Confirmation of request All Students with Military Service. All Students with prior college. Required Documentation! CH 33 (Post 9/11 ): The Certificate of Eligibility is required by us to protect your courses from being dropped for nonpayment beyond the first day of class.! Ch 31: A valid 1905 for Richland College is required to be on file to protect your courses from being dropped for nonpayment beyond the first day of class. ATTACHED VONAPP Application (see reverse for application number) And Certificate of Eligibility Print your submission confirmation with notations on the page indicating the form you submitted. Copy of Certificate of Eligibility All Veteran and Dependent students that have applied, but not yet received, their certificate of eligibility letter. All Transfer students. All Veteran and Dependent students that have completed the VONAPP application and received this letter. DCCCD Datasheet Completed & signed All Students. Certificate of Understanding Completed & signed. All Students. OTHER Official Degree Plan Request Form Completed & signed (Apply for Financial Aid You may be eligible even if you are receiving VA benefits.) New students or prior students requesting a change in degree plan. Registration Receipt Received from advising or printed from econnect after signing up for classes. All Students. See reverse for information regarding payment. DD214 Provide member- 4 OR service - 2 copy (Copy must show character of discharge). All Veterans. 2 Official Military (mandatory) REV 05/13/13 Veterans must submit military transcripts in order for an Official Degree Plan to be provided. Veterans may earn college credit for their selected degree program based on their military experience and training. Request transcripts online: Joint Services Army, Coast Guard, Marine Corps, and Navy: Active Duty, Reserve and Veterans. Air Force CCAF: USCG : All Prior College Official The VA only pays for courses that have not been met by prior coursework. Failure to submit all transcripts could affect your benefits. Official transcripts are requested, by you, from prior schools. They will be provided in a sealed envelope with the college registrar's signature or other marking across the seal. The official transcript must remain in the unopened, sealed envelope until it is presented to admissions. Some institutions may use a secure system to provide electronic transcripts, direct to the institution. These transcripts are required in order to process the official degree plan needed to receive VA benefits. Certificate of Eligibility (COE) This letter, from the VA states the benefit you are eligible for and the number of months available. If you do not have this letter you may provide the VA Application printout, as described below, until letter is received. Post9/11 (CH33) Veterans: This letter is required to hold classes beyond start date, without payment. VA APPLICATION The VONAPP (Veterans On Line Application) website is an official U.S. Department of Veterans Affairs (VA) website that enables service members, veterans and their beneficiaries, and other designated individuals to apply for benefits using the internet. Allow at least 3 weeks for the VA to respond to new applicants Determine the correct form from list below: Voc Rehab Veterans (CH 31) complete with VA Counselor First Application: Veteran/ Active Duty/Reserves R VRAP Application: Veteran School or Degree Change: Veteran/ Active Duty/Reserves First Application: Dependent/Spouse (CH 35) School or Degree Change: Dependent/Spouse (CH 35) DCCCD Datasheet Certificate of Understanding Official Degree Plan Request Form Official Degree Plan Registration Receipt The name and contact information listed on this form will be used when we report your enrollment to the VA. You will receive an automated , at the address you provide, when your enrollment is reported to the VA. This form is your acknowledgement that you have read (or asked for clarification of) and understand your responsibilities in ensuring your benefits. The Official degree plan is special in that it is the document that shows where all of your prior transcript credits have been applied. This is the documentation used by our office to verify that you are enrolled in courses that you MUST take to be granted your degree. VA benefits are only payable for courses REQUIRED (not recommended ) by YOUR Official Degree Plan or advising report. When this request form is submitted, all prior coursework and military credits are evaluated and applied to the degree. You will only receive benefits for the courses that remain required after this evaluation. If registering BEFORE your transcripts have been evaluated, Veterans should not enroll in Health/Wellness or Elective courses as these are generally met with the military transcript. After signing up for classes this document will be available. It shows your courses, their cost and the payment due date. For information regarding payment requirements see attached Certificate of Understanding. DD214 Certificate of Release or Discharge from Active Duty. Issued upon a military service member's retirement, separation or discharge from active- duty military. If you have lost your DD- 214 apply for copy at: service- records/

5 Certificate of Understanding Standards of Progress Administrative 1) Apply for Admission to the college, online or at the admissions office. 2) See an advisor. When signing in inform them you are using VA benefits so you can be referred to the appropriate advisor. 3) Take all the College Assessments that are required by Admissions. 4) Failure to submit ALL prior college transcripts may cause a debt or slow receipt of payment. 5) Register Early. The Veteran Services Office can take 30 days plus for our office, followed by the Regional VA Office which can take equally as long. Be financially prepared. 6) Only classes REQUIRED to complete your Official Degree Plan or Primary School Letter will be paid by the VA. Classes described as recommended or needed for transfer do not meet this requirement. 7) Developmental Coursework (i.e. DMAT, DWRI), will NOT be PAID unless taken in- person ( lecture ), AND unless required by test results. 8) Monthly verification at (WAVE link) is REQUIRED by students using VRAP, CH 30, 1606/7 students. 9) REPORT all class schedule changes (add/drop) to the Veteran Services Office to prevent delays or debts. 10) Benefits may be SUSPENDED if satisfactory progress (e.g. 2.0 GPA) toward completion of your degree is not maintained. 11) Degree plan changes must be processed through the college Veteran Services Office. 12) Students may be required to pay back portions of the benefit received: a) If your instructor indicates that you have not attended or stop participating in the course. b) If you drop a course or receive a non- punitive grade (i.e. I, E, W, CR). c) If you receive a punitive grade (i.e. F or N) and have unsatisfactory attendance. 13) Registration Payment Requirements (assumes all students have completed page1 checklist and additionally, CH33 students have provided COE and CH31 have provided ): a) Payment NOT required: CH % (POST 9/11) OR CH 31 (VOC REHAB) In- state tuition. b) Payment required before next registration period: CH 33 < 100% (pay % owed) or CH 33/31 out of state must pay amount over in state rate (take COE to admissions to see if you qualify for waiver). c) Payment in full or Payment Plan required by tuition due date: This includes Chapters 30/35/1606/1607 and CH 33 student who are not at 100%. 14) NOTE: If using Federal Financial Aid (i.e. Pell, loans), or the Texas Hazlewood Exemption, or Tuition Assistance, it is your responsibility to have approval prior to your tuition due date. I further certify that, where unclear, I have sought a Veteran Services Representative to explain the Standards of Progress, and received satisfactory answers to all of my questions. I am aware that the U.S. Department of Veterans Affairs may suspend my benefits during any period for which I am not in compliance with their established standards. I understand that my educational benefit is federally funded and can be changed at the discretion of the U.S. Department of Veterans Affairs.
