SUMMARY In support of the Clayton County Board of Health s (CCBOH) effort to integrate services across the entire agency, the Clayton County Board of Health is accepting bids from firms or individuals for the development and design of a display that can be used to represent the programs and services provided by the Clayton County Board of Health. 2. PROGRAM BACKGROUND The Clayton County Board of Health and its programs have many opportunities to exhibit and inform target populations, partners, community and faith based organizations, profit/non-profit entities and other governmental agencies about Clayton County Board of Health programs. In the past, multiple programs have often been exhibited at the same events. In the current era of budget accountability and constraints, the Clayton County Board of Health can maximize resources and coordinate and integrate the promotional efforts of all its programs through this multi-purpose display. In order to inform professionals, stakeholders, elected officials, the media and the public health issues in Clayton County, this display will be designed to build on the State of Health report on Clayton County published in October The display will also use the logo and brand identity also developed in October RECOMMENDED (OR SIMILAR) SPECIFICIATIONS Two (2) displays are needed. One (1) will be a tabletop exhibit, approximately 4ft in height, and one (1) will be free-standing, approximately 8ft in height. Both will have five (5) panels and meet the following additional criteria: A curved end cap A three-panel photomontage depicting beneficiaries of the health department services in Clayton County (digital inkjet mural panel print on polyester with luster over laminate mounted to Lexon or other cost-effective current technology or similar specifications). Clayton County Board of Health will be superimposed in the center of the photomontage. The colors, design, and photos used will compliment CCBOH logo and brand identity. Color choices TBD. The name of Clayton County Board of Health Operating Departments (TBD) will be superimposed in the Clayton County Board of Health 2

PROGRAM BACKGROUND The Clayton County Board of Health and its programs have many opportunities to exhibit and inform target populations, partners, community and faith based organizations,

3 center of the photomontage. The colors, design, and photos used will be consistent with the photomontage. The new Clayton County Board of Health logo will be at the bottom of this panel. A fourth panel that is interchangeable: Full-length fabric panel that can be customized for each program unit participating in initial design. These include Adolescent Health and Youth Development, Children and Youth with Special Needs, FOCUS/Maternal and Infant Health, Men s Health, Environmental Health, and Emergency Preparedness and Response. Note: Other Units or programs can order additional printed panels for their programs (not a part of this proposal) A Specialized panel to house a flat screen TV and DVD/VCR combo player with two sets of shelves (size TBD) or similar specifications. This section can be used to highlight the County s State Of Health Report, show loop power point shows, or videos. Hardware and magnetic frame 14 /0.35 Cylindrical Case that can be converted into a free standing counter with two 3 pocket literature holders. Set of 2 Premium Halogen Lights and Electro Strut in a box or similar specifications Collapsible literature stand with four (4) pockets 2 deep or similar specifications Three separate shippable cases on wheels, 46 lbs. each or similar specifications The bid must include the cost of purchase of display board/hardware and the cost of the graphics design work/graphics designer On site training in set up, dismantle, and packing techniques 4. PROPOSAL GUIDELINES AND REQUIREMENTS This is an open and competitive process. Proposals received after 3:00pm (EST) on Friday, April 15, 2011 will not be considered and returned unopened. The price you quote should be inclusive. If your price excludes certain fees or charges, you must provide a detailed list of excluded fees with a complete explanation of the nature of those fees. The vendor should include the cost of purchasing the display board based on the recommended specifications above and the costs of a graphics designer to design and incorporate the photos, graphics, logo, and messages identified above. CCBOH has action-shots/photos that should be used in the design of the displays. Clayton County Board of Health 3

4 If the execution of work to be performed by your company requires the hiring of sub-contractors you must clearly state this in your proposal. Sub-contractors must be identified and the work they will perform must be defined. In your proposal please provide the name, address, and EIN of the sub-contractor. The Clayton County Board of Health will not refuse a proposal based upon the use of sub-contractors; however we retain the right to refuse the sub-contractors you have selected. Provisions of this RFB and the contents of the successful responses are considered available for inclusion in final contractual obligations. 5. CONTRACT TERMS CCBOH will negotiate contract terms upon selection. All contracts are subject to review by CCBOH legal counsel, and a project will be awarded upon signing of an agreement or contract, which outlines terms, scope, budget and other necessary items. 6. TIMELINE This RFB is dated April 7, Proposers may also request a PDF copy be sent via by contacting clmclean@dhr.state.ga.us. Proposals received via USPS or overnight courier must be postmarked by April 15, Hand-delivered proposals are due no later than 3:00 p.m. (EST), Thursday, April 15, Bid technical evaluation (based on requirements in Section 7) will occur beginning on Tuesday, April 19, 2011 and expect to be completed by Thursday, April 21, The name of the candidate/firm who has been selected will be decided on or about Friday, April 22, 2011 and we will announce award on or about Monday, April 25, Awarded vendor should plan for a meeting with CCBOH program staff on Wednesday, April 27, The complete project needs to be delivered no later than Wednesday, May 18, All other candidates will be notified on or about April 29, BUDGET LIMIT CCBOH has allocated $8, for this project. The Clayton County Board of Health will not refuse a proposal based upon the use of sub-contractors; however we retain the right to refuse the sub-contractors you have selected.

5 8. BID REQUIREMENTS Bids should not exceed five (5) type-written pages (excluding the cover sheet). Bids should be typewritten using Times New Roman, 12 pitch font. Vendors should submit an original and five (5) copies of their proposal. Each proposal must include the following: a. A completed Bid Cover Sheet (See Attachment A) b. A proposed line-item budget c. Drawings, pictures, diagrams, and examples of work products should be included with the bid. Vendors can attach to the bid or download onto a CD-ROM. 9. STAFF RESOURCES The CCBOH will provide the following: Programmatic information on programs included in the initial design to include: Program name and background; rationale; purpose; statistics; description of services offered; location of services; and target population served. Limited agency stock photos are available, but the vendor should be prepared to identify additional stock photos as appropriate. Agency logo and brand identity details Access to the Agency s Public Information Officer and Agency leadership. Review and approval of final designs for displays. Clayton County Board of Health 5
