petri dishes. Each vital, healthy stem cell divides, forming a colony of

19 PhytoCellTec TM cells, which is visible to the naked eye and can then be counted. Stem cells that have aged and are weakened do not die, but they no longer form such colonies. Researchers were thus able to show that using plant stem cells (all PhytoCellTec products) is proven to increase the longevity and colony-forming efficiency essentially, the vitality of epidermal stem cells.

The apple The Uttwiler Spätlauber is an old Swiss variety, which is very rare today. 22 PhytoCellTec TM Preserves the ability to form new epidermal tissue Young, vital epidermal stem cells can form a complete, multi-layer epidermis in laboratory conditions. When this experiment is conducted on older stem cells, these no longer multiply and no epidermis is formed. These stem cells are depleted and have reached the end of their lifecycle. If, however, Malus Domestica stem cell extract is added to cultivate the stem cells, even old stem cells can still generate a stratified epidermis. Under the influence of Malus Domestica stem cell extract, the epidermal stem cells retain their vitality longer and are capable of forming new tissue and renewing the skin. Anti-wrinkle effect The anti-wrinkle effect of PhytoCellTec Malus Domestica was studied in a clinical trial with 20 subjects (aged 37 to 64). An emulsion with 2% PhytoCellTec Malus Domestica was applied to the crow's feet area over 28 days. The results showed a significant, visible reduction in wrinkles in 100% of the subjects.

The Alpine rose The alpine rose is a typical Swiss flowering plant from the Alps. It has bright pink blossoms and grows at altitudes up to 3200 m. Its evergreen leaves are especially resistant to high UV radiation and extreme dryness and cold. The sturdy leaves are a shiny green, even once the snow has melted in the spring. This phenomenon is due to the presence of a special protection protein called dehydrin in the leaves of the alpine rose. Plant stem cells for skin stem cells For years researchers at Mibelle Biochemistry have investigated the compounds of the alpine rose and its special protective substances, in particular, the waterretaining dehydrin. Treatment with PhytoCellTec Alp Rose increased the stem cells' colony-forming efficiency (vitality) and provided significant protection against UV rays. Ski holiday study Twenty-two subjects were treated with an SPF 30 sunscreen formula during their ski holidays (1 week in March, Alps) to test the effect of PhytoCellTec Alp Rose in extreme environmental conditions. Sunscreen without the active ingredient was applied to one side of a subject's face, while the other side was treated with sunscreen containing the active ingredient PhytoCellTec Alp Rose. The loss of skin moisture in the face was then measured after another week of treatment at home. The results showed that water loss was reduced by 39% after 2 weeks treatment with PhytoCellTec Alp Rose. This indicates that PhytoCellTec Alp Rose strengthens the skin barrier and makes it more resistant to environmental stresses such as UV, wind, cold temperatures and dryness.

31 PhytoCellTec TM Adult skin stem cells Dermal stem cells The main function of the dermis is to provide mechanical support and elasticity to the skin. In order to maintain these important functions, healthy skin undergoes constant cell turnover, which renews and repairs the dermal connective tissue. New cells are produced to replace damaged or dead ones. Responsible for this regenerative process are adult dermal stem cells, which reside in the dermal papilla of the hair follicle. These cells differentiate into dermal cell types like fibroblasts, which are responsible for the continuous production of the extracellular matrix proteins elastin and collagen. However, dermal stem cells do age, they become less in number and exhibit reduced activity. As a consequence the production of collagen and elastin drops and the skin loses its firmness - wrinkles appear. Therefore it is important to protect the dermal stem cells and to maintain their activity. Plant stem cells to protect human dermal stem cells The activity of stem cells is regulated by specific compounds, epigenetic factors among others. Plant stem cells also have such epigenetic factors. In-vitro studies demonstrate, that an extract made of argan stem cells has a positive influence on the activity of human dermal stem cells.

32 PhytoCellTec TM A new assay to measure dermal stem cell activity In order to evaluate the activity of PhytoCellTec Argan on dermal stem cells, a stable human dermal stem cell line was used as a new test system: stem cell activity is assessed based on the expression of the stem cell marker Sox2 (a key regulator of pluripotency in dermal stem cells) and on the property of dermal stem cells to grow in three-dimensional spherical colonies. These two characteristics are only expressed by vital dermal stem cells. Dermal stem cells, which are cultivated in the presence of the Argan stem cell extract show enhanced Sox2 expression compared to the untreated control culture.moreover, when seeded on a culture dish these aged stem cells exhibit a better ability to form 3-D spheres than the untreated control. Thus, PhytoCellTec Argan helps the dermal stem cells to maintain their stemness even after long-term cultivation.

