For assistance, please call Definitions As used in this list: * Targeted Case Management is a type of service a person may receive when seeking mental health treatment. Working together, our staff and the individuals they serve, will identify resources in the community that will be helpful in promoting recovery. Individuals are then linked to these additional resources. ** Mental Health Services are provided for people with severe emotional and mental distress. They may include group therapy, individual therapy, and targeted case management. All services are geared toward improving one's quality of life and experiencing recovery. *** Psychiatric Inpatient Hospitals are facilities that specialize in the 24 hour care and treatment of people greatly effected by a serious mental disorder. **** Cultural Specific Services are services that are offered in languages other than English or that are designed to address cultural differences. 14 KCMH Assessment 2151 College Ave a.m. - 5 p.m. Center Bakersfield, CA Spanish Assessments, Crisis and Psychiatric Evaluation The Assessment Center is a central point of access into the Kern County Mental Health Department for adults aged 18 and older. Initial screenings and assessments are provided to adults and seniors. Minors seeking non-crisi mental health resources will be given information about how to access services through their geographic provider of children s mental health services. The Assessment Center provides assistance with community services and referrals to determine proper service placement. The center includes specialized assessment, and referral for substance abuse as well as **mental health services. 15 KCMH Northeast 3715 Columbus a.m. - 5 p.m. Brief Therapy Clinic Ave. Bakersfield, CA Spanish languages Adult Services The Northeast Brief Therapy Clinic (NBTC) is an ACA team whose primary goal is to provide brief, recovery-oriented therapy. Case management and medication management is provided on a limited basis, as needed basis. The NBTC serves adult clients by assisting them with moderating their mental health symptoms and impairments, then re turning to the community supports for continued services. The client s natural strengths and resources will be explored and expanded and those who do require medication management will be referred back to their primary providers after reducing the impairments and symptoms that necessitated specialty mental health services. 16 KCMH Crisis Case 3715 Columbus a.m. - 5 p.m. Management Ave. Outreach, Adult Bakersfield, CA Wraparound, Freise a.m. 12 pm Saturday & Hope House Sunday Liaisons (Freise only) Spanish Adult Services, Wraparound Services Crisis Case Management Outreach (CCMO) is the brief intermediary between initial crisis contact and appropriate KCMH and community services. Individuals served are new to the KCMH System Of Care (SOC). Referrals are received from inpatient psychiatric units. Services include screening, discharge planning, intensive *targeted case management, and linkage. Adult Wraparound assists KCMH SOC providers to discharge individuals from inpatient units, and provide brief intensive outpatient treatment to augment that of existing KCMH teams. Services are provided in the greater Bakersfield area. The Freise Hope House Liaisons provides screening and linkage to the Freise Hope House. Services are provided for individuals living at the facility, and 30 days post discharge from the facility. Services include screening, hospital discharge planning, intensive *targeted case management, and community linkage. Referrals are received from inpatient psychiatric units, the psychiatric evaluation center and KCMH outpatient teams. CCMO, AWA, and the Freise Hope House Liaisons have bilingual (Spanish speaking) staff available. C Bakersfield, CA Bilingual Staff Spanish, Filipino Language Line Transitioning Adults (ATT)/Homeless Adult Team (HAT) Provides services to individuals involved in the legal system that are homeless, or at risk of homelessness. Focus on crisis and jail outreach services, both of which are needed to prevent recidivism and homelessness. Homeless Program Provides intensive, short-term *Targeted Case Management, which targets adults 18 years and older, who are homeless with chronic and major mental illness, or dual diagnosis. Case managers assist individuals in obtaining services for mental health, medical, legal, financial, housing, daily living skills, and various community support services. KLP provides groups, individual therapy, and family therapy in Spanish. Substance abuse counseling is also available in Spanish. Staff at this clinic provide services to people from all ethnic and cultural backgrounds. Assertive Community Treatment Team (ACT) Provides services to individuals with severe and persistent mental illness who require more intensive service treatment. The ACT team follows an evidence based model with demonstrated effectiveness in assisting individuals who may not of benefited from traditional **mental health services. Provides mental health and physical health services, vocational/educational, life skills, housing and social opportunities to help in recovery. Wellness, Independence, and Senior Enrichment Team (WISE) Provides service to the older adult population ages 60 and older. Provides mobile services to reach out and provide age appropriate services in the home, primary care offices, and other settings where older adults are most comfortable. Services include integrated case assessment/planning and management, intensified **mental health services, crisis intervention when needed, and referrals to community resources. cultural groups. Long Term Care (LTC) LTC admits clients for services when they are court ordered to either LPS Conservatorship (for grave disability) or a Post- Certification 180-Day Hold (for dangerousness). LTC oversees these clients admissions to state hospitals, county-contracted locked facilities and enhanced board and cares. In addition, LTC also monitors their treatment there and plans their discharges. LTC works collaboratively with the Conservator s office, psychiatric acute and residential facilities, County Counsel, and co-manage treatment with outpatient teams until the client is returned to the community. clients from the following cultural groups: older adults, Hispanic American, and LGBTQ. Children Services Children s Outpatient Service provides a variety of assessment, *Targeted Case Management, medication, and treatment services to individuals (ranging from preschool aged children to young adults over 18 years, if they are still enrolled in school) who will benefit from these services. Children s Outpatient Service strives to be consumer centered, racially and culturally appropriate, flexible, with a focus on individual/family s strengths, to promote positive peer and professional relationships, and incorporate each individual/family s natural support system within the treatment process. Services provided include, but not limited to: screening/triage, assessment, psychiatric services, i.e. medication management and medication support, psychological testing, individual, group, and/or family therapy, psycho-education, individual/group rehabilitation services, social skills groups, and *Targeted Case Management. Specialty Services Provides services to children and adolescents with inappropriate touching issues referred from Juvenile Court or the Probation Department for treatment. The team also provides treatment for victims of sexual abuse as well as their parents or guardians who are referred by social workers, probation officers, and/or other mental health teams. Blanton Academy School based **mental health services are provided to first time juvenile offenders who are directed to attend by Kern County Probation as well as the Juvenile Court. Services include individual treatment, group treatment, family treatment, crisis intervention, and substance abuse prevention. Juvenile Probation Psychiatric Services Provides the full spectrum of **mental health services to incarcerated youth at Juvenile Hall. Emphasis is placed on linking youth with their geographic mental health provider upon release. A bilingual therapist is assigned to each team. Staff at this clinic provide services to people from all ethnic and cultural backgrounds.
