1 Minnesota Housing Directory Compiled by: Minnesota Department of Corrections Community Reentry Unit Updated Fall 2013

2 Central Office 1450 Energy Park Drive Suite 200 St. Paul MN, PH Fax TTY The Community Reentry Unit of the Minnesota Department of Corrections produced this booklet. The aim is to address the numerous requests made by caseworkers, corrections agents, community workers and transition support people for a detailed directory of housing options available to offenders reentering the community and the public at large. This directory is not a complete listing of the varied array of housing options currently available across Minnesota. We recognize that housing resources shift and change dramatically over time. Therefore, we welcome feedback on housing information that will allow us to expand and improve the directory. An online edition of this directory is available at under the Publications link. This online directory will be updated as new housing information becomes available. Kelley Heifort Community Reentry Director Kelley.Heifort@state.mn.us 2

147 Hennepin County Journey Homes Lutheran Social Service Hayes-Gibson 2414 Park Avenue, Minneapolis, MN Permanent Supportive Housing (PSH) is similar to transitional housing in that families receive affordable housing and support services. However, the primary difference is that PSH is intended for families with greater needs and who require more intensive and long term services than transitional housing offers. In transitional housing, families can only stay in the program for a set length of time; however, in PSH, families may stay in the home for as long as they need. Permanent Supportive Housing connects families to their home and their community. Through this model, families have access to other professionals that include a case manager, mental health support counselor, parent educator, and life skills worker. Following are the three basic criteria to be eligible for PSH: Families must be homeless as defined by the U.S. Department of Housing and Urban Development The head of household must have a documented disability as defined by the U.S. Department of Housing and Urban Development Families must need low-cost permanent housing with supportive services Journey Homes is the site-based Permanent Supportive Housing program of LSS Metro Housing and Homeless Services.
