1 Boise Y Swim Team Registration for New Members (Please Use Our On Line Registration Process If At All Possible!) Welcome to the Boise Y Swim Team! We have put together a packet which we hope will provide you with all the information you need to begin swimming with our great team. Please read the material carefully. If you are registering more than one child, please download additional forms as needed. Only one Volunteer Form is needed from each family. All forms requiring a signature must be returned before starting practice. Please mail all completed forms, PROOF OF AGE, and your payment to: PATTY STRATTON, 2017 S. ROOSEVELT, BOISE, ID Please be sure to apply the correct postage to your envelope! Read and follow the instructions below. ALL ITEMS MUST BE RETURNED IN ORDER FOR YOUR REGISTRATION WITH THE TEAM TO BE CONSIDERED COMPLETE! Include a check for $84 made out to The Boise Y Swim Team. This check will pay for the non refundable Joining Fee of $25 and for the 2015 USA Swimming Registration Fee of $59. If you provide proof your family receives free or reduced hot lunch and if you complete the Outreach Form, the USA Swimming Registration Fee will be reduced to $7. If this is the case, please write your check for $32 ($25 Joining Fee plus $7 USA Swimming Outreach Fee). Swim Team monthly dues and other charges will be billed. You will receive swim team bills via . Your USA Swimming registration will be valid from now until December 31, Membership in USA Swimming provides your swimmer(s) with secondary accident insurance. Please fill out the 2015 USA Swimming Outreach Registration Form ONLY IF you qualify for the Outreach rate of $7. In order to register at the $7 rate, families must provide a copy of the letter sent to you by the school district granting your family free or reduced lunches, a copy of your Medicaid card, a food stamp assistance program benefits letter, or other documentation consistent with income eligibility. You must provide Proof of Age by providing a photocopy of your child s birth certificate or passport. The photocopy will be mailed to the Snake River Swimming s Registration Chairman. You may mark out any information (other than legal name and birth date) which you would prefer not be known. Fill out and return the Boise Y Swim Team Registration Form. Read the Team Membership Requirements, the YMCA Swimmer Code of Conduct, and the Parent Pledge Form. Sign and return the Signature Form which attests you have read and agree to the terms of the above listed forms. Read the Parent Service Responsibility Policies and, fill out, and return the Parent Service Responsibility Form. We have listed several opportunities for parental involvement with the swim team. All parents are expected to help with our team meets or other team functions. The envelope you mail to the Team Admin should include the following: 1. 4 Boise Y Swim Team Team Membership Requirements To be a member of the Boise Y Swim Team (BYST), swimmers and parents must agree to the following in order to be considered a Boise Y Swim Team member eligible to practice with the team, represent the team in competitions, and participate in team functions: 1. Swimmers must be approved for membership, complete all team registration forms, and be current, dues paying members. 2. Swimmers must be members of the Treasure Valley Family YMCA and of USA Swimming. 3. Swimmers must practice exclusively with the BYST and under the direction of the BYST coaches and not with any other USA swimming team. Exceptions for travel, vacation and family visitation situations. Please check with the coaching staff two to three weeks beforehand. Exceptions for official collegiate, high school and novice/summer programs (i.e., Blue Marlins at the West Y). 4. Swimmers must compete exclusively for the BYST. Exceptions for official collegiate, high school and novice/summer programs. 5. Swimmers must enter all meets through the BYST meet entries coach. 6. All meets must be agreed upon in advance with the BYST Coaching staff. 7. For meets without a BYST coach in attendance, a Treasure Valley Family YMCA Liability Waiver will need to be on file for each swimmer/parent before any said meet. 8. Families must remain current on their swim team account. 9. Swimmers and parents are expected to participate in fundraisers and to volunteer when asked. 10. Swimmers on a Leave of Absence (LOA), who decide to swim in a competition, must swim unattached. If a swimmer/member does not adhere to all of the above, the member s account will be inactivated and their membership terminated.