33 PhytoCellTec TM PhytoCellTec Argan improves the sphere-forming-efficacy of dermal stem cells. Improvement of the extracellular matrix structure reduction of wrinkle depth The dermal tissue structure of forearms from volunteers was visualized by ultrasonography before and after treatment with PhytoCellTec Argan. In the ultrasonographic images the dermis appears in bright colours, whereas the so-called subepidermal low echogenic band (SLEB) produces dark patches. This SLEB is the result of a disrupted architecture of the dermis and commonly found in aged and photodamaged skin. Results clearly show, that a 56 days treatment with PhytoCellTec Argan reduces the SLEB and significantly improves the dermal tissue density. Our addtional brochure shows ultrasonographic images made from a women s thigh, where the dermis is depicted in bright colours and the hypodermis shows up in black. The irregular dermis-hypodermis junction is caused by adipose tissue protruding into the lower dermis. This is a typical image for cellulite. A 56 days treatment with PhytoCellTec Argan shows a tightening of the dermal tissue, leading to a much smoother dermis-hypodermis junction.

34 PhytoCellTec TM The anti-wrinkle effect of PhytoCellTec Argan was demonstrated in a clinical study with 21 volunteers, average age 49. A cream with 0.4% PhytoCellTec Argan was applied to the crow's feet area around the eye over 56 days. The results reveal a reduction in wrinkle depth by remarkable 26 %. PhytoCellTec Argan is the first active ingredient capable of both protecting and vitalizing human dermal stem cells. It speeds up skin renewal through the activation of stem cells in the epidermis. The plant Symphytum officinale (comfrey) Symphytum officinale has been used as a medicinal plant for many centuries. The name symphytum derives from the Greek word symphyo which means to grow together and phyton which means plant. In ancient Greece, physicians used symphytum, to treat bone fractures and accelerate wound healing. Nowadays, it is still notably used externally for wound healing and inflammatory and traumatic lesions of muscles and joints. Epidermal stem cells and their function Epidermal stem cells that are located at the base of the epidermis continuously divide and differentiate into tissue cells, whereas old cells are shed on the skin surface. This constant renewal is important for the quality of the barrier and ensures that the skin remains smooth and even.

37 PhytoCellTec TM However, as we age, this renewal process slows down considerably. This is because epidermal stem cells also age and their capacity to produce new cells decelerates. This leads to a thinner, dry and rough skin that in turn results in a dull complexion. Aging of skin stem cells can be prevented Isolated human epidermal stem cells were cultured in a normal cell culture medium that was rich in nutritional and growth factors and which imitated the environment in a young skin. These stem cells were able to form a nicely stratified epidermis with high hyaluronic acid (HA) content. This result was then compared to epidermal stem cells that were cultivated in a specifically designed cell culture medium, which mimicked the environment in an aging skin ( pro-aging medium ). Epidermal stem cells that grew in this medium showed a reduced ability to proliferate. The epidermis grown in the pro-aging medium was much thinner, as well as having a lower HA content and less cell layers.

38 PhytoCellTec TM However, epidermal stem cells that were cultivated in the pro-aging medium treated with Symphytum stem cell extract were able to form a thicker and more compact epidermis with an increased HA content as compared to the pro-aging control. This result demonstrates that the Symphytum stem cell extract helps the epidermal stem cells in the pro-aging environment to maintain their ability to build new tissues during the aging process. Increase in both skin smoothness and skin renewal time In our studies, we also measured the effect of PhytoCellTec Symphytum on the renewal time of the epidermis. Twenty women aged between 40 and 60 applied a cream that contained PhytoCellTec Symphytum twice daily over a period of 56 days. The results revealed a reduction in skin renewal time by a remarkable 8% as compared to the placebo and this was accompanied by an improvement in skin smoothness of approximately 12%. Thus, PhytoCellTec Symphytum rejuvenates the epidermis from the innermost layer and ensures that the skin appears both smooth and even.

Only stem cells can divide over the course of a person s life and create new skin cells, continuously renewing the epidermis. As we age, the vitality and number of skin stem cells decreases, the epidermis takes longer to renew itself and wrinkles form. What is the difference between embryonic and adult stem cells? Embryonic stem cells are pluripotent cells derived from the inner cell mass of an early-stage embryo. They have the potential to develop into any type of cell in the body. Adult stem cells are responsible for supplying the necessary replacement cells in adults. They have been identified in over 20 organs and tissues. Unlike embryonic stem cells, adult stem cells can only generate cells of the organ in which they are found. They are therefore referred to as multipotent.

47 PhytoCellTec TM How was the effect of the stem cells tested and what tests were conducted? The effect of PhytoCellTec active ingredients was tested on cultures of human epidermal stem cells. The tests showed that the active ingredients can maintain the stem cells colony-forming efficiency longer even if the cells were stressed through exposure to UV light. Tests on the 3D epidermis model showed that even aged skin stem cells can form a complete epidermis when PhytoCellTec Malus Domestica is applied.
