2 The new challenges resulting from computerization policy of the healthcare sector such as the introduction of electronic medical records, e- prescriptions, registration of visits via Internet, electronic billing and clearing of healthcare services and monitoring of costs of medical treatment and financial standing of a healthcare units. The implementation of such tasks is not possible without the support activities using advanced technologies including IT systems. Introduction The Cardinal Stefan Wyszynski Institute of Cardiology in Warsaw is a one of the largest clinical cardiology and cardiac-surgery centre in Poland with the highest degree of referentiality and its statutory objective is to organize and conduct research and development and implementation of R&D results within the healthcare system. The Centre has an unique organizational and research potential in the nationwide scale, it conducts the specialized clinical treatments and professional services as well as advanced scientific research. To facilitate these tasks the Institute has taken the decision about expansion its IT infrastructure referring to the project named: An integrated IT systems supporting research on cardiovascular disease. Thanks to its broad, more than a decade of experience in this filed, the company Pentacomp SI S.A. was chosen to became a partner responsible for the execution and implementation of Data Warehouse and analytic and reporting solutions (Business Intelligence class). Our competence in this area was gained during the implementation of IT projects for telecom, financial and public market leaders such as AEGON Life Insurance Company, the Agency for Restructuring and Modernization of Agriculture, Post Bank, Credit Information Bureau, BRE Bank, Centre for Human Resources Development, Central Statistical Office, Ministry of Finance, Ministry of Internal Affairs and Administration, T-Mobile. Our offer in the area of Business Intelligence covers following areas: A complex data warehouse development with implementation of ETL mechanisms A supply of analytical and reporting solutions in the scope of OLAP, Data Mining, KPI s and Dashboards A supply of the solutions for business, technical and operational metadata management Data Warehouse maintenance services including monitoring and administration of environments, efficiency optimization, data quality management Goals A delivery of advisory, training services and audits of existing solutions The key goal of the Project was to reach the strategic goals in various areas of the Institute s activity: clinical-medical by the support for the doctors by taking the clinical decisions research&development by raising the scientific potential of the Institute through gaining and proper processing the large
3 medical data scope coming from various sources into one, consolidated research material business & administrative in raising the efficiency of the work of Institute through enabling information allowing the better utilization of Institute s resources and taking strategic decisions by the management and by enabling the precision counting of the technical cost of the medical services production (TKW) Implementation The true measure of individual development such as the Institute of Cardiology is a steady progress in creating a clinical, research and teaching database. In practice, this means the need to develop and implement a dozens of unique medical procedures, a collection of a huge body of clinical presentation of hundreds scientific studies and the preparation and implementation of new therapeutic programs on a national scale while taking care of the economic outcome of the project. These tasks cannot be properly and effectively carried out without the support of modern technologies. The trick is not to have a multiple solutions to support specific business areas but integrating them in a way that allows a multidimensional processing of the collected data. Within the project "Integrated system supporting research on cardiovascular diseases," the company Pentacomp Information Systems based on existing infrastructure and operating systems has designed and implemented the Digital Library for the Institute of Cardiology Medical Cases (CBPM). It is a highly specialized solution for data processing and analyzing (including the conduct of statistical analyzes) stored in Institute s databases such as Hospital System, the Data Archive of Diagnostic Imaging (PACS), the Scientific Research System (SMS), HR and Payroll system as well as the general Accounting System. The library consists of two key components: Challenge Data Warehouse with a set of ETL tools for data integration and improve their quality and tools for the construction and distribution of reports and advanced analytical models, administration and management of metadata, along with software analytics with data mining - data mining Business Intelligence system and its tools for analyzing and reasoning based on data collected in the Data Warehouse, in terms of clinical, scientific and administrative. The only way to achieve the profitability within the area of healthcare services is a dynamic adaptation to the current market situation, a quoting the prices of a health benefits and reduce any unjustified cost sources. One of the most important aspects affecting the economic result is therefore a proper valuation of medical services. The great emphasis put by the Institute of Cardiology to ensure the managers need to get the fullest possible information on the amount and structure of costs associated with the implementation of specific types of medical procedures as well as the cost of medical consultations and visits of patients.

