2 IT Student Internship Checklist Please submit this form into B lackboard according to the due dates listed on the syllabus. An IT internship is required for students accepted into the IT - Computer Support Specialist and IT - Web Designer Associate degree programs. Other IT programs can opt to add the internship requirement to their program by enrolling in the IT Career Skills and Internship class, (2- credit class). Student must complete prior to securing the internship: Completed/Date Register for Career Skills with Internship class (2- credit class) Meet with Internship Advisor by the first week of class Review course syllabi Attend resume, cover letter workshops Register on Wisconsin TechConnect and receive internship postings Visit the NTC website for internship job postings Complete a resume and cover letter Complete the IT Internship Business Research Form Complete the Student Intern Information Sheet Apply and interview for internships Submit all required internship forms/assignments to the Internship Instructor via Blackboard according to the semester schedule check for due dates listed in your syllabus. Student must complete prior to starting the internship: Set up First Employer Site Visit Send Internship Advisor and Internship Instructor a confirmation to attend the First Employer Site Visit Attend the First Employer Site Visit with the Employer and Internship Advisor Complete the IT - Internship Training Agreement Form during the site visit Student must complete during the internship: Have Employer complete the Mid- term Evaluation of Student form Have Employer complete the Final Evaluation of Student form Log and submit internship hours worked each week thru the Form Boss Site Complete and submit the Student Post- Internship Satisfaction Survey at the end of your internship experience. Submit all required internship forms/assignments to the Internship Instructor via Blackboard according to the semester schedule check for due dates listed in your syllabus. 2

Below is a list of questions to consider when choosing an internship site. Please research five (5) companies that you would be interested in doing an internship with, to help you find a good fit. Please list the companies below. 1. Does the internship site offer opportunities to help you achieve your learning goals? This best fit approach is intended to provide you with field experience and insight into the daily activities of your chosen interest area. 2. Will this internship expand your knowledge and permit professional growth and development? 3. What new skills will you develop? 4. Who are the clients served by the internship site? 5. With what specific project(s) will you be involved? Are you enthusiastic about these projects? 6. Will you primarily work behind the scenes or directly with clients/community members? 7. Where is the internship located? Do you have reliable transportation to the internship site? 8. What type of networking opportunities are available to you as an intern at this site? 9. Is the internship paid or unpaid? 10. Is there a possibility you may be hired by the internship site upon completion of your internship? Are they currently hiring? Name of Business Researched:

5 IT Student and Employer Internship Agreement Form This form should be complete during the First Employer Site Visit between Student and Employer. The purpose of the Internship Program is to assist the student in applying concepts and skills acquired in school to the workplace. A valuable component of the IT Internship Program at NTC is to enhance the development of the Core Objectives through the cooperative efforts of Northcentral Technical College and its business partners. It is agreed that will perform the duties and (Student s Name) responsibilities of an Internship student with (Employer s Name) for the semester beginning through. The College agrees to do the following: Provide an Internship Advisor and Internship Instructor to coordinate the internship experience. Make periodic contact with the employer relative to student progress. All interns will be instructed of the importance of good personal and vocational relationships which include: appearance and grooming; health principles; courtesy and etiquette; interpersonal relationships; and ethical and legal responsibilities, including the importance of extreme confidentiality while doing work at the internship business. The Employer agrees to do the following: Provide training experience for the Internship student and assist the student in developing and completing goals related to the Core Objectives listed on this form. Pay salaries at the wage agreed upon. Complete the Mid- term and Final Evaluation of Student forms and review evaluations with the student intern. Sign and Date the Student Weekly Log Sheet to document hours worked each week, for the student intern. Employ the student the minimum number of hours required during the semester specified. Additional hours can be negotiated between the student and employer. Inform the Internship Advisor and Internship Instructor of any concerns regarding on- the- job training so possible adjustments can be made. Provide the student intern with a brief job description or learning plan. Provide a work environment free of discrimination and harassment. The Student agrees to do the following: I agree to follow the employer s policies, practices, procedures, dress code, and/or standards of conduct. If I do not understand any of my employer s requirements, I will request clarification. I will keep Northcentral Technical College informed of my current contact information at all times during my internship. I understand that my employer is counting on me to complete my assigned work accurately and on time. I will notify my employer immediately if I will be absent due to illness or another serious circumstance. I will immediately report any changes in my internship status (i.e., layoff, reduction of hours, termination) to my Internship Advisor and Internship Instructor. I will conduct myself professionally at all times. This includes, keeping all company/business related information completely confidential. Any intentional or involuntary violation of confidentiality may result in the termination of this Internship at this site. Any confidentiality violation reported will result in a failure of this IT Career Skills & Internship course ( F grade). 5
6 IT Student and Employer Internship Agreement Form Provide my employer with the Mid- term and Final Evaluation of Student form and review the completed evaluation forms with my employer/internship supervisor. Submit Mid- term and Final Evaluation of Student form to Internship Instructor via Blackboard by due dates. Keep track of IT work completed (72 hours is required) and submit the Student Weekly Log Sheet to Internship Instructor via Blackboard each week during the internship experience. NTC students pay a $6.00 per semester fee for Student Accident Insurance, and if working in an internship that relates to coursework, during the defined time of a particular semester, a student may be able to make a claim if injured on the job while doing an internship. To determine if you are eligible for Student Accident Insurance, please refer to the NTC website: content/uploads/accident- Insurance- FAQ pdf Students taking their program 100% online are not eligible for the Student Accident Insurance. If you have questions about your eligibility, please contact Shawn Sullivan, Director of Student Development at or at Sullivan@ntc.edu. This agreement is not a contract nor is it legally binding. Either party may terminate this agreement for just cause or at employer s discretion after discussing the difficulties with the Internship Advisor. Hardware 1) Set up computer systems. 2) Configure network hardware components. 3) Perform routine hardware maintenance. 4) Add hardware peripherals. 5) Make computer and network cables. B. Systems/Application Software 1) Install, maintain, backup, restore data. 2) Load and debug various operating systems. 3) Create and/or edit MS Word, Excel, PowerPoint files to support company s needs. 4) Create, edit, report using company s databases (MS Access/SQL Server, etc.) to support their needs. C. Help Desk A. Troubleshooting and problem- solving hardware/software problems. B. Assist in computer training. C. Help computer users use software and hardware. D. Work cooperatively with other IT areas. Software Developer A. Develop user friendly interfaces. B. Code and debug in various programming languages. C. Test new and changed programs/systems (unit and integrated). D. Prepare program and system documentation. E. Follow company s programming standards. F. Train and/or assist users. G. Work cooperatively with other IT areas. Web Design A. Design, code and maintain web pages and web sites. B. Incorporate video and audio as needed. C. Debug web site problems. D. Prepare documentation as needed. E. Train and/or assist users. F. Work cooperatively with other IT areas. Networking A. Perform network administration tasks using various operating systems. B. Design physical and logical networks. C. Perform basic router and switch configurations. D. Create simple router access control lists. E. Prepare documentation as needed. F. 8 IT Student and Employer Internship Insurance Information Paid and Unpaid Internships Health Insurance, Accident Insurance, Liability, Minimum Wage 1. Paid Internships Northcentral Technical College promotes paid internships for our students, as unpaid internships create more risks for students, and may result in violation of the Fair Labor Standards Act for employers A paid internship is when a student is paid by the employer and is provided the benefits according to the terms of employment. Work related Liability issues and Worker s Compensation would be covered by the employer. Health Insurance may or may not be covered, and will be determined by the employer. NTC students pay a $6.00 fee for student accident insurance, and if working in an internship that relates to coursework, during the defined time of a particular semester, a student may be able to make a claim if injured on the job while doing an internship. Unpaid Internship Wage and Hour Guidelines All unpaid Internship Wage and Hour Guidelines are defined by the Fair Labor Standards Act while working for for profit employers. Employers must follow the six criteria for The Test for Unpaid Interns, set forth in this document. If an employer meets the test for an unpaid internship, students are responsible for their own liability insurance. NTC students pay a fee for student accident insurance (check the Schedule of Benefits and Exclusions and Limitations in the attachments above) and the student would be responsible for their own health insurance. Worker s compensation would not be granted by either the employer or the school in a qualified unpaid Internship. Student name/date: Employer Supervisor/date: Instructor/date: 8

This form should be completed by the Employer/Internship Supervisor, with the student present. Please evaluate the student s performance during the internship period. The comment sections are to be utilized to include highlights of the job performance, areas for improvement, and any suggestions. Please provide the student intern with a copy of this evaluation form to turn into their internship instructor and internship advisor.