5 Boise Y Swim Team Code of Conduct The Boise Y Swim Team strives to provide a meaningful experience for swimmers at all levels with emphasis on helping members exemplify the YMCA core character values of Caring, Honesty, Responsibility, and Respect. Expectations: All swimmers will be at practice on time with all required equipment. If swimmers are going to be late or need to leave early, they will notify the coach ahead of time. All swimmers will follow the directions and instructions of the coaching staff and display proper respect and sportsmanship toward coaches, officials, administrators, teammates, fellow competitors and the public at all times.. All swimmers will display proper respect and sportsmanship towards teammates, competitors, other coaches, meet officials, Aquatic staff members, and the general public. All swimmers will refrain from using inappropriate language or humor. All swimmers will refrain from intimate displays of affections towards other participants while participating at any Boise Y Swim Team practice, event or activity. All swimmers will only engage in age appropriate activities while participating at any Boise Y Swim Team practice, event or activity. No deck changes are permitted. Athletes are expected to use available change facilities. Team members are reminded when competing in meets, traveling on trips, and attending other meet related functions, they are representing both themselves and the Boise Y Swim Team. Athlete behavior must positively reflect the high standards of our Team. All swimmers will report any inappropriate behavior to the coaching staff immediately upon recognizing or hearing about such behavior. Unacceptable Behaviors: Insubordination to any coach, meet official, chaperone, parent volunteer, or facility staff. Displaying or promoting unsafe actions in or around the pool or locker room. The possession or use of alcohol, tobacco products, or any non prescribed drug by any athlete is prohibited. The possession, use, or sale/distribution of any controlled or illegal substance is strictly forbidden. The possession or use of any form of weapon during swim team events is strictly forbidden. Theft, borrowing or souvenir taking. Fighting, bullying, or disorderly conduct. Team members and staff will refrain from any illegal or inappropriate behavior that would detract from a positive image of the team or be detrimental to its performance objectives. Inappropriate communication, behaviors, or interaction between any BYST members (i.e. inappropriate texting messages or pictures. as well as inappropriate messages or pictures on any social media communications). Consequences of Failing to Comply with the Boise Y Swim Team Code of Conduct: Failure to comply with the Code of Conduct as set forth in this document may result in disciplinary action issued by the Head Coach after consultation with Y leadership staff and the BYST Advisory Council Executive Committee. Such discipline may include, but may not be limited to: a. Verbal reprimand, suspension, or dismissal from the team. b. Proceedings for a LSC or USA Swimming National Board of Review. c. If traveling, dismissal from the trip and immediate return home at the athlete s expense. d. Disqualification from one or more events, or all events of a competition. e. Disqualification from future team travel meets. f. Financial penalties associated with cancelled team travel.

7 BOISE Y SWIM TEAM PARENT SERVICE RESPONSIBILITY POLICIES The Y is an organization dedicated to building strong kids, strong families and strong communities. Youth participating in Y programs are more resistant and resilient to the negative social pressures, which they are exposed to every day at school, in your neighborhood, or through the media. By having positive role models, engaging in physical activity, volunteering, learning about YMCA core values, kids on the swim team are more likely to have a higher number of internal and external developmental assets, which ultimately can help them live healthier lives. Because of reasons like these, combined with an integral need for cooperative support at events and functions, the Boise Y Swim Team has implemented a new Parent Service Responsibility Policy. Team Membership Parent Service Responsibility As members of the Boise Y Swim Team, parents are needed to serve on the Swim Team Advisory Committee, coordinate special events, support the Aquathon fundraiser and, most importantly, to help with swim meets. However, whichever areas you participate in, you can take great satisfaction in knowing that you are key in making the Boise Y Swim Team experience something that all swimmers will remember for the rest of their lives, and that they will be healthier in spirit, mind and body because of your support and cooperative help. Home Swim Meet Parent Service Responsibility Parents of all Boise Y Swim Team swimmers who have been entered to swim in any given meet are required to work at that meet, and they will be assigned jobs and shifts to coincide with their swimmer s events, unless otherwise requested. The required minimum number of shifts (which are usually 2 hours in length) per family per meet is one shift per day of competition. The maximum number of shifts assigned per family per meet will vary it will depend on the size of the meet and the number of Boise Y swimmers entered in that meet. Either the Meet Director or the Parent Service Coordinator will assign jobs and shifts. However, parents are able to request specific jobs and shifts up until the meet entry deadline. Pre meet and non timing assignments, such as Meet Set Up/Take Down, Warm Up Marshal, Heat Sheet Sales, Awards, Hospitality Helpers, and Raffle Ticket Sales, will be first assigned to parents of 8 & Under swimmers. All meet workers must ensure they sign in at the Meet Director s table upon arrival for their shift. If a parent is unable to work an assigned shift, any other family member, relative, friend, or