Linking the accounting data from the data warehouse with data from cost and management support the Institute through the implementation of advanced analytical tools. 5 The specificity of health care benefits makes the development of technical diagrams unit production cost of medical services (TKW) is always a complex process and requires a lot of commitment and close cooperation of the contractor and the client. It is necessary to take into account a very large diversity of products manufactured in health care (diagnostic imaging, laboratory diagnostics, research invasive surgery, drug therapy, nursing services, rehabilitation, consultations etc.) and how to implement individual tests and treatments, and therefore the materials used, equipment, specialized medical equipment and medical staff workload. for medical treatment by using different measures (eg. the number of services provided specific category, the number of procedures performed, the number of tests etc.). The software has a dedicated interface for business users who do not need to have a specific programming knowledge and allows them o conduct such activities as editing the allocation keys of costs, modifying the logic of the division of indirect costs, configuration and definition media cost calculation position of reports, configuration and modification dimension of cost for TKW schemes. With the strong involvement of the Institute the Pentacomp s specialists have developed and offered the technical schemes of a unit cost of production which were implemented further as a part of CBPM, after acceptance by the customer. The prepared schemes include: a costs related directly to the performance of medical services - the Institute conducts medical records of technical measures and medications that are stored in the system of Digital Library Medical Cases assigned directly to the related medical service performed, a registration costs not related directly to the medical services; the scheme takes into account the transfer of direct costs, indirect and internal settlements (by centers of costs) that unit costs of a particular medical service; and enable the calculation of a unit costs of medical services by transferring the cost of each cost center
6 Customers Benefits A support for doctors in making the clinical decisions (clinical reasoning) CBPM provides clinicians by another valuable source of diagnostic knowledge. Pentacomp has implemented a mechanism for advanced text search in the base of a research results. It allows the doctors to perform the search using multiple criteria simultaneously in the results of the tests listed in the hospital system and in the form of narrative and descriptive data not related to the execution of specific audits. Clinical operations support system gives the ability to search the unique relationship between disease entities and parameters of the results of diagnostic tests in different categories. It also enables various analyzes relating to medical ordination taking into account the data such as medication, dosage, duration of treatment, the doctor s ordinating and other information regarding the stay and treatment in hospital. It is possible to perform analyzes related to the grouping of patients according to predefined criteria. The patient's physician is able to trace a treatment (medications, tests, results) cases similar to those currently being analyzed by him; the statistical methods for clinical decision-tips with visualization of results are also available. Support for scientific activities (scientific inference) the Institute including the Hospital System along with its entire structure. Standardized and suitably processed data enable a deep analysis supports the research in a basic clinical activity. It is possible, inter alia, to search a database of cases of patients that meet specific criteria. The optimization of the utilization of the resources (administrative inference) The data collected in the Data Warehouse also serves business analyzes in the medical and nonmedical activities of the Institute. Thereby supporting business decisions by the management of the facility. The system allows among others: the verification of the correctness of encoding the services performed by comparing the course of hospitalization and attributed to the settlement with the payer code, including a statistical distribution, performing a sensitivity analyzes of financial results for change of the encoding parameters of selected services in the settlement with the payer, a settlement of costs on a single patient with conducting a various analyzes, broken down into cost centers, an analysis of income or expense of individual organizational units of the Institute including a grouping on cost items and taking into account a time series with a statistical distribution, The Digital Library of Medical Cases collects the data gained from numerous systems operated by a precise calculation of costs of medical services through the appropriate assignment
7 of direct and indirect costs to the service of medical treatment, an analysis of the work of physicians, i.e. medical consultation, commissioning surveys together with the possibility of calculation of the costs generated from different perspectives and consolidation, an analysis related to the grouping of patients according to predefined criteria taking into account the cost of treatment and stay. Summary The implementation of data warehouse and business intelligence system created a unique in nationwide scale a computerized medical facility in which the Integrated Information System serves patients through supporting the doctors in the current medical activity, the scientists in research and development work as well as the managers in the daily management of the hospital and its effective development.
