1 Access the FICURMA isafetyexpert Portal Open a web browser and insert into the URL location Bookmark the site (Add to Favorites) On future visits, you will Login here The first time you access the system, click Register For detailed guidance on registering, click on Registration Instructions Complete all fields on this form and make appropriate selections from the two drop-down menus. Upon selecting User Type, you will input the corresponding identifying info (i.e. Policy #) Insert the letters/numbers shown in the box. This is a security feature. Click here to report a WC or GL Claim Click here to report an Auto or EPL Claim Initial Access: Technical Support will forward an to new users with log-in credentials to the claim reporting systems. 3 Workers Compensation and General Liability After you select the WC and GL button at the FICURMA isafetyexpert portal, you will see this screen: Insert your Login (User I.D.) and Password, then select Login. If you see this message, select "Yes". Select the type of claim to Open, then click Proceed to Claim Entry. FICURMARPTGv1 L. Sudbury 3

4 Workers Compensation At the next screen, choose State Workers Compensation Act and Next to Insured Confirmation Page Refer to this page and confirm that you have the minimum information needed to open a claim before you proceed. Phone number of employer representative Accident Location Date and Time of Accident Date and Time Employer became aware of Accident Injured Employee s full name, address, SSN and phone number Type of injury or illness, cause and result FICURMARPTGv1 L. 5 Workers Compensation Check the box next to a selection and the information inserted here will default each time you input a new claim. The field will remain editable; check the box to save time and change it whenever you need to. Click any Question Mark for help completing that field. 6 Workers Compensation Input the Social Security Number, then click on the Tab key. If an employee has prior claims in the YCE system, much of the information on this page will autocomplete. Be sure to note here if: Employee was paid for the date of injury Salary is continued FICURMARPTGv1 L. Sudbury 6

7 Workers Compensation Be sure to use the correct format for Date (01/01/2001, , or ) and Time (09:00) For a Lost Time claim, you must check Future major medical / Lost Time anticipated To open a Medical Only, select Minor, Emergency Care or Hospitalized, as applicable FICURMARPTGv1 L. Sudbury 7

8 Workers Compensation If the employee began losing time immediately, Date last worked is the date of accident. Date disability began, in this case, is the next day. If the claim type shown here is incorrect, choose from the following in the drop-down menu: Asbestosis Hearing Loss Indemnity Medical Only-Complex Medical Only-Minor Reporting Purposes Only Silicosis Uninsured FICURMARPTGv1 L. 9 Workers Compensation The Completion screen provides final confirmation and generates the claim number. From this screen: Open another claim Enter York Claims Expert or Log out Refer to the previous few pages for basic instruction in reporting claims for all lines. 11 General Liability By inputting the number of claimants here, you tell the system to provide adequate space on the next screen to input all claimants. Select either the Incident Address OR the Describe Location button, then populate with complete location information. Be as thorough as possible. You must input either the correct address where the loss occurred or a description of the location, including State and County. FICURMARPTGv1 L. Sudbury 11

12 General Liability Select Claimant Injured, Property Damaged or both for each Claimant. Here it has been determined there are 2, not 3, claimants select Remove This Claimant Select Add Claimant if space is needed for additional claimants. FICURMARPTGv1 L. Sudbury 12

13 General Liability Review this page to confirm that the correct number and type of claims are about to be opened, then click on Next to Completion Page. As with a Workers Compensation claim, the Completion page: Confirms that a claim (or claims) has been opened; Assigns a Claim Number to each claim; Assigns a number to the Occurrence linking the claims; and Offers the choices to Enter a New Claim, Log out, or Proceed to YCE Instructions for inputting Auto and EPL claims are found in the following pages. 14 Auto and Employers Professional Liability After you select the Auto & EPL button at the FICURMA isafetyexpert portal, you wil see a screen with ClaimsExpert and this button Click on it to log in. (But you knew that, right?) If you see this message, select "Yes". At the next screen, insert your user i.d., password and, at the Company prompt, indicate York If you are unable to log-in, York Claims Expert (version A) allows you to respond to security questions and reset your password without contacting the help desk. 15 Auto and Employers Professional Liability Click on Functions, then on Open a Claim Menu You will see this screen Click on Open a Non-Workers Compensation Claim Search for the insured and select next Select applicable insured (NOTE: The system should default to FICURMA as the insured. 16 Auto and Employers Professional Liability This page displays available coverages. This is an information only page. Click Next to input a claim. Instructions for Auto claims are found on the next pages, followed by instructions for reporting EPL claims. FICURMARPTGv1 L. Sudbury 16

17 Auto Input the claim. If a single incident involves both bodily injury and property damage, TWO separate claims must be entered, as instructed here. Date requires 8 digits in one of these formats: or 08/01/2013 or Responses for #2 and #3: For BI claim: Yes and No For PD claim: No and Yes Input as much detail as possible, then click Next. Select Auto, then click Next FICURMARPTGv1 L. Sudbury 17

18 Auto The next screen relates to the responses you provided earlier. If you stated that there was Bodily Injury, but no Property Damage, you will see this screen. Select the button at Auto Liability Bodily Injury, then click Next. If you stated that there was Property Damage, but not Bodily Injury, you will see this screen. Select the button at Auto Liability Property Damage, then click Next. Remember, you MUST report BI and PD claims one at a time. As you complete the next screen, please select from the drop-downs in the order they appear on the page, as the selections available in some drop-downs are dependent upon others. 20 Auto Mandatory Fields Select the damage property it if appears on the list. A list of scheduled property is displayed and select FICURMARPTGv1 L. Sudbury 20

21 Auto Supervisor and Adjuster assignments are made automatically AFTER the claim is reported. Do Not Edit these drop-downs. The Adjuster Assignment page indicates the adjuster, the supervisor, and allows the user to add comments that are sent to the assigned adjuster and added to the file. The adjuster and supervisor should not be changed. At the Confirmation page, confirm that all information is correct before selecting Open Contract Claim. This is your last opportunity to cancel or change anything before the claim is submitted. FICURMARPTGv1 L. Sudbury 21

22 EPL To report an EPL claim for Damages, you will follow the same steps. Screen shots specific to an EPL claim are shown here. At this screen, if the claim involves damages but no Bodily Injury or Property Damage, answer NO at #2 and #3, and YES at #5. At the next screen, select Liability, then click on Next. 23 EPL Choose Professional Liability Damages, then Next. At this screen, select the Line of Coverage as shown and choose the Sub Line from the drop-down menu. The rest of the process for EPL is identical to the process for reporting an Auto claim. FICURMARPTGv1 L. Sudbury 23
