Depending on your desired level of coverage, you can choose a basic or a comprehensive service plan. Both include an annual tune-up of your heating system and oil tank insurance based on a pre-qualifying tank inspection. The service plans auto-renew annually in September at the existing level of coverage and can be cancelled at any point. 4 Budget Plan Automatic Oil Delivery We offer a payment plan that will make things easier for you by spreading your heating costs into smaller, predictable payments. We will track your usage and make a delivery to your home when you need it, for the same per gallon price and we will offer a discount for payment within 10 days of delivery. PAY AS YOU GO BUDGET PLAN We multiply your estimated annual fuel usage by this year s projected price and split that total into equal monthly payments to minimize high payments in the winter months.

5 Comfort Plan - $269 Annual Tune-Up Maintaining and tuning up your oil burner annually increases efficiency of your system and provides fuel savings of up to 10%. Regular maintenance affects the life span of your equipment and provides a check on safety and operating controls. During our annual tune-up we will: Brush and vacuum heat exchanger, burner housing, and chimney pipe Change oil filter cartridge, 1 standard air filter, nozzles, and fuel pump strainer Flush oil line Clean electrodes, lubricate motors, and check and adjust fan belt Perform combustion efficiency test TankSure Program BEST FOR WARM AIR HEATING EQUIPMENT (Participation is based on a pre-qualifying inspection of your oil tank.) For homes with above ground oil tanks, ultrasonic testing equipment is used to measure the thickness of your oil tank and to alert you when you should consider a tank replacement. 7 Peace of Mind with TankSure TankSure Program The TankSure Program is an option for homeowners with above ground oil tanks, offering a proactive approach toward oil tank replacement. Oil tanks typically corrode from the inside out, making it almost impossible to visually monitor the integrity of a tank. We use ultrasonic testing equipment for measuring the thickness of your oil tank, and alerting you when you should consider a tank replacement. The program helps to protect your home, your property, and our environment by doing the following: Provide an annual inspection Monitor corrosion of your oil tank Provide $1000 toward replacement of your tank Minimize unexpected oil tank leaks Participation is based on a pre-qualifying inspection of your oil tank.

8 Terms & Conditions General Conditions New service plan coverage is contingent on inspection of the heating system by Lombardi Oil & Gas. Lombardi Oil & Gas reserves the right to refuse to perform any service or repair work on equipment deemed unsatisfactory. Service performed that is not explicitly listed on the plan will be charged at the prevailing rate. Lombardi Oil & Gas reserves the right to cancel the plan upon 10 days written notice. Customer's account must be in good standing. Customer agrees not to purchase fuel oil from other sources. It s the customer s responsibility to arrange for annual tune-up. Reminder cards are mailed once per year. Plans auto-renew annually in September at existing level of coverage unless opt out notification is received. All plan payments are due within 30 days of billing to remain active. All non-emergency service covered by plans is scheduled during normal business hours. Exclusions This agreement does not cover replacement of a complete boiler, furnace, burner, hot water heater (indirect or oil-fired), mechanical chimney (power venter), tankless coil, radiant heat, humidifier or piping, baffles, oil lines, oil storage tanks, auxiliary lift pumps, electric wiring, domestic plumbing, draining of expansion tanks and purging of radiators, obsolete equipment, neglect, upgrades necessary due to government regulations,or any parts or services not explicitly mentioned in the agreement. Except to the limits of protection under the TankSure Program for those customers enrolled in such program, Lombardi Oil & Gas is not liable for any environmental pollution or pollution cleanup due to tank rupture, oil line leak, or any contamination resulting from the oil distribution system. Other Conditions Lombardi Oil & Gas will not accept responsibility for the loss of heat in any unoccupied buildings, for delays or damages resulting from fires, accidents, power interruptions, water in basement, water in storage tank and/or lines, labor difficulties, acts of the Government, flood, war, hurricanes, earthquakes, any acts of God, or other causes beyond our control.
