I am aware of the Code of Practice on Display Screen Equipment (DSE) and the DSE user information available on the University Health & Safety web pages including my entitlement to eye and eyesight tests. I have read all the questions in this Form. I am comfortable and do not wish to report any problems with my DSE workstation or work. Complete and return this front page to the DSE Assessor and discard the rest of the form. There will be no further action at this time. An assessment can be requested if circumstances change. The DSE Assessor will review in about a year, or if there are significant changes such as relocations. I wish to report problems with my DSE workstation and request an assessment by the School/Office DSE Assessor. The problems are not causing me pain or discomfort. ABOUT YOUR DSE WORKSTATION AND WORK - Give brief details of any problems with your DSE workstation or work. You only need to answer the questions that are relevant to the problems you wish to report. 1. Can you normally have a break from DSE work for a few minutes every hour? If No, give brief details of the problem Do you have other tasks at other times? (eg meetings, front desk duties, paperwork, teaching, workshop/laboratory work) Can you interrupt your DSE work with small tasks? (eg phone calls, queries, printing) Can you vary your DSE work? (eg reports, s, spreadsheets, databases, programming) Do you normally get away from the desk during rest breaks? 2. Is the lighting comfortable for your DSE and other work? If No, give brief details of the problem Is sufficient natural light available? Can blinds be adjusted to control sunlight if necessary? Is the office lighting bright enough? Is the office lighting free from glare? Can you adjust the lighting in your part of the office, without affecting colleagues? Have problems been reported to Campus Facilities Management (FM)? 3. Is the temperature and humidity in the room usually comfortable? If No, give brief details of the problem Are nearby doors and windows free from significant draughts? Are radiators free from obstructions that block the heat? Can heat-producing office equipment be relocated, without affecting colleagues? Can you easily reach and open windows to improve ventilation? Have problems been reported to Campus Facilities Management (FM)? 4. Are the general noise levels in the room usually comfortable? If No, give brief details of the problem Can noisy equipment be repaired? Can noisy office equipment be removed or relocated, without bothering colleagues? Can noisy work be done elsewhere? Can colleagues be asked to be a little quieter? Can noise from outside the room be reduced?

3 5. Is your chair comfortable? If No, give brief details of the problem Is your chair stable and in good condition? Does your chair move freely on the floor surface? Does your chair swivel freely? Are you familiar with all the available adjustments on your chair? Can you adjust the backrest height and angle so that the bulge of the seat back is in the small of your back? Do you sit right back in the chair with your back supported? Does the seat pad give good support without too much pressure on the backs of your legs? Can you adjust the seat height so that your shoulders are relaxed and your forearms and wrists are about horizontal when typing? Are your feet flat on the floor, or on a footrest, with the seat height correctly adjusted? If the chair has arms, are they low enough for your shoulders and elbows to be relaxed? If the chair has arms, can you sit close enough to the desk with the seat height correctly adjusted? 6. Is there enough space around and under your desk? If No, give brief details of the problem Can you move your chair, open drawers and get to other equipment easily? Is there enough leg room under your desk to sit comfortably and to change your position for different tasks if necessary? Is the area free from trailing leads, boxes and personal belongings? 7. Is there enough space on your desk? If No, give brief details of the problem Can you place items to your left and right as you prefer? Can you comfortably see and reach your most frequently used work items? Can you comfortably hold your phone and reach other items, make notes, use the keyboard and screen during calls if necessary? If you do copy typing, do you have a document holder next to the screen and at the same height, angle and distance? If you need to annotate or refer to documents frequently, can you reach and read them easily without leaning or twisting? eg place them between the keyboard and screen Is your desk surface suitable, i.e. not too shiny? 8. Is your display screen comfortable to use? If No, give brief details of the problem Is the screen directly ahead of you? Is the screen at a comfortable height? eg top about eye level for touch typists, or lower if you cannot type well and need to look at the keyboard Is the screen at a comfortable viewing distance? eg about arm s length Can you adjust the distance of your screen if you wish? Is the screen free from distracting glare or reflections from the sun or from room lighting? Can you adjust window blinds to control sunlight? Can you swivel or tilt the screen slightly to achieve a comfortable position, and to remove glare or reflections if necessary? Can you alter the brightness and contrast? Is the screen size and specification suitable for the type of work you do? Is the screen text a comfortable size and easily readable? Is the screen image stable and free from troublesome flickering? Is the screen clean?

4 9. Is your keyboard comfortable to use? If No, give brief details of the problem Is the keyboard separate from the screen? Are your shoulders relaxed and your forearms about horizontal when sitting at the keyboard? Are your wrists about straight when typing? If you tend to rest your wrists on the desk when typing, do you have a wrist rest? Can you use the keyboard without reaching too far forward? Do you have space in front of the keyboard to rest your hands when not typing? Can you adjust the tilt of the keyboard? (eg raise/lower feet under the keyboard) Do you type with a light touch and relaxed fingers? Are the characters on the keys clear and readable? Are you a touch typist? 10. Is your mouse (trackball etc) comfortable to use? If No, give brief details of the problem Is the mouse close to you? (eg right beside the keyboard) If you use just the mouse for some tasks, such as graphics, do you place it directly in front of you? Do you use a mouse mat to provide cushioning for your hand/wrist? Does the mouse move freely? Is your mouse adjusted to give comfortable accuracy, speed and range of movement? Do you move the mouse with arm movements rather than flexing your wrist? Are your fingers relaxed when using the mouse? Is the double-click speed comfortable? Are you familiar with keyboard shortcuts that can be used rather than the mouse? 11. Is the software suitable for the kind of work you do? If No, give brief details of the problem Do you know how to use the software provided for the work you do? Have you requested general training courses? Is there an expert colleague who can advise on specific queries? Have you made adjustments for your work and preferences eg shortcuts, tailored menus? 12. Is your laptop comfortable to use? If No, give brief details of the problem Is your laptop used only infrequently and for short periods? Do you link your laptop to a full-size pc and properly designed workstation whenever possible? Do you normally use your laptop on a standard height work surface? Do you normally sit in a standard height upright (preferably adjustable) chair? Do you sit right back in the chair with your back supported? Do you sit directly in front of the laptop? Are your shoulders relaxed and forearms/wrists horizontal when keying? Does your laptop have as large a screen as possible? Can you normally position the screen to avoid reflections and glare? Can you separate the screen from the keyboard? Does the keyboard have as large keys as possible? Can you adjust the angle of the keyboard? Are your fingers relaxed and hand flat when using the mouse or other tracker device? Can you normally position reference documents close to the keyboard/screen? Are the software, memory, speed and add-ons suitable for the work you do? Do you take plenty of breaks and change your posture from time to time? Is your laptop, and its accessories, as light as possible? Do you carry your laptop in a lightweight case with shoulder straps?
