1 Extreme Adventure Ltd Risk Assessment Activity Street Slide What are the Injury due to inflatable moving in wind /Spectators/Staff Inflatable weighted down at various points along the length of the slide. If on grass the slide is staked to the ground Severity of Risk (S)- 3 Overall Risk (S x L)= 6 MED Observe safe practice in securing Inflatables safely Minor bodily injury to participant Sliders by other participants A one way policy to run so participants are always sliding in the same direction Good observation by staff members at all times MEDIUM Observe safe practice by staff members. Footwear removed prior to sliding. Injury caused due to contact with hard floor i.e. tarmac or concrete Padding installed underneath slide to protect inadvertent contact with hard ground Ensure padding is secure and the slide is not exposed to the ground
2 What are the Injury from objects falling from pockets of participants Empty pockets of loose objects e.g. coins, mobile phones etc Severity of Risk (S)- 1 to remove all loose items from pockets. Suspension injury from loose clothing, jewellery, spectacles or other loose items Ensure loose clothing is secured and dangling jewellery/spectacles or other such items are removed to avoid potential injury Severity of Risk (S)- 1 to remove all loose items from pockets Injuries occurring during unsupervised use/ access Restrict access when not in use Activity is made secure whilst not in use
3 What are the Injuries caused by people wandering onto the activity /Staff Barriers are used to separate the public from the course. This means only users that have been safety briefed. Likelihood of Risk (L)-1 Signage and the set out of site makes it clear where people can or cannot go not suited to the course i.e. being too heavy or too small to ride Clear signage showing the allowable user minimum heights accompanied and unaccompanied are displayed. Signage also dictates the maximum weight of a user. MEDIUM Clear signage and training staff in the clear procedures and policies of the company to maintain a safe operation Over enthusiastic participants Management by Extreme Adventure Staff Severity of Risk (S)- 1 Observe safe practice at all times Jumping off the Slide instructed not to jump off the Slide Staff to be vigilant at all times
4 What are the Larger participants colliding with smaller participants Management by Extreme Adventure Staff. Avoid allowing large participants to be using the activity at the same time as small participants Severity of Risk (S)- 3 Overall Risk (S x L)= 3 Staff to be vigilant at all times Tripping over anchorage points / spare equipment / electrical cable All anchor points must secure and installed as per manufacturer s instructions. All cables must covered to avoid trips etc Daily checklist carried out being hit by another as they slide down Strict control by staff to ensure only one person at a time slides down with an adequate gap between sliders also that the exit point of the slide is clear i.e. other participants standing on the end of the slide MED Constant vigilance by staff The slide has
5 sliding off the end of the slide and onto the ground a pool at the end of the slide which acts as a natural stop. Care should be taken in rainy conditions MED Calculation of Risk Evaluation Constant vigilance by staff Severity (S) Severity of Risk is judged by evaluating the effects of the hazard if the risk occurs. This is evaluated as Minor = 1, Major = 2, Serious = 3 Risk Likelihood (L) The likelihood of the harm occurring is evaluated on the following basis: Unlikely =1, Possible = 2, Likely = 3 Overall Risk Overall Risk is calculated by multiplying the figure for Severity (S) x Likelihood (L). The figure calculated is related to the rate of risk as follows 1 to 3 Low, 4 to 6 Medium, 7 to 9 High Circulation Assessor Management, Staff & Show or Event Organisers Mr Caldwell Date Assessed 3 rd November 2015 Review Date Every 12 months next review 3/11/2016
