(ProTeX) has completed a prerenovation survey of roofing materials at 2636 South Wilson Street. The survey was performed by Mr. Charles Helms, Asbestos Emergency Response Act (AHERA) certified building inspector. During the survey, four (4) homogeneous suspect asbestos containing material (ACM) areas were identified. Laboratory results indicate that one sample of mastic (Sample # 2636 C-1), along the top edge of the metal flashing, contains 5% chrysotile asbestos in the black layer of sealant. The sample was collected on the west wall along the top edge of the metal flashing. This material is considered Category I Non-friable ACM. We appreciate this opportunity to be of service. (ProTeX) has completed a prerenovation survey of roofing materials at 2636 South Wilson. Scope of the survey was to identify asbestos containing material (ACM) that will be disturbed during the planned 112,860 square foot roof replacement of the commercial warehouse located at 2636 South Wilson Street, Tempe, Arizona. During the survey, four (4) homogeneous areas of suspect ACM were identified. 2.0 ACTIVITIES The survey was performed on April 30, 2015 by Mr. Charles Helms, Asbestos Emergency Response Act (AHERA) certified building inspector. Twelve (12) bulk samples of suspect building materials were collected from the roof of the commercial warehouse building. Samples were submitted under chain-of-custody to EMLab P&K, Phoenix, Arizona for analysis by polarized light microscopy (PLM) (Environmental Protection Agency method 600/R ). 3.0 FINDINGS Federal regulation defines any building material, containing greater than one percent (1%) asbestos, as determined by PLM, to be asbestos containing. A summary of findings, from the sample activity conducted on April 30, 2015 at 2636 South Wilson, follows. There is a possibility that additional suspect ACM may be found during roof demolition. It is recommended that any additional suspect materials, not sampled or assessed in this report, be sampled and analyzed prior to disturbance or should be considered ACM and managed as such. 4.0 LIMITATIONS ProTeX the PT Xperts, LLC warrants that the findings contained herein have been prepared in general accordance with current regulatory guidelines and accepted professional practices as applied by similar professionals in the community at the time of its preparation. Changes in practices or in applicable, relevant, and appropriate regulations cannot be anticipated and have not been addressed within this report. Results reported herein are considered sufficient in detail and scope to determine the presence of ACM. This report is a survey only, and not intended as an abatement specification.
