PO BOX 316 HAMMOND, OR SCHOLARSHIP APPLICATION PACKET Warrenton High School Scholarships, Inc. was founded on May 12, 1994 for the sole purpose of providing scholarships to graduates of Warrenton High School. Over $1,000,000 has been awarded to over 300 Warrenton graduates since the creation of Scholarships, Inc. to help students continue their education. To be considered for a scholarship, a student: 1. Must be a Warrenton High School senior. Eligibility 2. Must have made specific plans for education beyond high school consistent with educational goals. 3. Must plan to attend an accredited or licensed educational/professional/technical institution as a fulltime student. 4. Must be willing to be interviewed by the selection committee. Application Packet To apply for a scholarship, you must submit all information requested in this packet, including (in this order): 1. A cover sheet that includes your full name and a recent picture 2. The completed General Information sheet (page 3 of this packet) 3. A current copy of your most recent high school transcript 4. The Scholarship Checklist (page 4 of this packet) 5. An essay presenting your specific educational goals beyond high school and how you plan to finance your education. Include how this scholarship will assist you with your academic endeavors and how this scholarship would help you reach those goals. This essay should be no more than two pages. 6. An essay stating how you have prepared yourself academically for success in college. This essay should also be specific and include information about activities in which you have participated in high school and the community as well as any awards, honors or recognition you received during high school. If you are a transfer student, please include information from other high schools attended. This essay should be no more than two pages. 7. Any additional essays requested for certain scholarships offered within this packet. 8. Remaining documents: FAFSA verification, letters of acceptances from colleges, etc. See page 3. 2 In addition to the above submitted documents, you must obtain three completed recommendations that will be added to the application packet by WHS office personnel. Recommendation Forms are available in the Senior File at WHS. 1. Recommendation forms are to be returned directly to the WHS office by the person completing the form (due to confidentiality) so they will not be included in the application packet you submit. 2. It is your responsibility to insure that the recommendations have been returned to the office before the established deadline. General information relating to the application packet 1. All submitted documents must be typed or computer generated and must be double-spaced using Times New Roman, 12-point font. 2. Sixteen double-sided copies of your application package, including all documents listed above, must be submitted to the WHS Office on or before 3:00 on April 3, No folders or slip covers allowed. 3. Please make your copies somewhere other than the high school. 4. Only candidates with completed applications, submitted by the due date will be invited to interview. 5. Please be aware that there is competition for all scholarships. Applying and interviewing for a scholarship is not a guarantee that a scholarship will be awarded to you. 6. Interviews will take place on April 17, You are required to dress appropriately. 7. You need to plan ahead when requesting recommendations. You may not be eligible for all scholarships. Also, please be aware that some scholarships have additional application requirements and guidelines. DONNIE ADAMS MEMORIAL SCHOLARSHIP - $500 Scholarship Awarded in memory of Donnie Adams, selected by the Adams Family Considerations for community involvement, school involvement, college/career plans. (No GPA requirement) BRUCE & BEVERLY BOBEK SCHOLARSHIP - $500 Scholarship Awarded on behalf of Angel Medical, selected by the Scholarships, Inc. Committee This scholarship was established in The general eligibility criteria of the scholarship application apply. ASTORIA CLOWNS SCHOLARSHIP - $500 Scholarship Awarded on behalf of the Astoria Clowns, selected by the Scholarships, Inc. Committee This award was established in 1999 in memory of Jimmy Welch. The recipient must attend Clatsop Community College on a full-time basis. He or she must have some community and school involvement. There is no GPA requirement. COLUMBIA BANK SCHOLARSHIP - $750 Scholarship Awarded on behalf of