2 ESS Mission ESS Support Services is a not-for-profit agency committed to supporting seniors in their desire to remain in their own homes and community. ESS Vision We envision a community of independent seniors sustained through excellence of operations, enthusiasm and continual innovation in the delivery of community support services. 3 Report of the Chair and CEO In our thirty-first year of operation, we have successfully continued to expand the reach and the variety of services we provide to our clients. In our efforts to fulfill our mission, and to meet the ever-growing community of seniors and caregivers in Etobicoke, we continue to hold ourselves to our commitment of maintaining the highest standards of excellence. The following highlight some of the many accomplishments ESS has achieved over the past year: In June 2013, ESS was accredited with Exemplary Standing; a status awarded to those who have met the highest level of performance standards of the accreditation program. Our Board of Directors, staff and volunteers were commended by Accreditation Canada for using accreditation to improve the quality and safety of services offered to our clients and community; Through the priorities set in our Strategic Plan, our programs are sensitive and responsive to client needs and preferences including those based on ethnic, spiritual, linguistic, familial and cultural requirements. The ongoing financial support of the GH Wood Foundation has enabled ESS to create a sustainable self-help network in Etobicoke apartment buildings by engaging previously isolated seniors in social, education and recreational activities. These activities promote the independence, good health and quality of life of these seniors. ESS was chosen by the Central LHIN to lead an emergency preparedness project to ensure agencies are better prepared for future emergencies. A cross-agency survey was completed to determine emergency equipment and supply needs. Based on survey results, required equipment was purchased and distributed with the funds provided. Funding for Health & Wellness programs from both the Toronto Central LHIN and the Central LHIN has allowed us to: Provide twelve falls prevention and exercise programs across a broader area of Toronto;
4 Implement the Assisted Living Services for High Risk Seniors to provide care for individuals living in the community who are deemed at high risk and require intensive service to enable them to remain in their community; As leaders of innovative partnerships, the Board Chairs of Community Care Partners (CCP) presented at an Effective Governance Collaboration to Advance Integration seminar to share their knowledge about the benefits of partnership to a variety of health care CEOs and health care leaders from across Ontario; CCP has achieved the following during our three year agreement: Raised funds, marketed services and enhanced community awareness; New business associations were established through the creation of a joint a golf tournament; Researched areas of Client Safety and Emergency Preparedness by utilizing the expertise of a co-op student; Carried out cost-effective joint staff and volunteer training in areas of infection control and computer software. We continue to be proud of our team of professional staff and dedicated volunteers, all of whom recognize the ever increasing and extensive need for quality services to assist the seniors in our community. On behalf of our organization, we would also like to recognize the support, wise guidance and compassion that Donna Cansfield, MPP for Central Etobicoke and her staff have provided to us over so many years.
