3 Recommendation Letters Three confidential letters are to be mailed directly by the recommenders to the Admissions & Registration Office. It is your responsibility to provide the enclosed Letter of Recommendation forms to the persons whom you select. These letters should include two letters from professors of educational institutes, a letter from the applicant s church (pastor, Board of Deacons, congregation, or equivalent persons or bodies). Application Fee A non-refundable application fee of HK$1,000 or US$200 (for international application) should accompany your submitted application. The payment must be made by crossed check payable to the Hong Kong Baptist Theological Seminary. Cash is not accepted. 8. The Definition of Personal File Information 1.1 All information that is submitted at the time of application, for example, information on my personal background, transcripts or other academic records, entrance or language examination records, a personality assessment, and all recommendation records. 1.2 All assessments and recommendations made by the Director of Graduate Studies, the Graduate Studies Committee, and the Academic Board during the application and admission process. 1.3 All records, including academic, financial, housing, personal conduct, and any other performance and personality assessments accumulated during the period of the Doctor of Theology degree study within the Seminary. 1.4 All correspondence with the Seminary after graduation. 2. The Handling of Personal File Information 2.1 The ownership of all documents that were submitted by me at the time of my application to the Doctor of Theology program will be transferred to the Hong Kong Baptist Theological Seminary. The documents will not be returned to me, whether I am accepted or not. 2.2 The Seminary has the right to access or add other documents to my personal Doctor of Theology file that are related to the operations of the Seminary, such as those that deal with application; admission to student status; registration; a change of an area of Concentration; advancement to candidacy; course grades; disciplinary matters; my use of the Library; applications for scholarships, financial assistance, or other Seminary awards; Seminary recommendations on my applications for scholarships available outside the Seminary; and records of all other applications and activities, such as the occupation of Seminary housing, with the Seminary. 2.3 If any external institution or organization inquires about information within my file other than that concerning my status as a student, enrollment year, and degree pursued, the Seminary will always seek my consent before disclosing such personal information. 2.4 I agree to waive my right, without qualification, to access, evaluate, or possess a copy of the recommendations made by my selected personal references during my application process.
