Please allow 2 working days before collection your prescription in order to enable us to process your request. Test Results Please telephone reception after 2 pm for the results of tests taken by the practice.

3 Specialist Clinics: BABY CLINICS Mondays and Thursdays 12:30-13:30 (No appointment necessary) To avoid spreading illness, please do not bring sick children to the baby clinic. You can have your baby vaccinated and have a 6 week check by the doctor and/or the nurse who will be available to offer help and advice. Bring your red book to be completed at the same time. The Health Visitor does not attend the surgery. You may contact her on Travel Clinic Travel vaccinations are available for most destinations (often free under the NHS). We are a Yellow Fever Centre. Minor Operations Warts and skin tags can be treated in surgery, thought not if they only cause a cosmetic problem. Please make an appointment to discuss this with your doctor. Regular Check Ups If you are between 40 and 75yrs you will be invited for a full check up with sugar and cholesterol tests every five years. This will be carried out by the health care assistant. If you are outside this age range and would like a check up please discuss this with your doctor. Smoking Cessation. If you would like to stop smoking counselling is available at The Hounslow Stop Smoking Programme on Prescriptions for nicotine replacement therapy can be obtained from your GP in conjunction with counselling.

4 How to see your Doctor Morning Surgery Appointments to see the GP between 08:30 am and 12:00 noon are available to book from 12:00 noon the day before. Some pre-booked morning appointments are available to book up to 6 weeks in advance. We also operate a Walk In Clinic which will accept patients from 08:30am 10:30am. It is run by one doctor and you will have to wait your turn. Afternoon/Evening: By Appointment only Dr Burbidge: Mondays ; Fridays Dr Morris: Mondays ; Wednesday Dr Caballero: Tuesdays Dr Ramos: Wednesdays, Thursdays and Fridays Dr Cassar: Tuesdays 17:00 19:30, Wednesdays 15:00 17:00 Thursdays 16:00 18:00 Appointments can be booked online, via phone or visit the surgery during opening hours. If you do not have an online login yet, please see our Reception team with a valid photo ID; we will be happy to give you your login details. Length of Consultation Appointments with your GP are 10 minutes in duration. If you feel you would like longer please advise the receptionist.

Sexual health and Chlamydia screening for men and women Cystitis & Thrush. HRT Checks. Travel Vaccinations. Flu & Pneumococcal Vaccinations. Wound Dressings & Removal of Stitches. Anti-coagulation monitoring Travel Health Assessment forms can be obtained at reception or downloaded from our website. Please complete and return 6-8 weeks before departure, if possible. PHLEBOTOMIST/HEALTH CARE ASSISTANT Appointments only each weekday morning Our phlebotomist/health care assistant can help you with the following: Blood pressure checks Blood tests Dietary Advice Health Checks Chaperone policy Patients are entitled to have a chaperone present during any consultation, examination or procedure where they feel one is required.

6 Charges Fees for non NHS services are displayed at reception. Medical Students Occasionally medical students join the practice for a short period as part of their training. If you do not wish them to sit in on your consultation please tell the receptionist. Your Rights and Responsibilities Patients will be treated with respect and courtesy at all times and the practice staff also expect the same from patients. We have a zero tolerance policy towards violent or abusive patients. Threatening behaviour towards our staff could result in your removal from our list. Personal Health Information Patients personal information will be held in confidence by the practice staff. Occasionally the practice collates statistics which supports audit and research. This information is anonymised and used for audit research purposes only. Any request for information concerning our patients has to be accompanied by a signed authorisation from the patient concerned. We are registered under the Data Protection Act Cancellations If you are unable to keep the appointment you have booked with a doctor, nurse or health care assistant please call or go online to cancel the appointment, giving as much notice as possible so we may offer the slot to another patient How to register at our practice We register new patients every weekday morning either by booked appointment or during the walk in hours (8.30 to 10.30) Please bring your NHS card (if you have one), details of your previous registration, proof of address (eg utility bill, bank statement) and passport/birth certificate or English photo driving licence. New registration takes longer than regular consultations so please allow plenty of time. Children under 5 s can be registered without being seen as long as one of the parents is registered with the surgery. In order to register your child a passport or English birth certificate is needed and record of any immunisations. Change of Name and/or Address/Phone number/s It is essential that you keep us informed of any changes - we may need to contact you. If you move away from our catchment area you will need to find a new GP closer to home.

7 Continuity of care You are welcome to express a preference for a particular GP and we will do our best to comply. However it may not always be possible to see the same GP due to overbooking, annual leave, or illness. Sick Children A doctor will see a very sick child in surgery as soon as possible. Please ask the receptionist to bring the problem to the attention of the GP/Nurse when you arrive during normal surgery hours. Home Visits If possible please try to get to the surgery as we can offer more comprehensive care here. If a visit is required phone reception before 11am unless an emergency arises later in the day. Your message will be passed on to the on call doctor who will contact you to discuss your problem and arrange a visit if necessary. Telephone advice If you wish to speak to a doctor or nurse, phone reception who can arrange this. The receptionist will advise you when they will call you back. Comments, Complaints or Suggestions We welcome any suggestions as to how you feel we may improve our service. If you have any comments, compliments or complaints please bring them to the attention of our practice manager, who will try to help you. Hounslow CCG We are a member of the Clinical Commissioning Group who are based at Sovereign Court, 15-21Staines Road, Hounslow, TW3 3HR You may contact the Trust for details of primary care medical services on Access to the first floor Some of our clinicians are based on the first floor of the surgery. Please ask at Reception to be seen on the ground floor if you have difficulties using the staircase. Disabled Access The whole of our ground floor is accessible by wheelchair. Our reception is fitted with a loop. If your hearing is impaired please turn your hearing aid to the T setting. A Portable loop is available for your consultation with your health care professional. Please ask the receptionist to arrange this when you arrive at the surgery.