even a high school student who needs community service hours, may be substituted (they must be over 12 years of age). It is the parent s responsibility to find a substitute should one be needed. Parents must inform the Parent Service Coordinator prior to the first day of the meet, and have their substitute signin at the Meet Director s table upon their arrival for the shift. Once a meet job and shift schedule is finalized (usually by the Tuesday before each meet), it will be posted on the Team s website, ed in the weekly update and either mailed or phoned through to each assigned family. At the meet, the schedule will be posted around the pool on brightly colored boards and at the Meet Director s table. Parents may opt out of working a meet provided they give the Parent Service Coordinator at least two weeks notice (by or in writing) and pay a $25 meet opt out fee (this fee only covers one meet and will be billed directly to their account). The Meet Director may waive this fee if there are extenuating circumstances (which should also be communicated by e mail or in writing as soon as the situation arises). However, it is requested that parents either work their shift/s or do their best to find a substitute swim meets need workers, not money! Families who do not turn up for their assigned shift/s at a meet will be charged a $25 per shift no show fee. The Meet Director may waive this fee if there are extenuating circumstances. Failure to show due to parent negligence in reading s, checking mail or otherwise neglecting to find out if they had a shift assigned, does not constitute waiving of this fee. At the conclusion of a meet, the Meet Director will give the Team Administrator the names of those families who should be charged a fee and the amount. Away Swim Meet Parent Service Responsibility If a Boise Y Swim Team parent accompanies a swimmer to an away meet, they are required to work at least one timing shift per day of competition. Officials working at away meets are exempt from this requirement. It is extremely important to note that no child will be discriminated against, kept out of the water, or turned away from the team, due to lack of parental service participation. In addition, good communication goes a long way to preventing misunderstandings and ill will. We ultimately rely on the honesty, responsibility, respect and the caring attitude of all parents.

Please PRINT. Parent Alternate Parent Parental involvement is crucial to the success of the Boise Y Swim Team. As swim team members, parents are needed to serve on the Swim Team Advisory Committee, coordinate special events, support the Aquathon fundraiser and, most importantly, to help with swim meets. Being involved with the Boise Y Swim Team is a great way to meet people, have fun, and teach your swimmer(s) about commitment and responsibility. 9 BOISE Y SWIM TEAM FEES, FUND RAISING, & OTHER CHARGES FOR Becoming a member of the Boise Y Swim Team means fun, fitness, and teamwork. It also means a financial obligation to pay your monthly dues and other charges in a timely manner. The following outlines what you can expect to pay as a member of the Boise Y Swim Team. At first glance, it may seem as if the swim team is expensive. However, when taking into account the hours of coaching your swimmer receives for their monthly dues, the swim team offers a reasonable team experience designed to promote self esteem, self discipline, and good sportsmanship. YMCA MEMBERSHIP: USA SWIMMING: TEAM JOINING FEE: MONTHLY DUES: FINANCIAL ASSISTANCE: Because the Swim Team is a Treasure Valley Family YMCA program, each Swim Team member must also be a member of the Treasure Valley Y. The membership fee varies depending upon whether you have a youth or family membership. Arrangements for becoming a Y member should be made at the front desk. Your membership allows full use of the Downtown, West, and Caldwell facilities. Currently, the youth (ages 10 18) membership is $26.39 (includes tax) per month if paid by monthly bank draft. If you choose not to do the bank draft, monthly dues are $39.59 (includes tax) payable in advance. There is no joining fee required for a youth membership. As the Boise Y Swim Team is a member of USA Swimming, all members are required to pay a yearly membership fee. The annual fee for 2015 is $59. This fee is reduced to an annual fee of $7 if your family receives free or reduced hot lunch. The USA Swimming membership provides secondary health and accident insurance in case of injury during swim team practice and during swim meets. A non refundable joining fee of $25 will be charged to all NEW swimmers. This is a one time only fee unless swimmers leave the team for longer than two years. Unlike most Treasure Valley Family YMCA programs, the Boise Y Swim Team is responsible for paying all program costs including coaches salaries, team travel, financial assistance, and operating expenses. Therefore, the Boise YMCA Swim Team must charge each swimmer monthly dues separate and apart from their Y monthly dues. Swim team dues are dependent upon the group which your swimmer is placed in and will change as your swimmer moves from group to group. Monthly dues are as follows: Sockeye II $53; Sockeye I $55; Bronze $60; High School I and II $64; Silver $72; Gold $80; Senior $96; National $96. A discount will be given at the rate of 10% for a second swimmer, 50% for a third swimmer, and no charge for additional swimmers after three. (The swimmer in the most advanced practice group will be charged 100%, the second highest group 90%, etc.) The discount applies to dues only. Families will receive a dues invoice via each month which can be paid by bank draft, credit card, or check. Financial Assistance (FA) for Boise Y Swim Team monthly dues is available. If you are in need of Financial Assistance, the swim team asks that you apply through the Treasure Valley Family YMCA for assistance with their membership fee.

