5 PREFACE Saying goodbye is difficult and at the time of death, the pain of parting may be complicated. Yet, the way we say goodbye can help in healing. Rituals of goodbye vary from family to family, culture to culture, and faith to faith. When designing a ritual, it is important to find a comfortable way, privately or publicly, to honor the love, commemorate the life and make a commitment to the future. Funerals or memorial services are often chosen as a way to create a public goodbye. These rituals offer a way to recognize the death, express faith and hope, and celebrate the life of the one who has died. Today, many families choose to personalize this process by encouraging all members, including children, to participate in whatever way is comfortable for them. These ceremonies can be made personal through music, poetry, flowers, photograph collages, videotapes that portray the life journey, or other ways of sharing memories. Releasing (biodegradable) balloons, writing and reading letters to the deceased, placing gifts in the casket, or other remembrances might also be part of the family ritual. Formal or informal settings are equally suitable for a goodbye ritual. Some families choose a mortuary or place of worship. Others prefer a home, outdoor setting, park, lake, or other special place. Rituals are for the living and may help family and friends to begin a process of healing. It is important to remember the value of gathering family and friends, sharing personal memories, laughing, crying and even enjoying favorite foods. Rituals may help loved ones face the death, affirm one another and be reminded that life can be embraced even after such a loss. If assistance is desired to plan such a ritual, Hospice of the Valley s chaplains are available. They can offer assistance, participate in the ritual, or recommend others who would be appropriate. There may be an opportunity to involve the entire family in planning. Your loved one, though critically ill, might find it very meaningful to assist. This can be a beautiful way to converse, comfort and celebrate the life you have shared together. PLANNING GUIDE FOR MORTUARY AND FUNERAL ARRANGEMENTS 1
6 MAKING FUNERAL ARRANGEMENTS After death, funeral arrangements must be made and authorized by spouse, next-of-kin, or other responsible party, even if prepaid. State law requires that the funeral establishment provide the responsible party with: Consumer Guide to Arizona Funerals. General price list before discussing funeral arrangements. Casket price list (casket costs are separate from funeral service costs). Outer burial container price list before discussing purchase of outer burial containers from establishment. You will be asked at time of funeral arrangement to sign a contract called a Statement of Funeral Goods and Services, which lists your selections. Read the contract carefully before signing. The contract includes a professional fee and other itemized charges. Total charges vary widely, so you may wish to compare with other establishments. (Funeral homes are required to provide price information over the phone, if requested.) You are entitled to an explanation of each charge on the contract. You may change your selections before you sign the contract, or you may go elsewhere. Note: Funeral establishment services, fees and quality vary so you may wish to visit several establishments before you contract. Save your contract and related documents. Funeral establishments may charge in advance for certain items (cash advances) that they purchase from a third party on your behalf. These items may include cemetery or crematory charges, obituary notices, death certificates, flowers, clergy, musician honoraria and police escorts. State law prohibits charging more than the actual cost of these items without informing you that there is a service or handling fee. CEMETERY REQUIREMENTS Cost may include cemetery plot, opening and closing of grave, grave liner or vault, and memorial or marker. Cemeteries usually require a minimum container or casket and an outer protective vault. Options include earth burial, entombment (placement of casket in a building called a mausoleum), or placement of cremated remains in a columbarium niche (a room or space in a mausoleum). For each option offered, cemeteries may have specific requirements concerning type of memorial or marker and minimum casket or container requirements. Check with the cemetery of your choice to determine its requirements and related costs. 2 HOSPICE OF THE VALLEY
7 CREMATION REQUIREMENTS Cremation is performed by placing the deceased in a combustible casket or container that in turn is placed in a cremation chamber and subjected to intense heat/flame. Bone fragments and dust are brushed from the chamber after cremation; however, it is impossible to remove all of the cremated remains. Because some dust and residue always remain in the chamber, there may be an inadvertent or incidental co-mingling of residue from previous cremations. This also may occur as a result of mechanically processing cremated remains. Cremations must be authorized by the County Medical Examiner to ensure that any evidence of criminal activity is not destroyed. The mortuary will arrange this. State law requires that the spouse, next-of-kin, or other person with legal authority sign a form authorizing cremation. This person is called the authorizing agent. If there is no spouse, most funeral establishments require written consent of all next-of-kin (children or siblings) sharing the same relationship to the deceased. A legally competent adult may prepare a written statement directing the cremation or other lawful disposition of his or her remains. This written statement may, but need not be, part of the competent adult s will. The legally competent adult must sign and date the document in front of a notary public, and prepay or provide financial arrangements so a crematory, cemetery or funeral establishment can execute the adult s wishes as expressed in the document. If such arrangements are completed by a competent adult prior to his/her death, the crematory, cemetery or funeral establishment may not need to obtain consent from next-of-kin. Unless arrangements are made with the funeral establishment, the responsible party is solely responsible for disposition of the cremated remains. Cremated remains should be collected upon notice of availability. The crematory authority may dispose of the remains in a legal manner 120 days after the cremation or after agreed to pick-up date. Cremated remains may be buried, entombed, placed in a niche, scattered over private land with permission of owner, over public property (may require permit), or remain in the family s possession, usually in an urn (wood, marble, or metal container). 9 Mortuaries are required by law to provide an itemized list of all costs before contract with mortuary is signed. Please be aware that mortuary costs are separate from burial and cemetery costs. Funeral Costs to Consider Casket: Caskets may be purchased from the mortuary, or from casket discount stores. See Yellow Pages for list of these outlets. Urn: Urns may be purchased from the mortuary, or purchased privately and supplied to mortuary. Cremains may be divided into more than one container.
