2 Little Miss/Mister Halloween Fest 2016 General Information and requirements Age Groups: 4-6 & 7-8 years old and not older than 8 on October 30 th, Halloween Fest Queen & 1-4 runners up in both age groups, one Halloween Fest King chosen from each age group. Register by mail or sign up at Children s Halloween Fest stage area at the Voorhees Town Center (the Mall) 2120 Voorhees Town Center, Voorhees NJ from 1pm to 2pm. Contestants must be from New Jersey When: Where: Sunday, October 30 th at 2:00pm Voorhees Town Center, (Inside the Mall) Pageant will be held on main stage. Theme: Halloween colors, semi-formal to formal attire. Dresses and suits can have spider webs, witches gowns, dressy costumes and accessories are encouraged. A fun themed pageant. Remember this is still a semi-formal to formal pageant. Rich colors of purple, orange, dark greens, rusts, burnt reds and golden yellows. Remember to include something with the outfit that reflects the fun Halloween season. Sponsorship Fee: $15.00 per contestant. Pay at the event or prepay by mail. Pre-Registration: Simply fill out page #2 of this application and mail with sponsorship fee of $15 to B & K Enterprise, P.O. Box 925, Millville NJ You will receive notification by letter that your registration has been received and accepted. Upon arrival to the pageant on Oct. 30th you can pick up your child s assigned number. The girls/boys are brought up on stage in the order of the numbers assigned. A stage person will escort contestants on and off the stage. Registration at the event: Registration can also be done at the event on October30th from 1pm to 2pm at Voorhees Town Center (the mall). Check in at the main stage area. please bring a check or money order only payable to B & K Enterprise. Each contestant receives a ribbon and goody bag for participating. Judging: Will consist on scoring of the dress/suit, Halloween themed, hair and accessories, overall appearance, behavior on stage and more. This is a fun filled pageant and a great way for families to spend the day with your child and celebrate the Halloween & fall season. Sponsors name will be read as contestant comes out on the stage. Any contestant that appears afraid, moms may walk them out on to the stage. Prizes: Sashes, Crowns, Pageant Pins, Trophy, gift cards & baskets. Ribbon and goody bag for all contestants.

As a parent of legal guardian of a participant in the 2016 Children s Halloween Fest Little Miss/Mister Pageant, I assume the full responsibility for any injury, damage, or loss which may be sustained as a result of my child and myself participating in any manner, associated with this program. I further agree to waive release and relinquish any and all claims which may result from participating in the Little Miss/Mister Pageant, to be held on October 30th, 2016, at the Voorhees Town Center (The Mall) 2120 Voorhees Town Center, Voorhees NJ. I also agree to release the B & K Enterprise, Always Positive Group, Voorhees Town Center management, the staff, sponsors, vendors, volunteers, or helpers from all reliability during the 2016, Children s Halloween Fest and Pageant. I also give B & K Enterprise to take photos of my son/daughter/contestant for marketing, advertising, and websites of Always Positive Group, B & K Enterprise.
