Starting with year 2010, The Ministry of Health Public-Private Partnership Authority initiated the PPP (Public Private Partnership) projects. These projects are also known as City Hospitals. ÇORAPÇIOĞLU Healthcare Consultancy Services Company focused on both planning and professional architectural services and even operational services of the several projects. ÇORAPÇIOĞLU Healthcare Consultancy Services Company provides turnkey healthcare projects and offers health counseling services to several institutions and health care organizations. In summary services in this area include the following key points. 1. Advisory services in healthcare projects, 2. Planning the devices which are going to be used in the health business. Determining the list of equipment and supplies and the provision of recruitment, 3. Planning and the implementation of the organizational chart of the business based on the project details. 9 SECTION IV 5 PLANNING, SETTING-UP AND MANAGING HOSPITALS ÇORAPÇIOĞLU provides turnkey healthcare projects/ hospitals and offers healthcare counseling services to institutions and organizations in the healthcare sector. All in all, the services provided in this sector consist of the below key points; 1. Offering consultancy services in the healthcare projects and putting these projects into practice, 2. Determining the lists of medical equipment, furnishing and supplies, and making purchases, 3. Designing and implementing the flowcharts, organization charts and procedures based on the nature of the healthcare center, identifying potential problems and creating solutions that may arise during the operation, 4. Providing the healthcare institutions with guidance on achieving the international standards of service quality, 5. Providing Turnkey Consultancy Services. It is possible to outline the healthcare consultancy services offered by ÇORAPÇIOĞLU in the following five steps: Planning and setting-up, preparing feasibility study, determining the need for medical equipment and supplying them, guidance on management, offering turnkey consultancy services. 5.1 PLANNING AND SETTING-UP 1. Carrying out the regional analysis where the healthcare center is planned to be set-up 2. If the healthcare center doesn t have a set region and location, giving advice on the suitable region and location 3. Determining the content, management format and care units of the healthcare center according to the specific features of the region 8

Designing the healthcare center project according to the Ministry of Health norms and the requirements of high level business activity, and ensuring it is ready to form a basis for an architectural project draft 5. Informing and guiding the establishment managers about the architectural project that is formed according to the healthcare center format 6. Ensuring the architectural project is in accordance with the chosen location for the hospital and the Private Hospitals Regulations 7. Working in cooperation with the architect to facilitate the operation of the architectural project, to fulfill the legislation and to guide the architect on this matter 8. Ensuring the architectural drawing of the hospital building is in accordance with the outlined norms by the Ministry of Health 9. After drawing the architectural project, ensuring that the necessary documentation is ready and the pre-authorization is provided by the Ministry of Health 10. Communicating with the licensing department whilst the architectural project is in the approval process by the Ministry of Health and ensuring the project is approved as quickly as possible 11. Providing guidance on how to acquire the general and local permissions to set up the healthcare center and the procedures that need to be followed 12. If there is an existing hospital building, adjusting its architectural plan according to the Ministry of Health norms, and with regard to this, revising the construction as required by these norms to ensure the building becomes effective with no flaws or inadequacy 13. Inspecting the construction is made according to the adjusted architectural project in accordance with the Ministry of Health norms 14. Ensuring the procedures and tasks related to the hospital are accelerated at the Ministry of Health and any other public and private institutions 15. Ensuring the construction is made according to the adjusted architectural project 16. Assisting the related construction company while the hospital building is under construction and inspecting whether the productions are in accordance with the specification 9
11 17. Inspecting whether the construction company is using the material that is in accordance with the norms set by the Ministry of Health 18. Inspecting the construction company to make sure it applies the architectural plan appropriately, informing the Hospital Management Staff in terms of the missing parts or things to be done in the construction, and providing guidance in terms of resolving matters and fulfilling deficiencies 19. Coordinating the right establishment of relationships with The Ministry of Health and The Provincial Health Directorate and maintaining these relationships 20. After the construction is completed, ensuring permissions for the hospital license are acquired from the Ministry of Health 21. Choosing ventilation, heating, cooling, hygienic material and similar material for specialty departments according to the international standards and inspecting their installations 5.2 PREPARING THE FEASIBILITY STUDY 1. Preparing the feasibility study for setting-up and management according to the set format of the healthcare center 2. Calculating the costs of preliminary research, architectural project, construction, infrastructure, gas systems, heating, cooling and ventilation that are necessary to set-up the healthcare center 3. Determining the lists of medical equipment, furnishing and supplies to be used in the healthcare center 4. Determining the surgical instruments to be used in the healthcare center 5. Preparing a detailed list of the costs for setting-up the healthcare center 6. Determining and detailing the costs for the management of the healthcare center 7. Calculating the costs of the norm personnel that is necessary for the Healthcare center and the costs of the employees specific for the units that need improvement 8. Analyzing and calculating the routine costs of the healthcare center and the costs that are related to certain criteria and preparing expenditure charts related to them 10