10 Applications for Financial Assistance are available at the front desk. Once you have been approved for assistance by the Treasure Valley Family YMCA, notify the Administrative Assistant of the swim team. The Swim Team will then grant Financial Assistance at the same percentage and in the same method as the Treasure Valley Y. Financial Assistance provided by the swim team is not available for anything except monthly dues. In addition, families on Financial Assistance are expected to regularly volunteer and to participate in Aquathon. Please direct all financial questions to our Team Admin, Patty Stratton ( ). ADMIN FEE: FUNDRAISING: In January, every family will be charged $24 to cover all relays, credit card and bank draft fees, miscellaneous fees, etc. This fee is pro rated for new members who join anytime from January thru August. Swimmers joining the team for the first time after September 1 will not be charged this fee. Because of the expense of staffing three pools and providing a quality program for almost 350 children, monthly dues are simply not enough to cover the costs involved in running our team. Therefore, the Boise Y Swim Team offers two ways for swimmers to meet their fundraising obligation. Each year in October, swimmers can opt to participate in Aquathon. The Boise Y Swim Team feels Aquathon is a fair and equitable way to have everyone participate in fundraising. Swimmers obtain tax deductible pledges from neighbors, friends, and family for swimming a certain number of laps (max of 200 in 2 hours). Families also have the option of making a tax deductible cash contribution to the team in lieu of participating in Aquathon. Recommended goal amounts for 2015 November Aquathon are as follows: Sockeye I and II Group Swimmers = $100; Bronze and High School Group Swimmers = $200; and Silver, Gold, and Senior Group Swimmers = $300. We also have a SCRIP program which runs year round. Details on SCRIP are available for our members on the Fundraising Tab of the website. Swimmers who do not complete their suggested fundraising goal by December 31 will be charged a $50 coaching fee ($150 for national meets) for swim meets with qualifying standards held outside of the LSC. Fees will be applied to meets they attend during the following year. Swimmers who join the team for the first time after December 31 will pay prorated coaching fees based on their Aquathon level and at what point during the year they join the team. LEAVE OF ABSENCE: Because of the size of our team, it has become more and more difficult to keep track of swimmers who take a leave of absence for a short period of time. In addition, it is extremely hard to prepare an annual budget which takes into consideration the income lost by swimmers who decide to take a leave of absence of any length. Therefore, we have developed a Leave of Absence policy. This policy is as follows: A reinstatement fee of ½ of one month s dues at your current monthly dues rate will be assessed upon the swimmer s return to the team after the leave.

11 This reinstatement fee will be waived if the following two criteria are met: 1) Prior to the LOA, the swimmer had been an active member of the swim team for 9 continuous months and 2) the LOA is less than or equal to 3 months. LOA requests must be received no later than the 25 th of the month preceding the leave. UNIFORM & EQUIPMENT: SWIM MEETS: We require all swimmers purchase the Boise Y Swim Team swim suit, team t shirt, and team cap. These items are not required to be worn during practices, only during meets. We also require swimmers purchase swim fins, water bottles, paddles, and snorkels as follows: Sockeye I & II Swim Fins Bronze & High School Swim Fins, Water Bottle, Snorkel, Mesh Bag Silver/Gold/Senior/National Swim Fins, Water Bottle, Paddles, Snorkel, Mesh Bag Most of these items may be purchased at the Swim & Run Shop. Please talk to a member of the coaching staff to purchase a team cap or team shirt. The Boise Y Swim Team hosts 2 3 major swim meets each year. In order to run these meets, we need many volunteers. We ask each swim team family to volunteer 3 4 hours of their time during these weekends. Volunteering is a great way to meet new people and get involved in your child's activity. Swim meets are also major fund raisers for the team. Through the sale of t shirts and heat sheets, we generate funds to help us meet our annual budget. The average meet will cost a swimmer approximately $30 40 in entry fees and may or may not be dependent upon the number of events entered. Some host teams charge a fee based on the number of events entered and some teams charge a flat rate no matter how many events are entered. Swimmers are billed for their entry fees. Our team must pay the host club for all entry fees in advance. A swimmer who enters a meet and does not cancel the entry with Patty Stratton before the check is sent to the host club must pay for his/her entry whether or not he/she ultimately attends the meet. 12 BOISE YMCA SWIM TEAM POLICY FOR CONTROLLING DELINQUENT ACCOUNTS REASON FOR THE POLICY. As a volunteer, not for profit organization, the Boise YMCA Swim Team depends on the cooperation of its members, especially in the