The following is a list of community agencies that provide direct services or link stroke survivors and caregivers to other community programs that can assist in recovery from stroke. Though the list may not contain all of the community services related to stroke, each of these organizations can direct you to the appropriate resource or information. Note that there may be additional fees for some of these services. Also, where applicable, you will see references made to the Yellow Pages. It is suggested that you check with your insurance carrier to see if you are eligible for additional coverage. If you are a veteran you may also qualify for assistance from Veterans Affairs Canada (see under Financial Resources). 7 Page 20 Page 1 FINANCIAL RESOURCES It is advisable to contact your NECCAC first when attempting to access community services IN-HOME SUPPORT SERVICES Assistive Devices Program (NECCAC) Canada Pension Plan (CPP) Guaranteed Annual Income System (GAINS) (English) (French) Guaranteed Income Supplement (GIS) Income Security (Involuntary Separation) Old Age Security (OAS) Ontario Disability Support Program Ontario Health Insurance Plan (OHIP) Ontario Works Ontario Trillium Drug Funding Veterans Affairs Canada (VAC) Volunteer Services of United Way (Assist with tax preparation) Ext. 209 with the changes associated with having a stroke. Services may also include financial counseling. NORTH EAST COMMUNITY CARE ACCESS CENTRES (NECCAC) NECCACs provide services funded by the Ontario Ministry of Health and Long Term Care. The NECCAC can help you access the following services: Information and Referral: To assist you in finding community support services in your area. Other services provided to eligible clients: Nursing: Provide nursing services such as health assessment, medication and wound management and teaching. Personal Care/Caregiver Respite: Help with personal hygiene and activities of daily living, e.g., help with bathing. In-home caregiver relief. Stroke Rehabilitation: Services that help you regain independence, e.g., Occupational Therapy, Physiotherapy, Speech Therapy. Information on private rehabilitation services can also be obtained from the yellow pages under Rehabilitation Services. Nurse Practitioner: Provides primary health care to clients 65 years and older who do not have a physician or nurse practitioner and are housebound.
