1 MISSION STATEMENT To promote, develop, and enhance collaborative efforts between school, behavioral health, community resources, child protective service systems and parents to improve the well being of children in County 1

2 Updated 01/2014 nb GOAL To bring together a group of system, community, and school partners to raise awareness of the various services available to children and their families. We aim to ensure that families are matched appropriately so they receive needed services. A MESSAGE FROM THE BRIDGING THE GAP COMMITTEE: Bridging the Gap committee members realize the child serving systems can be confusing. This guide contains a list of resources that provide or assist families with housing/utilities, other basic needs, employment search/placement, education/skills training, transportation, child care, substance abuse, healthcare, disabilities, mental/behavioral health, self sufficiency skills/support, LGBTQ, recreation activities, legal assistance and domestic violence issues. Some of the resources listed in this guide are locally accessed and funded. Some services are state contracted. Currently, the State of New Jersey Department of Children and Families maintain a contract with PerformCare to provide a central point of entry when accessing behavioral health services. PerformCare provides information to families and connects them to the appropriate resources. 3 New Jersey Department of Children and Families provides families with a virtual single point of contact that registers and coordinates care for children who are screened at any level - into its System of Care. To provide this simplified access, DCF contracts with PerformCare, which is a private entity that administers this service system To access services for children and youth, call the 24-hour, toll-free Access Line at: The following services are available: Mobile Response and Stabilization Services Care Management Organization Family Support Organizations Who is PerformCare? ( PerformCare functions as the single point of access for youth into the Children s System of Care. They are a phone-based service and serves as a source of information and referral: (TDD: ). Remember, this is a toll-free call! PerformCare is contracted through the Division of Children s System of Care (DCSOC) to coordinate the care for the needs of youth. You can call PerformCare at any time. Clinical staff assess the needs of your youth. Parents, family members, school employees, mental health providers, and other professionals helping children and families may contact PerformCare on behalf of a youth in need of services. Remember, the parent/legal guardian of the youth must give consent for services. Examples of PerformCare responsibilities include: Provide 24-hours a day, 7 days a week availability. Coordinating access to services for all eligible youth. Helping youth obtain any necessary specialized behavioral health services. Supporting the DCSOC goals of promoting best practices and assisting the state in assuring compliance with state and federal guidelines. Offering complaint, reconsideration, and appeal processes. 3

The Red Cross can provide immediate food, clothing and temporary housing impartially, based on individual need, free of charge. It also offers referrals to other social service agencies that assist in the resident's personal recovery. Other emergency services include training classes in shelter care, family services (including house fire response), and disaster preparedness for hurricanes, floods, tornadoes and other natural disasters. Armed Forces Emergency Communication Notification to and from military personnel for emergency verification of urgent situations anywhere in the world they are stationed. Red Cross health and safety services offer first aid, CPR (cardio pulmonary resuscitation) and AED (automated external defibrillator) trainings for both the lay responder and professional rescuers. Additional health and safety classes include babysitting, basic aid training for elementary age children and first aid training for even pre school participants.