matter of finances. Some members are chronically late, erratic, and even delinquent in meeting their obligations. In the past, we have had no established policy in this area and unpaid balances have become alarmingly large without having any way to control them. Financial aid and special arrangements for hardship are supported by the Swim Team and the YMCA, so seriously past due accounts are unfair to everyone those who owe them and those who subsidize them. This policy is not intended to punish anyone. It is a necessary consequence of being a large team requiring business like administration. Few members will be affected by it, but we all need to be reminded our responsibilities are real and not meeting them will have direct, tangible consequences the whole swim team supports. The policy defines the club's expectation for payment of dues and fees and allows for exceptions in special cases. It then defines membership in good standing, how one can lose that status, and what the consequences of that loss are. It also provides for tracking and reinstatement. 1. PAYMENT EXPECTATIONS. Members are billed monthly for swimmer's fees, meet entry fees, and other items which members have agreed to charge to their accounts. Payment is due upon receipt, and will be considered past due on the 15 th of the month. Bills are ed to members after the 25 th of each month; therefore, an account becomes past due if balances owing are not received by the Team Administrative Assistant by the 15 th of the following month. 2. EXCEPTIONS. Members with special financial situations will be accommodated if they make arrangements with the Team Admin. Such arrangements must include a written payment schedule, signed by the member, which brings the balance current within a reasonable time period. It is the responsibility of the member to bring the issue to the attention of the Team Admin, Treasurer, and the Head Coach. 3. MEMBER IN GOOD STANDING. A member in good standing is any member whose account is: a. Current or b. Less than 60 days past due and who has made special payment arrangements with the Team Admin as described in Exceptions. 4. LOSS OF STATUS AS MEMBER IN GOOD STANDING. A member who ceases to be a member in good standing loses membership privileges for themselves and their swimmers, including elective or appointive positions on the management council, appointive positions for team events, subsidy or reimbursements for meets, and eligibility for participation in workouts and meets. -- over --

13 5. PROCEDURE. a. This policy will be included in the registration packet for all new swimmers and will be posted on the team website. b. If a member's account becomes 60 days past due, the Team Admin will mail a copy of this policy, along with the current month's bill, to the member. Families will be notified by the Head Coach that their swimmer should not attend practice until a payment has been made. If no payment is received within 10 days of receiving the mailing containing the bill and delinquent policy or if no payment arrangement has been agreed upon by the family with the Team Admin, the family will be required to put their swim team account on auto pay in order to continue practicing with the team. c. If a member's account becomes 90 days past due, and no satisfactory arrangement has been made with the Team Admin to cure the past due balance, the member will be considered to have lost good standing status and will no longer be allowed to attend practice with the team. At this point, the member s name will be turned over to the Treasure Valley YMCA and their membership account with the YMCA will be flagged and will remain so until a satisfactory arrangement has been made as described in Exceptions. Flagging an account may result in the swimmer not being allowed to proceed past the front desk of the YMCA. Leaves of Absence/Resignation Policies: Members wishing to either resign from the team or temporarily take time off from the team to pursue other activities and interests, must complete the Leave of Absence form by the 25 th of the month prior to the leave (or resignation) and submit this to the Team Admin for processing, in order to avoid being billed and held liable for fees and charges during the period of absence. The team Leave of Absence form can be downloaded from the team website: Because of the size of our team (250 families), it has become more and more difficult to keep track of swimmers who take a leave of absence for a short period of time. In addition, it is extremely hard to prepare an annual budget which takes into consideration the income lost by swimmers who decide to take a leave of absence of any length. Therefore, we have developed a Leave of Absence policy. This policy is as follows: A reinstatement fee of ½ of your current monthly dues will be assessed upon the swimmer s return to the team after the leave. This reinstatement fee will be waived if the following two criteria are met: 1) Prior to the LOA, the swimmer had been an active member of the swim team for 9 continuous months and 2) the LOA is less than or equal to 3 months. LOA requests must be received no later than the 25 th of the month preceding the leave. -- over --
