2 American Cancer Society - Central Ohio 870 Michigan Avenue Columbus, OH (888) Administrative Profile: Support services to individuals with cancer and educational programs for the general public.education and support for individuals diagnosed with cancer and their families. Smoking cessation programs and educational programs. Retail "thrift" stores with proceeds going to support research. Mission Statement: The American Cancer Society is the nationwide, community-based, voluntary health organization dedicated to eliminating cancer as a major health problem by preventing cancer, saving lives, and diminishing suffering from cancer, through research, education, advocacy, and service. LEE SLATTER HOLIDAY PROGRAM * Cancer Service Description: Offers an adopt-a-family program for individuals with active cancer and their families. Those in need are matched with a sponsors who will provide clothing, toys or gift cards for the family for the holidays. Eligibility: Franklin County residents with active cancer (adult or child). Must not be receiving other holiday assistance and cannot have used this program before. Hours: Mon-Fri 9 am-5 pm. Intake Process: Application required. Applications will be accepted until December 13. Documents: Will be advised about required documents. Center for Groveport Madison Human Needs PO Box 542 Groveport, OH (614) Administrative [email protected] Profile: Aiding residents of Groveport Madison School District in emergency situations as well as applying for future aid. Mission Statement: To connect people in crisis with resources and to promote cohesive households through community partnerships. Service Description: Seeks donors for the adopt-a-family program. Eligible children in the Groveport Madison School District and their families will receive a coat, hat and gloves, socks and underwear, pajamas, two items from the children's wish list, a box of pantry food items and a $25 gift card for perishable items. Donors will be provided the wish list. Hours: Items can be dropped off unwrapped from 3 pm-6 pm on December 12-13, 2011 at Groveport Zion Lutheran Church. . Notes: Groveport Zion Lutheran Church is located at 6014 Groveport Road. This service is not for families looking to be adopted. 1
3 Central Community House 1150 East Main Street Columbus, OH (614) Administrative (614) Fax Profile: Community center offering social, recreational, health and basic needs assistance. Mission Statement: Community center serving the near eastside of Columbus. Family services includes child care, after school programs, teen groups, tutoring, parent group, older adult programs and limited financial assistance. Neighborhood development activities, employment and senior services. Service Description: Accepts donations of unwrapped new toys or gifts certificates for needy families at Christmas time. If desired, volunteers may sponsor an entire family (pre-selected clients) based on a wish list. Hours: Mon-Fri 9 am-5 pm. Appointment. Notes: This is for volunteers wanting to sponsor a family; not for families wanting to receive toys. Clintonville-Beechwold Community Resources Center 14 West Lakeview Avenue Columbus, OH (614) Administrative [email protected] Profile: CRC is a Settlement House and neighborhood development support center providing social services, advocacy and outreach for families, youth and senior citizens. Major programs include: food pantry, after-school and summer youth programs, social club and activities for seniors, home repairs and maintenance for the disabled and seniors, and senior transportaton services. Mission Statement: To improve the quality of life for all people in the Clintonville-Beechwold community in the best tradition of settlement houses. ADOPT-A-FAMILY VOLUNTEERS NEEDED Service Description: Recruits sponsors to donate holiday gifts and food to low income client families during the holidays. Those interested in sponsoring a family should call for more information. Hours: Mon-Fri 9 am-5 pm. Intake Process: Walk in. 2

Mission Statement: Clinic provides cancer prevention, early detection, home care and emotional support services to cancer patients and their families in Central Ohio, regardless of their ability to pay. Service Description: Offers companies and individuals the opportunity to adopt a family for the holidays. Companies or individuals can either donate money to buy gift cards for the patient and family, or the company or individual can buy, wrap and deliver the gifts to the families. If the company/individual prefers to buy the gifts, the patient s sizes, names, ages, and wish list will be provided. If the company/individual does not want to deliver to the patient s home, Cancer Clinic staff will deliver them. Hours: As arranged. Communities in Schools of Central Ohio 510 East North Broadway Street Suite 4A Columbus, OH (614) Administrative (614) Fax [email protected] Profile: Communities in Schools is a community-based drop-out prevention organization that is helping kids stay in school and graduate. Works with Columbus City Schools, Whitehall City Schools and Catholic Schools to connect children and families to needed community resources so that the children stay in school, improve academic success and graduate. Mission Statement: To surround students with a community of support, empowering them to stay in school and achieve in life. HOLIDAY HELP Service Description: Allows people the opportunity to assist with a family in need. Individuals have the ability to select how many people they would like to "adopt." Communities in Schools will match individuals with an appropriate family. The amount of spending is not determined, although "big ticket" items are discouraged. Gifts must be wrapped and labeled before giving them to staff. Hours: November 1-December 15, It is only for families in Columbus and Whitehall that Communities in Schools has already committed to support. 3
