Hooksett, NH (603) REQUEST FOR PROPOSAL TOWN OF HOOKSETT SNOW PLOWING SERVICES FOR SCHOOL PARKING LOTS BID # Acceptance Date: Tuesday, December 22, 10:00am Sealed bid proposals, plainly marked Snow Plowing Services for School Parking Lots Bid # On the outside of the mailing envelope as well as the sealed bid envelope, addressed to the Administration Department, 35 Main Street, Hooksett, NH 03106, will be accepted until Tuesday, December 22, 2015 at 10:00 when all bids will be publicly opened and read aloud. Bid packages may be obtained from the Town s web site: or by contacting Administration Department at the above address, or by calling (603) The Town of Hooksett reserves the right to reject any or all bids, to waive technical or legal deficiencies, and to accept any bid that it may deem to be in the best interest of the Town.

2 Snow Plowing Services for School Parking Lots The Town of Hooksett is seeking a proposal for snow plowing for the Town School parking Lots during the winter season. General information and specifications are as follows. Scope of Work All equipment, vehicles, and labor will be provided by the Contractor. 1. Snow plowing will be done at the following schools (will consider salting and sanding) Underhill School Memorial School Cawley School 2. Remove all snow from entrance roadways, drives, parking lots, and other areas of the three facilities (sidewalks are excluded). 3. Snow must be plowed in any storm with an accumulation in excess of 1.5 inches of heavy snow, or 2 inches of fluff or light snow, or at the direction of the Hooksett Highway Crew Chief or the Department of Public Works Director. 4. In the event of a school day, all snow must be plowed prior to 7:00AM. 5. In the case of No School and snowfalls occurring during weekends or school vacations, all plowing must be completed by 9:00AM of that day. 6. In cases of storms accumulating two (2) inches or greater commencing after school is already in session, school drives shall be plowed prior to 2:00 PM 7. Snow shall be directed away from buildings and sidewalks and to the outside areas of parking lots. 8. The Contractor shall keep snow levels at the traffic intersections and merge areas below the level for clear visibility of vehicle operators

3 9. If negotiated, sanding and salting shall be started immediately in the event of slippery conditions due to ice storms, freezing rain, or icy conditions or communication to the Highway Crew Chief must be made immediately. 10. Contractor must be capable of maintaining telephone contact with the Highway Crew Chief or the Public Works Director to receive calls on no school or school/delayed openings, or to respond to emergency service calls for 24 hours a day, 7 days a week. 11. Contractor shall provide Certificate of Insurance with the Town of Hooksett to be named as an additional insured prior to commending any work. Insurance and Indemnification CONTRACT REQUIREMENTS To the fullest extent permitted by law, the Contractor shall indemnify and hold harmless the Town of Hooksett, and their boards, agents, officers and employees or any of them from and against any and all claims, damages, losses and expenses, including but not limited to attorneys fees arising out of or resulting from performance of the work provided that such claim, damage, loss or expense is attributable to bodily injury, sickness, disease or death, or to injury or to destruction of tangible property caused by negligent and/or intentional acts or omissions of the Contractor, a sub-contractor or anyone directly or indirectly employed by them or anyone for whose acts they may be liable, regardless of whether or not such claim, damage, loss or expense is caused in part or by a party indemnified hereunder. The indemnification shall survive the completion of work and is binding on the Contractor and any of its subcontractors. Insurance to be Carried by Contractor Prior to commencement or work under this contract, the Contractor will provide the Town of Hooksett with Certificates of Insurance, in companies acceptable to the Town of Hooksett, evidencing: 1. Worker s Compensation in accordance with the laws of all jurisdictions which may apply to work being performed and including coverage for Employer s liability with a limit of not less than $100, each accident/$500,000 Disease-Policy Limit/$100,000 Disease- Each Employee 2. Comprehensive General Liability with Limits of not less than $1,000, per occurrence. The insurance will include the following hazards and the certificates will so indicate: a. Independent contractors covering the Contractor for any work performed by subcontractors b. Contractual including coverage for the above indemnity agreement. c. Completed Operations covering work performed under this contract 3. Comprehensive Automobile Liability covering all owned, non-owned and hired vehicles of not less than $1,000, Combined Single Limit 4. The certificate of insurance and policy endorsements shall name the Town of Hooksett as an additional or co-insured under the general liability and auto coverages.

4 These certificates and policy endorsements will indicate that the Town of Hooksett will be advised not less than thirty (30) days prior to any change or cancellation. Such insurance will be maintained for the duration of this contract. Completed operations coverage, where applicable, will be maintained for not less than three (3) years after end of operations under this contract. PRICING: The Town of Hooksett will provide a weekly payment of five (5) hours stand by rate to the regularly scheduled hired trucks during the winter maintenance season, from the contract signed day through March 25 th, This payment would be based on the equipment contracted to use. However, note that these hours will be adjusted for the hours actually worked during the week long pay period. For instance, if during the week pay period a truck plowed for 4 hours, the Contractor would be paid for the four (4) hours plow rate and one (1) hour of standby rate. If a truck plows for ten (10) hours, the contractor would be paid for the (10) hours actually worked and no standby time. Pricing per hour is all inclusive (vehicles, labor, fuel) 6 Wheel dump Truck w/wing $70.00 per hour 6 Wheel dump Truck (1 ton) $55.00 per hour 4 Wheel Drive (pick up) $50.00 per hour 10 Wheel dump $75.00 per hour Any other equipment can be negotiated if applicable. Qualifications: Please list all qualifications and references on similar projects Criteria for Reviewing Proposals: In reviewing proposals, the Town will carefully weight qualifications, experience and pricing. The bid award will be made by Wednesday, December23, Submission: Pricing must be inclusive, clear and concise, including such other information as requested or required. Please use attached form. Be sure the proposal envelope is completely and properly identified. The face of the envelope shall indicate the title of RFP Snow Plowing Services for School Parking Lots, Bid # Proposals must be received by the Town of Hooksett ON or BEFORE the hour specified on the acceptance date. Proposals maybe either mailed or hand delivered to the Town Hall, 35 Main Street, Hooksett, NH 03106, Attn: Dean E. Shankle, Jr., Town Administrator.