Specifying the income sources for the healthcare center, analyzing these sources and calculating how much revenue they will bring to the hospital, and creating distinctive charts related to them 10. Calculating the prediction of periodical, annual and ten years period term income and expenses by projections and making their statistical accuracy meaningful 11. Calculating the profit of investing in the healthcare center and the return on investment 5.3 DETERMINING THE NEED FOR MEDICAL EQUIPMENT AND SUPPLYING THEM These steps are followed respectively: Determining the lists of medical equipment, furnishing and supplies, choosing the best quality medical devices and equipment, purchasing them at the best rate and under appropriate conditions. 5.4 MANAGEMENT CONSULTANCY SERVICES 1. Preparing possible risk calculations and predictions that might occur during the management of the healthcare center and informing the Management staff about these matters 2. Forming the personnel standards according to the legislations and acquiring the Activity permission from the Ministry of Health that is necessary for the hospital to become fully functional and operational 3. Identifying the specialty sections of the hospital and distributing the personnel accordingly 4. Distributing the beds capacity of the hospital according to the sections and planning intensive care and operating rooms 5. As required by the Ministry of Health, forming the Quality in Healthcare Standards and ensuring these standards are applied during the hospital s daily operations 6. Creating specific organizational schema for the hospital 7. Creating hospital workflow charts and procedures 11
13 8. Defining the job description of all the staff that will be employed in the hospital such as the management staff, doctors, healthcare personnel and administrative staff 9. Ensuring the core personnel starts working and giving managers the necessary training to make sure they become capable of offering training 10. Together with the core personnel training, ensuring doctors and other staff are employed and provided with the necessary and suitable training 11. Creating desired employee behavior models and measuring their performance 12. Offering guidance on the necessary arrangements that will motivate the employees in terms of participation in the system and developments 13. Choosing the hospital automation program and ensuring it is set up by the Hospital Management Staff 14. Ensuring the automation program is integrated into the system and providing the users with hands on training 15. Offering guidance on the necessary structure that will render the healthcare center appropriate for the legal procedures 16. Providing guidance to the healthcare center on the necessary procedures to make agreements with the official institutions, private institutions, private health insurance companies, social security institutions, etc. and ensuring the contracts with them are signed 17. Determining the preventions needed to make profit without compromising the system s quality of service and boosting productivity by making the financial analysis of the healthcare center 18. Identifying and developing methods to prevent possible deficiencies in the healthcare center, training the related staff about these matters, and putting the developed system into practice 19. Determining priorities by preparing feasibility reports on new investment plans in the healthcare center 20. Identifying essential preventions to keep the idle capacity level to the lowest possible, providing relevant training, and inspecting the employees regarding this matter 12

After the healthcare center becomes fully operational, identifying disruptions in the operations and providing the management staff and other employees with in-service training to improve these situations 22. Ensuring the invoices sent to SGK (Social Security Institution) and other similar institutions are prepared in accordance with the legislation 23. Building the infrastructure that is essential to avoid interruptions in sending out invoices and training the related staff about this matter 24. Identifying the steps that are necessary to be followed to bring the hospital to the international standards and guiding towards this goal 25. Ensuring all the procedures and adjustments to the service offered in the hospital are permanently up to the international standards 5.5 OFFERING TURNKEY CONSULTANCY SERVICES This is the process of offering all above mentioned services. This starts from the decision to setup a healthcare center, including all the necessary steps, processes and services. 13
15 SECTION V 6 PPP (PUBLIC-PRIVATE PARTNERSHIP) PROJECTS CONSULTANCY SERVICES In PPP Projects, there are different types of operation and organization structures compared to the other hospitals. There is a long and broad process of Consultancy that consist of creating projects for the PPP Hospitals to start with, followed by constructing the hospital building in accordance with the international standards, planning all the services (P1-P2) that will be offered and determining their methods, resolving the workflow, applying all the services, and at last, the process of managing the hospital. 6.1 FORMING PROJECTS FOR THE PPP HOSPITALS It s essential to make an effort to create human based projects in PPP Hospitals as it should be the case in all hospitals. Our priority is to construct hospital buildings and units that focus on patients and employees. The PPP Hospitals can be used as Healthcare Campuses that consist of several hospitals. Even though each hospital has a different function, there are also joint usage areas. Forming the PPP Hospitals project compatible with the international standards depends on planning all the units in accordance with the Turkish Laws and Regulations in effect, Circular Letter and the Quality Standards. Whilst creating the PPP Hospitals project, International FGI standards are applied as determinative and directive. In addition, the Laws and Regulations published and in effect in Turkey, Circular Letter, and any other similar documents take part as support elements to form the project. While the PPP Hospitals are inspected to check the compatibility of all of the units with the International Architectural Standards, the workflow scenarios are provided at the same time. We work on solutions to reduce the time spent on procedures and the distance travelled by the 14
16 patients visiting the hospitals. In this regard, the best possible solution is determined and reflected on the project. In the PPP Hospitals, all the workflow is anticipated and planned according to the management scenarios that aim to provide the fastest and most accurate services with minimum flaws as to conduct technical services and all the below mentioned support and medical services without a hitch. 6.2 THE SERVICES OFFERED IN THE PPP HOSPITALS The medical and support services that will be offered in the PPP Hospitals are listed below. 1.
