2 REQUEST FOR QUOTES FOR 2015 PULVERIZING & MILLING ASPHALT PAVEMENT Sealed quotes for furnishing and delivery of 2015 Pulverizing and Milling Asphalt Pavement will be received by the Fond du Lac County Highway Department, P.O. Box 1234, 301 Dixie Street, Fond du Lac, WI until Wednesday, February 25, 2015 at 11:00 a.m. local. Quotes will be publicly opened and read aloud at that time at the Fond du Lac County Highway Department. Specifications, Special Provisions and Proposal forms may be obtained at the Highway Commission Office, 301 Dixie Street, Fond du Lac, Wisconsin, upon request. No quote shall be withdrawn after the opening of quotes for a period of thirty (30) days after the scheduled time without the consent of the County Highway Commission. The Fond du Lac County Highway Commission reserves the right to accept or reject any or all quotes or to accept any quote which in their opinion is the most advantageous to Fond du Lac County. 3 1. General SPECIFICATIONS FOR 2015 PULVERIZING & MILLING ASPHALT PAVEMENT The Fond du Lac County Highway commission is seeking quotations for Pulverizing & Milling Asphalt Pavement. The quotations shall be quoted on an hourly basis for equipment used in the recycling process. The County shall be supplying all manpower and equipment available to the County for the recycling process. The purpose of these informal quotations is to obtain hourly rates for machines needed in the recycling process not owned by the County. 2. Scope of Work Fond du Lac County has approved projects requiring pulverizing & milling asphalt pavement that are identified in the 2015 budget. The County shall be doing all work related to the cold in place recycling except the actual milling or pulverizing of the asphalt pavement. All hourly rates shall be quoted for production hours only. All machine repairs, breakdowns, teeth, fuel, labor and water supply for the machine shall be included in the hourly rate per hour. 3. Pulverizing and Milling Asphalt Pavement The cold in place recycling shall be accomplished by milling full depth into the gravel sub-base or to maximum depth of 16 if required. The depth of milling shall be determined by the asphalt pavement thickness. This milling depth shall be done in one milling pass to a width of 8. The recycled asphalt shall be milled to a size passing 1-1/2 sieve. This milled asphalt shall be free of asphalt chunks larger than 1-1/2 in a majority of the milled material. A few larger size chunks of asphalt can develop from time to time but they must be kept to a minimum. Existing milled asphalt shall be milled to a depth of 1 below the bottom of the existing asphalt layer full depth in one pass of the milling machine. All support equipment shall be supplied by the County crews such as: rollers, signs, labor for miscellaneous work, graders, flagging, detours if needed, and back hoes. The work shall be done with the road open to traffic. The Wisconsin Standard Specifications for Road and Bridge Construction most recent edition shall be considered a part of these specifications. The prices per hour quoted for milling equipment shall be considered full compensation for equipment, materials, labor, parts and repairs associated to the operation of the equipment. Page 2

4 Mobilization shall be quoted as a lump sum payment and an hourly rate quoted for moving the milling machine in Fond du Lac County from job to job. The County will require the successful bidder to provide two (2) machines on certain pulverizing and milling projects for the purposes of higher production. 4. Insurance Requirements Certificate of insurance indicating that required levels of insurance coverage s are enforced at the time the proposal is submitted should be included as part of this proposal. A. Workers Compensation Insurance consisting of statutory compensation benefits and $100,000 of employer s liability. B. Comprehensive or Commercial General Liability Insurance with limits of not less than $1,000,000 combined single limit per occurrence and aggregate. This coverage shall include contractual liability coverage. C. Comprehensive Owned and Non-owned Automobile Liability Insurance with the limits of $1,000,000 combined single limit per occurrence and aggregate. D. Professional Liability Insurance coverage with an annual occurrence and aggregate limit of not less than $1,000, Instructions All quotes are to be sealed and marked 2015 Pulverizing and Milling Asphalt Pavement, and addressed to the Fond du Lac County Highway Commission, 301 Dixie Street, P.O. Wage Requirements This work is to be performed on projects that require prevailing wages, including but not limited to: CTH G & S 3.09 miles (Co. Line to Youth Camp Rd), CTH T 4.96 miles (CTH W to CTH G), CTH BB 1.66 miles (Co. Line to CTH W), and CTH W 3.22 miles (STH 23 to CTH W). Pay no less than prevailing wages required by the State of Wisconsin Department of Workforce Development. 5 7. Estimated Quantities & Schedule The Highway Department anticipates approximately 1 mile of milling and 13 miles of pulverizing will be done, however final mileage is yet to be determined. The successful bidder shall be asked to coordinate the machine schedule with the County schedule which is expected to begin in early to mid May Page 4
