1 The President s Volunteer Service Award is coming to Crestdale Middle School for the school year!!!! What is this award? The President's Council on Service and Civic Participation (the Council) was established by President George W. Bush in 2003 to recognize the contributions Americans of all ages make in their communities through volunteer work. The Council created the President s Volunteer Service Award (PVSA) program as a way to honor Americans that volunteer and inspire others to volunteer. If you are a rising 6 th, 7 th, or 8 th grader at Crestdale, you may work towards earning this NATIONALLY recognized honor for next school year beginning on March 14, There are so many amazing ways to give back to your community through volunteering! The 2015 PVSA accrual period March 14, 2014-March 13, 2015 Volunteer requirements to earn the PVSA per this 12-month period: Bronze level: service hours over 14; hours for volunteers ages 14 and under; Silver Level: service hours over 14; for volunteers ages 14 and under Gold Level: 250 or more services hours over 14; 100 hours or more for volunteers ages 14 and under

2 What can YOU do to work towards earning The President s Volunteer Service Award? Read all rules and regulations carefully. Hours will accrue for the 2015 award period March 14, 2014 March 13, Volunteer in your community! There are two forms required to submit to receive this award: the application cover sheet and the volunteer log form(s). You may use more than one volunteer log form if needed. Document your hours with all required information on volunteer log form. It is recommended that you obtain the signatures on the volunteer form as you participate in various volunteer opportunities. Forms are due by April 1, o Turn in your completed application cover sheet STAPLED to your completed volunteer log forms to Catherine Daughtry in the front office at Crestdale Middle School. o Ms. Daughtry will document that all required forms were turned in for review, including the date the forms were received. o Only log sheets with minimum of 50 volunteer hours will be accepted to continue on to the verification process. In previous years, there has been a 2 hour per day limit of hours that count, this is a change for the 2014 school year. Community service credit can only be awarded for hours outside the school day Community service credit CAN be awarded for hours inside OR outside the school day Examples of service credit that may be accrued during the school day includes peer tutoring assigned/supervised by Crestdale staff. A minimum of 15 volunteer hours are required to earn the award. A minimum of 50 volunteeer hours are required to earn the award. Hours for this award accrued August 26, 2013 May 8, This is a change from previous years when hours were accrued June of the previous school year through May of the following school year. (for example, June 1, 2013 May 8, 2014) Hours are accrued March March. For the 2015 award period, hours will be accrued March 14, 2014 through March 13, Forms are due for the school year by May 9, 2014 Forms are due April 1, 2015 for the March 2014-March 2015 volunteer accrual time period.

4 A more detailed look: Examples of WHAT COUNTS towards the Crestdale PVSA COUNTS Volunteering to play an instrument/sing at nursing home. Time spent at nursing home with residents performing, and visiting counts. DOES NOT COUNT Practice time to prepare to volunteer and perform at the nursing home does NOT COUNT towards PVSA hours. Volunteering for Buddy Club, tutoring peers, supervised or directed by teacher or staff at Crestdale, on an individual basis or through the Leadership AE. There can be NO incentive provided monetarily or with grades/extra credit as this is considered payment. Helping a friend or classmate with homework or to study for a test. Participating in 5K for fundraising event either as a runner/walker OR as a volunteer to help out at the event. Time spent raising money for a cause (lemonade stand, etc.) does count towards volunteer hours accrued). Volunteering with youth groups for community projects (Habitat for Humanity, Operation Sandwich, Room in the Inn, etc.) Volunteer work with church, temple, synagogue You may babysit in a faith-based nursery, daycare, women or men s shelter, or community center. Donating money to a cause. Being part of a leadership club, student council at school or as a member of youth group/religious school. Meetings do not count, however, volunteer activities performed as part of leadership positions may count. Volunteer work with a religious organization may count so long as students do not proselytize (to try to persuade people to join a religion, cause, or group). Babysitting for friends or family, or for any compensation. Yard work done for elderly or homebound members of the community CAN count. You may not submit yard work for family, friends, or neighbors.

Signature: Service Hour Rules: 1. Be certain your activity qualifies! Volunteering is the practice of people working on behalf of others or a particular cause without payment for their time and services. Volunteering is generally considered an altruistic activity, intended to promote good or improve human quality of life. Singing in the choir, playing an instrument, participating in sports or dancing are all worthwhile activities, however, their primary focus is skill development rather than being altruistic in nature. If you are unsure that your activity qualifies, confirm with Amy Choffin at achoffin@carolina.rr.com or Margaret Musa at mmusa@carolina.rr.com. 2. List each day with activity performed and time spent. For example, if you are using 20 days for credit, list EACH DAY on a separate line with activities for each day. 3. Maximum hours allowed to volunteer per day for camp counselor, mission trip, etc. is 9 hours per day. Time spent sleeping, eating, socializing, etc. does not qualify. 5. Total all hours. 6. Log must be approved and signed by Youth Director, supervising adult, or sponsoring institution. Parent signatures are not accepted!