the Columbia Bank, selected by the Scholarships, Inc. Committee This scholarship is intended to provide recognition to an outstanding student who has excelled academically and has been exemplary as a role model of good citizenship in school and in the community. The recipient must have a minimum GPA of 3.0. The student need not attend a four-year school, but must be working toward a four-year degree at an accredited institution. The award is intended to be used toward the first year of college education and will not be limited to tuition. DICK & HARRIET BALDWIN SCHOLARSHIP Four-year Scholarship Scholarship amount varies Awarded on behalf of the Gateway Masonic Lodge #175 and is selected by the board of directors of the scholarship fund of the Gateway Masonic Lodge. Dick and Harriet Baldwin established these scholarships through a bequest. They were teachers at WHS and it was their desire to help graduates of Warrenton High School further their education. Recipients must have attained a GPA of 3.0 or better, excelled in citizenship, participated in school activities, been involved in community service, and displayed good leadership qualities. These scholarships may be used at any accredited two or four year institutions of higher learning and are not limited to colleges and universities in the state of Oregon. To renew a scholarship, the recipient must present evidence that he/she has been a full-time student and has maintained a GPA of at least 2.5. BAYSHORE ANIMAL HOSPITAL - $1,000 Scholarship Awarded on behalf of the Bayshore Animal Hospital, selected by the Scholarships, Inc. Committee This scholarship, established in 2002, is available to a senior seeking a career in veterinary medicine or any animal related field. The application must include a 1000 word essay answering the question; How will my education benefit the welfare of animals? The award may be used at any four-year degree institution, two-year community college, or vocational school of the recipient s choice as a full time student. A minimum GPA of 3.0 is required. BioOREGON PROTEIN INC. SCHOLARSHIP - $1,200 Scholarship Awarded on behalf of BioProtein, Inc., selected by the Scholarships, Inc. Committee The general eligibility criteria of the scholarship application apply. DORIS BULLETSET MEMORIAL - $500 Scholarship Awarded in memory of Doris Bulletset, selected by the Bulletset Family Student must be planning on attending Clatsop Community College. 6 BARB AND JEAN CAMP MEMORIAL SCHOLARSHIP - $500 Scholarship Awarded in memory of Barb & Jean Camp, selected by the Scholarships, Inc. Committee Seniors may apply for this scholarship. No minimum GPA requirement. The main criteria are good character, pleasant personality and school spirit. Student must be enthusiastic about school and representing it. This award may be used at any four-year degree granting institution or two-year community college of the recipient s choice. MARK & JOHNNY DAVIS MEMORIAL SCHOLARSHIP - $1,000 Scholarship Awarded in memory of Mark and Johnny Davis, selected by the Davis & Burkhart families The Davis and Burkhart families established this scholarship in 2005 in memory of Johnny and Mark, who were WHS Alumni and beloved brothers. A senior with a minimum of a 2.5 GPA may apply. Preference may be given to a student who has participated as a wrestler during their high school career. DEBBIE DUNN MEMORIAL SCHOLARSHIP - $1,500 Scholarship Awarded in memory of Debbie Dunn, selected by Scholarships, Inc. Committee This scholarship was established in 1977 in Debbie s memory. A senior with a minimum GPA of 2.0 may apply and renew that application again the following year. The award may be used at any four-year institution, two-year community college, or vocational school of the recipient s choice. KEITH DYER MEMORIAL SCHOLARSHIP - $500 Scholarship Awarded in memory of Keith Dyer, selected by the Dyer family This scholarship was established in 2006 in memory of Keith Dyer, a community and school leader, this scholarship will be selected by the Dyer Family. There are no specific criteria for this scholarship. DR. LEINASSAR HEALTH SCIENCES SCHOLARSHIP - $300 Scholarship Awarded on behalf of Dr. Leinassar, selected by the Scholarships, Inc. Committee This scholarship was established 1994 to provide financial assistance to worthy graduates who wish to continue their formal education in the sciences with a special interest in the healing arts. Financial need is not a determining factor in selecting