3 Our Work in Your SHELTERS AND HOMELESS SERVICES RAPID RE-HOUSING Provides services to households experiencing homelessness and those needing temporary assistance in order to obtain housing and retain it. HOUSING CATHOLIC HOUSING SERVICES Provides permanent housing for income-eligible individuals or families at the Monte Cristo Apartments in Everett. Some units are set aside for formerly homeless women with supportive service needs: Monte Cristo AUTUMN LEAF Provides a structured community setting serving chemically dependent women and their children. Residents receive four to six months of intensive case management focusing on life skills, parenting and selfsufficiency. Women must be at least 18 years of age, must be in treatment, or have completed treatment within the past 12 months, and be pregnant or parenting (which includes reunification). CHRONICALLY HOMELESS HOUSING (Whatcom County) (Snohomish County) Provides intensive case management and individualized service plans to support stable permanent housing. Programs are available in Snohomish and Whatcom counties. Must be at least 18 years of age, disabled and chronically homeless. HIV/AIDS HOUSING Moves clients diagnosed with HIV/AIDS from transitional housing to permanent housing and provides case management. Must be at least 18 years of age and diagnosed with HIV/AIDS. INVESTING IN FAMILIES COORDINATED ENTRY (I.I.F) Coordinated Entry provides seamless access to housing with the goal of prevention and rapid re-housing for families. This program seeks to remove many of the barriers faced by clients in finding housing and accessing services, thereby reducing the time families remain homeless and assisting their moving into permanent housing. PREGNANT AND PARENTING WOMEN HOUSING (PPW) A scattered-site transitional housing program which provides goal-oriented intensive case management to maximize housing retention and self-sufficiency for pregnant and parenting women who have a history of substance abuse. Women must be at least 18 years of age, be in treatment, or have completed treatment within the past 12 months, and be pregnant or parenting (which includes reunification). FAMILY AND YOUTH SERVICES CATHOLIC ADOPTION SERVICES OF WASHINGTON STATE or Whether a birth mother decides to parent or place for adoption, CCS provides adoption services throughout Washington State. CHILDREN S MENTAL HEALTH Provides core mental health services that include individual and/or family counseling, medication evaluation/management and coordination with primary care physicians, and case management. Group therapy for children and adolescents is also provided. EARLY FAMILY SUPPORT SERVICES Provides case management, intervention and resource connections to establish a strong family structure and help prevent a child s out-of-home placement.

4 Community FOSTER CARE Provides foster children, from birth to 18 years of age, with short- or long-term out-of-home placement, with resources for medical and dental care, counseling and school. This program strives to reunite children with their biological parents whenever possible and assists those who want to become foster parents obtain foster care licenses and training. LAYETTE SERVICES Provides low-income families with basic newborn necessities. Layettes include bottles, socks, one outfit, sleepers, a stuffed animal and a baby blanket. SENIORS AND PEOPLE WITH DISABILITIES FOSTER GRANDPARENT PROGRAM Seniors provide one-on-one emotional support, mentoring and tutoring for children from preschool ages to 18 years at schools and other sites throughout Western Washington. Income-eligible adults, ages 55 and over, receive monetary stipends for their volunteer hours, typically 20 hours a week. HOME CARE FOR ELDERS AND PEOPLE WITH DISABILITIES CCS provides affordable non-medical assistance to seniors and people with disabilities living in their own homes. Training for caregivers is provided in multiple languages including English, Spanish, Vietnamese, Somali, Russian, Cantonese and Korean. Bilingual staff are available for Korean. RETIRED SENIOR VOLUNTEER PROGRAM (RSVP) Volunteers 55 years or older are placed at work stations throughout Snohomish County, according to the type of volunteer work they enjoy and skill sets they have to share, for example hospitals, food banks, and Senior Centers. VOLUNTEER CHORE SERVICES Volunteers help elders and adults with disabilities remain independent in their own homes. Services are provided at no charge for those individuals who cannot afford to pay for assistance. VOLUNTEER TRANSPORTATION Provides necessary transportation for low-income elders and adults with disabilities to access medical appointments and other essential errands, such as grocery shopping. Also provides transportation to maintain social connections. VETERAN S SERVICES CHRONICALLY HOMELESS VETERAN HOUSING Provides intensive case management and individualized service plans to support stable permanent housing. DISABLED VETERANS TRANSPORTATION Provides door-to-door services for disabled veterans living in underserved areas of Snohomish County to access medical appointments and other essential errands. HOMELESS VETERANS RE-INTEGRATION PROGRAM (HVRP) Provides services to assist in reintegrating homeless veterans into meaningful employment within the labor force. SUPPORTIVE SERVICES FOR VETERAN FAMILIES (SSVF) VETS (8387) Supportive services and homelessness prevention/rapid re-housing assistance for eligible veterans VET CONNECT Volunteer-based program providing outreach to veterans informing them of benefits available through various Veteran Services Programs in the community.

5 ADDITIONAL SERVICES ADDICTION RECOVERY SERVICES CCS Recovery Centers provide a full continuum of outpatient addiction recovery services in Snohomish County for youth and adults and their families. Services are provided in both Everett and Marysville. CATHOLIC IMMIGRATION LEGAL SERVICES Provides quality legal immigration assistance to low-income residents of Washington. Services are provided in multiple languages including Spanish, Somali, Russian, Ukrainian, Belarusian, Kmhmú, Laotian, Thai and Burmese. ISLAND COUNTY PARISH COUNSELING Services include marriage enrichment, family, couples and individual counseling. All services are offered on a sliding fee scale. Parish counseling takes place at St. Hubert Parish on Whidbey Island. JAIL TRANSITION SERVICES Provides case management and counseling for up to three months for individuals with mental health diagnoses exiting the Snohomish County Jail. The program also offers assistance with gaining access to housing, medical coupons and regular medical care and finding employment. Re-Entry Ministries is a volunteerbased mentoring program working in conjunction with Jail Transitional Services to promote a supportive environment for individuals released from incarceration, to assist with their development and to restore their sense of human dignity through linkage to employment, education, vocational training and related services. MENTAL HEALTH SHORT TERM CASE- MANAGEMENT FOR NON-MEDICAID CLIENTS Assists individuals at least 18 years of age with no medical coverage (including Medicaid) at risk for hospitalization for recurring mental health crises in obtaining necessities, such as medical coupons, housing and medication, and referrals for chemical dependency treatment and other types of needed treatment. PROJECT RACHEL HOPE (6473) En Español Project Rachel provides healing and support in a confidential environment for women and men who have experienced abortion. For nearly a century people have entrusted Catholic Community Services and Catholic Housing Services to help children, families, individuals, elderly and people with disabilities in need throughout Western Washington.
