2 Assertive Community Treatment (ACT) Assertive Community Treatment is a set of intensive clinical, medical and psychosocial services provided by a mobile multidisciplinary treatment team. This service is for individuals living with a serious mental illness who require intensive services and supports and if, without ACT, would require more restrictive services and/or settings. ACT is a team-based service that is individually tailored combination of services and supports that may vary in intensity over time based on your needs and condition. Services include multiple daily contacts and 24/7 crisis availability provided by a team which includes psychiatric and skilled medical staff. The team also provides basic services and supports essential to maintaining your ability to function in community settings, including assistance with accessing basic needs through available community resources, such as food, housing, and medical care and supports to allow you to function in social, educational, and vocational settings. ACT services are based on the principles of recovery and person-centered practice and are individually tailored to meet your needs. ACT services are provided in your home or other community locations by members of the ACT team.
3 Multi-Systemic Therapy (MST) MST is a family- and home-based treatment that strives to change how youth function in their natural settings home, school, and neighborhood in ways that promote positive social behavior while decreasing anti-social behavior. This Multisystemic approach uses an ecological model, which views the youth as living in a network of interconnected systems other than the home, decrease the adolescent s involvement with deviant peers and promote friendships with pro-social peers, improve the adolescent s academic and/or vocational performance, and manage the challenges presented by criminal activity that may exist in the neighborhood. Treatment teams typically consist of three MST-trained, master s level counselors who receive clinical supervision from an experienced, MST-trained mental health professional. Each treatment team provides services for approximately 50 families a year. The MST training curriculum consists of a five-day orientation training, quarterly booster sessions, weekly on-site clinical supervision for treatment teams and supervisors, and weekly consultation from a doctoral- or master s-level MST expert. MST targets chronic, violent, or substance-abusing male and female juvenile offenders at risk of out-of-home placement. The typical MST youth is years old, lives in a home that is characterized by multiple needs and problems, and has multiple arrests. As shown below, research-based models of delinquency and drug use indicate that family, school, and peer factors influence delinquent behavior.
4 Home-Based Services Mental health home-based services programs are designed to provide intensive services to children age 0-17 and their families with multiple service needs who require access to an array of mental health services. The primary goals of these programs are to support families in meeting their child s developmental needs, to support and preserve families, to reunite families who have been separated, and to provide effective treatment and community supports to address risks that may increase the likelihood of a child being placed outside the home. Treatment is based on the child s needs, with the focus on the family unit. The service style must support a family-driven and youth-guided approach, emphasizing strength-based, culturally relevant interventions, parent/youth and professional teamwork, and connection with community resources and supports.
5 Outpatient Therapy Individual/group therapy includes activities designed for the purpose of reducing maladaptive behaviors, maximizing selfcontrol, restoring normalized psychological functioning, reality orientation and/or emotional adjustment and re-motivation, thus enabling improved functioning and more appropriate interpersonal and social relationships. This includes psychotherapy, play therapy, behavioral treatments, solution focused and others. Family or couples therapy is conjoint therapy of a beneficiary and family member(s), or other person significant to the beneficiary for the purpose of improving the beneficiary/family function. Family therapy does not include individual psychotherapy or family planning (e.g., birth control) counseling. Family therapy is provided by a mental health professional.
6 Targeted Case Management & Supports Coordination Targeted case management helps you design and implement strategies for obtaining goal-oriented and individualized services and supports. Services may include assessment, planning, linkage, advocacy, coordination, and monitoring to help you gain access to needed health and dental services, financial assistance, housing, employment, education, social services, and other services and natural supports developed through the person-centered planning process. Supports Coordination is ongoing services provided to insure that a full range of options is available to you and your family. This will include development of a treatment plan, and may involve linking to, coordinating with, follow-up of, advocacy with, and/or monitoring of DD specialty services, Behavioral Health Services and other community services/ supports as identified in the planning session. This also may include brokering of providers of services/supports, assistance with access to entitlements and/or legal representation, and coordination with QHP or other health care providers. These services are typically provided in non-office settings including your home, adult foster care, clubhouse, vocational program or in the community.