a winner of this scholarship. Applicants must have made specific plans for majoring in the sciences or healing arts. LUM S AUTO CENTER SCHOLARSHIP Scholarship Awarded on behalf of Lum s Auto Center, selected by Lum s Auto Center This award was established in This scholarship is intended to provide to students who have excelled academically, made a difference in the community and are role models of good citizenship and leadership. The recipient must be a graduating senior with a minimum GPA of 3.0. Lum s Auto Center will award two scholarships. The student need not attend a four-year school, but must be working toward a four-year degree at an accredited intuition. Lum s is committed to making our community a better place for all of us. ROY E. BOYLE MEMORIAL SCHOLARSHIP - $2,000 Scholarship Awarded in memory of Roy E. Boyle, selected by the Scholarships, Inc. Committee This scholarship is available to seniors and will be awarded based on character, values, financial need and community involvement. SANDY ADAMS SOFTBALL SCHOLARSHIP - $500 Scholarship Awarded in memory of Sandy Adams, selected by the Scholarships, Inc. Committee The recipient will be the graduating senior with the highest cumulative GPA, who was an active member of the junior varsity of varsity softball team in good standing for three or more years, and who has earned and received a varsity letter for WHS softball during two or more years. Enrollment as a resident student in an accredited two year or four year college, university or vocational school in the United States is acceptable. JARROD THEBIAY MEMORIAL SCHOLARSHIP - $1000 Scholarship Awarded in memory of Jarrod Thebiay, selected by the Thebiay Family This scholarship was created in 2002 in memory of Jarrod Thebiay. The recipient must be out-going, pleasant and have participated in sports for at least one year. There is a 2.5 GPA requirement. Financial need is not a determining factor in selecting a winner of this scholarship. The award may be used at any four-year degree institution, two-year community college, vocational school or accredited apprentice program of the recipient s choice as a full-time student. 7 WARRENTON MINI-MART SCHOLARSHIP - $500 Scholarship Awarded on behalf of the Warrenton Mini-Mart, selected by Don Patterson The general eligibility criteria of the scholarship application apply and Mr. Patterson says he has to like you. WARRENTON SUNRISE ROTARY SERVICE ABOVE SELF SCHOLARSHIPS Two $1500 Scholarships Awarded on behalf of the Warrenton Sunshine Rotary Club, selected by the Scholarships, Inc. Committee Scholarships for post-secondary education/training will be awarded to seniors who have volunteered in giving service to others, who have a minimum cumulative GPA of 2.5, and who plan to pursue a technical/vocational career. Examples of technical/vocational careers include, but are not limited to, automotive, plumbing, electrician, construction, nursing/medical technician, secretarial, and commercial fishing. WARRENTON VOLUNTEER FIREFIGHTER S ASSOCIATION SCHOLARSHIPS Scholarship amounts vary Awarded on behalf of the Warrenton Volunteer Firefighter s Association, selected by the Firefighter s Association These grants will be awarded to two seniors with minimum GPAs of 2.5. Applicants must be in the fire science or medical field to apply. The award may be used at any Oregon four-year degree institution, two-year community college, or vocational school of the recipient s choice as a full-time student. The Warrenton Volunteer Firefighter s Association will select the recipient. WARRENTON HIGH SCHOOL SCHOLARSHIPS, INC. Committee These scholarships were established in the spring of This foundation was the first non-profit corporation scholarship program for Warrenton High School. Designed to attract and recognize outstanding students who have demonstrated high academic achievement (minimum GPA 3.0), service to school and community, and leadership qualities, and who wish to continue their formal education. Recipients are also encouraged to contribute to the scholarship fund in the future. WARRENTON HIGH SCHOOL SCHOLARSHIPS, INC. FOR CLATSOP CC - $1,000 Scholarship Awarded on behalf of Warrenton Scholarships, Inc., selected by the Scholarships, Inc. Committee Students must have a cumulative GPA of 2.5. Preference is given to students with a Junior/Senior year GPA of at least 2.5. With your application, an additional 250 word response titled, Why I want to go the Clatsop Community College.
