Child House model Best practice models for helping abused children get back on track King s College Hospital on behalf of NHS England and MOPAC
Review of CSA services in London 2015 Low levels of children and young people identified and supported Lack of long-term, relational support for children and their families No clear pathway and isolated agency working Only 1 in 4 children offered medical support Re-traumatisation of children and young people through the criminal justice process High thresholds and long waits for therapy from CAMHS
Reducing re-traumatisation Re-traumatisation of children and young people through the criminal justice process Currently children tell their story multiple times Feel interrogated ABE suites not fit for purpose Adversarial legal system Children and young people waiting up to 2 years to go to trial Re-traumatised in court by cross-examination Jurys biased by pre-conceptions of victims and perpetrators
Lack of medical and emotional support Lack of long-term, relational support for children and their families High thresholds and long waits for therapy from CAMHS Only 1 in 4 children offered medical support Lack of commissioned services for child sexual abuse Poor knowledge of the pathway and options available Myths about CSA medicals (social care and police) High thresholds for CAMHS Significant gap in long-term relational support for emotional needs Few child or young people s advocates
Review of CSA services 3 year plan for London Historic sexual abuse Child House x5 NEW multi-agency support for children and families Support throughout the journey of recovery Disclosure Early emotional help Criminal justice process Recovery Recent sexual abuse (forensic window) Children and Young People s Haven Service Equitable to adults CAMHS assessment and bridging counselling Pilot ABE interviews with clinical psychologists CSA hubs x5 First step towards the Child House Best of the existing services Clear pathway
Our inspiration...barnahus in Iceland Child friendly house offers: Joint investigative interviews: court statements / CPS interviews Medical examinations Victim therapy Family counselling Consultation and advice to local CPS and police Education and research
Unique joint investigative interviews No need for repetitive and unstructured interviews which can distort the child s account Child friendly facilites and interviewers reduce retraumatization Rarely need to testify again in court as the judge hears interview live and within 2 weeks Victim therapy can start immediately Convictions per year has doubled
Our vision for the Child House in London Low levels of children and young people identified and supported Increased public and media awareness Ease of access without fear of statutory services No clear pathway and isolated agency working One stop shop Clear pathway and directory of services Re-traumatisation of children and young people through the criminal justice process ABE interviews with clinical psychologists Early, out-of-court cross examination (building on MOJ section 28 pilot) Lack of long-term, relational support for children and their families High thresholds and long waits for therapy from CAMHS Emotional support throughout the journey Building family/carer resilience Support to meet the needs at the time counselling, advocacy, therapeutic interventions Team around the key worker
What do young people say they need?
Next Steps in London and nationally Child House model supported by Met Police, CPS, NHS England, DH, MOPAC, HMCTS and Childrens Commissioner for England 1. Home Office funded a proof of concept of the Child House model (two sites in London and one in Durham) 2. Dept. of Health funded an early emotional support model in North London in 2016/17 (creating a CSA hub as a first step towards Child House) 3. London CCGs - CAMHS transformation plans are funding modelling work and early emotional support
