1 HAI - 1 Coronary Artery Bypass Graft (CABG) Surgical Site Healthcare-associated Infections A bypass surgery (Coronary Artery Bypass Graft - CABG) is typically done to treat heart attack patients who have experienced injury to their heart due to the heart muscles not receiving enough oxygen-rich blood. This condition is primarily caused by disease in the major heart arteries (coronary arteries) that supply heart muscles with blood. Diseased heart arteries may have fatty deposits and clots which prevent blood from reaching the heart muscles. A bypass surgery takes veins or arteries from another part of the patients' body and uses these vessels to reroute the supply of blood around the blocked arteries ( bypassing the diseased arteries) to the heart muscles. Healthcare workers must take precautions to prevent infections related to this surgery. The measurement of infection outcomes is challenging for many reasons: The tools and metrics used to track infections are evolving at the national level. Thus, there currently exists variability in the reliability of hospitals self-reported measurements of infections. Patients differ in many ways. Some patients have a higher or lower risk of infection compared to another patient. Hospitals that care for substantial numbers of patients at high risk of infection may have higher infection rates than hospitals that do not have substantial numbers of high-risk patients in their care. In addition to patient differences other factors that may impact differences between hospitals infection rates include hospital measurement capacity, referral status, and frequency of high risk procedures. This is a hospital self-reported measure of the rate that CABG surgery patients experienced an infection after their surgery. In Iowa in, approximately 3 out of every 100 CABG surgery patients experienced a surgical site infection. Lower rates for this measure are better. 11 HAI 5 - Central Line-associated Bloodstream Healthcare-associated Infections A central line (an intravenous [IV] catheter placed into a large vein) is inserted for many purposes - delivering nutrition, medicine, and assessing how well the heart or lungs are working. Because a central line catheter is inserted through the skin, into a vein, and delivers fluids directly into the bloodstream, it is easy for bacteria to enter the bloodstream and cause infections. Healthcare workers must take precautions to prevent such infections. In addition to patient differences other factors that may impact differences between hospitals infection rates include hospital measurement capacity, referral status, and frequency of high risk procedures. This is a hospital self-reported measure of the rate at which patients hospitalized in a medical, surgical, or combined medical/surgical Intensive Care Unit (ICU) with a central line catheter experienced a central line-associated bloodstream infection (CLABSI). In Iowa in, there was approximately 1 infection per 1,000 days a cental line catheter was utilized in medical, surgical, or combined medical/surgical ICU patients. 14 HAI 6 - Health Care Worker Immunization Rate Influenza (flu) is an infection of the lungs and airways with one of many flu viruses. The flu virus is easily spread from person to person. Some types of flu illnesses can be prevented by getting a flu shot (or inhaled flu mist ) containing flu medicine (vaccine). If a healthcare worker is not allergic to the flu medicine, have other illnesses or other reasons that prevent them from getting the flu medicine, it is recommended that healthcare workers get a flu vaccination every year to protect themselves, coworkers, patients and their families from the flu. This is a hospital self-reported measure of the rate of seasonal flu vaccination among hospital staff that are hospital-paid employees. In Iowa during the flu season, approximately 92 out of every 100 healthcare workers were immunized with the seasonal flu vaccine. Higher rates for this measure are better. Hospitals shaded in green ( ) had a high flu vaccination rate among their healthcare workers. 16 HAI - 7 Methicillin-Resistant Staphylococcus Aureus (MRSA) Surgical Site Healthcare-associated Infections Methicillin-resistant staphylococcus aureus (MRSA) is a type of bacteria that may cause a serious infection that is hard to treat with certain antibiotics. These antibiotics include methicillin and other common antibiotics such as oxacillin, penicillin and amoxicillin. Patients that get a MRSA infection, also known as a staph infection, are more likely to have longer and more expensive hospital stays, and may be likely to die as a result of the infection. Healthcare workers must take precautions to prevent such infections. This is a hospital self-reported measure of the rate that CABG (heart bypass surgery), colon, hip, and hysterectomy surgery patients experienced a MRSA infection after their surgery. In Iowa in, there were approximately 3 MRSA surgical site infections for every 1,000 CABG, colon, hip, or hysterectomy surgeries. Lower rates for this measure are better.
