1 lfd Veterans Services Vocational Rehabilitation, Chapter 31 Enrollment Packet If applying for the first time for your educational benefits, please submit the documents listed along with the information contained in this packet. If you are requesting a parent institution letter or will be transiently/concurrently enrolled here please see Veterans Services Office before completing any forms. Authorization and Certification of Entrance or Reentrance into Rehabilitation and Certification of Status VA form Degree Plan Worksheet Please have your advisor indicate where the classes you are taking fit into your degree. This will need to be updated every semester. Class Schedule Anyone with college hours from another institution will have a transfer evaluation done within the first semester of your attendance. All subsequent enrollments will only require you to submit: Request for Educational Certification Please Note: You must request certification for each semester you wish to receive benefits. You are responsible for checking with our office to see that we have received your from the VA office at Almeda by the regular registration payment deadline. 2 CHANGES IN YOUR ENROLLMENT IfYou Withdraw The first time you withdraw from up to 6 credit hours or equivalent of half-time training in any term, we will excuse the withdrawal and pay benefits for the period attended. Remember: This only applies to your first withdrawal. If you again withdraw from one or more courses after the end of your school's drop period, receive grades that do not count towards graduation requirements, you may have to repay any benefits from the beginning of the term. You may not have to repay the benefits if you show that the change was due to reasons beyond your control. The reasons must be unavoidable and unexpected events that directly interfere with your enrollment. Examples of reasons we may accept are: Extended illness Severe illness or death in your immediate family Unscheduled changes in your employment Lack of child care Examples of reasons we may not accept are: Withdrawal to avoid a failing grade Dislike of the instructor Too many courses attempted Note: We may ask you to furnish information to support your reason for a change. If serious injury caused the change, obtain your doctor's statement. If a change of employment caused the change, obtain your employer's statement. We Can Excuse The first time you withdraw up to 6 credit hours, we will excuse the withdrawal and pay benefits for the period attended. Remember: This only applies to your first withdrawal. Grades You Receive If your school reports that you received one of the following grades, you may have to repay benefits for the course: An "I" grade for an incomplete which is not made up during the time period required by your school or within 1 year from receipt, whichever is earlier A "W" or other grade for withdrawing An "AU" or other grade fro auditing Generally you do not have to repay benefits for a failing grade.

3 lfd Veterans Services Statement of Understanding for Vocational Rehabilitation Program *** The intent of this form is to clarify the issues about your subsistence allowance as well as the payment of tuition, fees, books, and supplies. *** SUBSISTENCE ALLOWANCE Your award is based upon certification of your enrollment provided by your UHD VS Representatives. Please be aware that VA can only pay for those courses that apply towards your approved curriculum, as outlined in your Individualized Written Rehabilitation Plan (IWRP). An academic advisor can provide you with a curriculum outline or degree plan that details the courses you should take and pass each term in order to graduate within the agreed upon time frame that you and your Vocational Rehab Case Manager agreed upon during the development of your IWRP. If these dates are no longer accurate, please consult with your case manager to discuss the inconsistency and to ensure that your plans covering current dates for the services provided. Also, be aware that the Veterans Services office completes your certification and does not provide subsistence allowance. If you have questions, contact your Vocational Rehabilitation Counselor. You also need to be aware that there are certain responsibilities you have during your training program. It is your responsibility to come to the VS Office to submit your class schedule, schedule changes or to make any changes to your record at UHD. Upon registration, you must submit a VA form 1905 to the Veterans Services office, Bookstore and Student Accounting. This form provides authorization to have your tuition and fees paid for. It is your responsibility to report your registration or add/drops to your Voc Rehab Counselor and the overpayments from the VA when you make changes to your registration. If you should ever receive a payment that you know you are not entitled to or question, you must contact your Voc Rehab Counselor. Failure to follow these guidelines can result in suspension of your training allowance. TUITION, FEES, BOOKS, AND SUPPLIES The VA will be responsible for the payment of your tuition, required fees, books, and supplies. VA will be billed for these fees directly from the Office of Student Accounting and the UHD Bookstore and in most instances you should have no out-of-pocket expenses for any REQUIRED costs. Please be aware that VA will only pay for those courses, fees, books and supplies applicable to your approved curriculum, required of all students in your program or special items necessary due to your disability. 4 complete your assigned coursework. If you have any special needs, please contact your Vocab Rehab Counselor. Occasionally, VA is forced to terminate a vocational rehabilitation program due to abuse of supply purchases. A 1905 and authorization letter will be provided to the Office of Student Accounting and to the bookstore for tuition, fees, books and supplies. 5 lfd Veterans Services Standard of Progress for Receipt of Veteran Benefits The function of the Veterans Services Office is to assist students with completion of proper forms and coordinate the certification procedures for monthly benefits: (a) Student receiving veteran educational benefits must provide official transcripts of all previous colleges attended in order to apply prior credits to current course of study. (b) Students receiving veteran benefits are eligible for tutorial assistance and should contact the Veterans Services Office if assistance is needed. Veteran regulations require that a student receiving veteran educational benefits select a degree objective and make satisfactory progress toward completion of that objective. Veteran students must maintain a cumulative 2.00 grade point average to receive benefits. Veterans who earn an accumulative grade point average of between 1.00 and 1.99 will be placed on probation and given two semesters to achieve the 2.00 requirement. Failure to obtain a cumulative 2.00 grade point average will result in VA suspension of benefits. Also students with less than a 1.00 cumulative grade point average will be suspended from benefits without a probationary semester. Once suspended, students must obtain a 2.00 cumulative grade point average to re-establish eligibility.
