1 AC SANDHILLS 2016/17 Volleyball Acceptance Agreement By accepting a position with an AC Sandhills Volleyball Team, I agree to be committed to AC Sandhills for the 2016/17 travel volleyball season. I understand that it is a full year commitment to the team, and my daughter is committed to playing for AC Sandhills for his/her entire 2016/17 volleyball season. I agree to meet any and all financial obligations/responsibilities for my player to play for their AC Sandhills team. I agree to make all payments according to the payment plan selected. 2 2016/17 FEE BREAKDOWN: Tryout Deposit: If your player makes a roster, $90.00 of the tryout deposit fee will go towards fees. The other $10.00 will pay for the tryout t-shirt given. Breakdown Insurance fee Uniforms NOT INCLUDED Club Liability Insurance Explanation Club Uniforms - to be paid for separately by each player (will be approximately $120-$140, 2 jerseys, 1 shorts, 1 sock, 1 bag) -except developmental teams- one t-shirt only, under $20 Volleyball Administration ACS Player Registration, Scheduling etc. (*USAV registration to be paid by each player separately- up to $55.00 NOT INCLUDED) Coaches Equipment Facility Rental Administration Fees For team Coach New balls, nets, antennas, etc. Facility Rental Fee Club Administration Fees (Office) Tournaments included U12/U13 Developmental teams- 2 or 3 one day events U13-5 region events and one 2 day tournament U14-4 region events and two overnight events U15-4 region events and two overnight events U16-4 region events and three overnight events U17-4 region events and three overnight events U18-4 overnight events (does not include travel expenses) Strength/ Conditioning NEW THIS SEASON -total events subject to change due to unforeseen circumstances U15-U18 fee includes two 30 minute sessions each week (by Sandhills Sports Performance) *USAV Registration: Every player must be registered with the USAV. This may be done online and costs are $45.00 for players ages 12 to 18, when registered before November 1 st. After November 1 st it is $55.00 for all players ages 12 to 18. Youth players ages 11 and younger are $15. This is not covered by the club and is required for all participating players.

3 2016/17 AC SANDHILLS Payment Options: Plan 1: Three Monthly installments starting November 14 and ending on January 15, 2017 Plan 2: One (1) Payment at Commitment Night. Payment Plan 1: (All numbers are in $) Three monthly installments starting November 14, 2016 and ending on January 15, 2017 All payments will be due on the 15 st of each month. If AC Sandhills has not received payment by the 30 th, we reserve the right to charge the given credit card account. There will be a fifteen (5) day grace period allowed by AC Sandhills for checks. If a payment is not received after that time, AC Sandhills reserves the right to charge the amount owed to the following credit card given.

I agree to pay all of my fees on time and in full based on the payment plan I choose below. I agree and give AC Sandhills the right to charge my credit card account I have willingly provided below in the case that I have become default in making my players payments. I understand that the AC Sandhills Volleyball Season is a full year commitment, and by signing this document therefore assure AC Sandhills that I will pay my dues in full regardless of circumstance. If for any reason I have to pull my daughter from the AC Sandhills Volleyball Program, before or during the 2016/17 Volleyball Season, I understand that AC Sandhills will retain a $ administration fee to provide for the loss of team income due to my leaving. If fees already paid do not amount to $ due to my selected payment plan, I will pay the remaining amount up to $ in order to be released from my club financial responsibilities upon agreement by AC Sandhills Executive Director and Board of Directors. I agree that if my daughter is found in default of their fees, that they will not be able to participate in any club activities. There will be no further use, unless verified by the card holder, then charging fees upon the stipulations of this document. If AC Sandhills or any affiliated organization illegally charges, transfers or deducts money from your credit card or accounts without the consent of the above card holder, then legal action will be recommended to the card holder as well as taken by AC Sandhills against the violator in question.
