5 Holiday and Overtime Rates For Physicians: No Holiday or overtime physician receives the normal hourly rate. For Allied Healthcare Providers (SINs ): 1.3x the normal hourly rate for any hours worked in excess of 40 hours or 1.3x the normal hourly rate for Federal Holidays. For Physicians (excludes Emergency Medicine) Weeknight On-Call Rate: Weeknight is a one-time flat charge equal to the negotiated hourly rate when a physician is required to be available by phone or pager and be able to provide phone consultation. The weeknight period is generally 4:30 pm 8:00 am, Monday - Friday. Physician must be within 30 miles or 30 minute radius from the facility. Weekend/Holiday On-Call Rate: Weekend on call is defined as a 24 hour period Saturday 8am- Sunday 8am and Sunday 8am Monday 8am. The weekend on call rate applies when a physician is required to be available by phone or pager and be able to provide phone consultation. Physician must be within a 30 mile or 30 minute radius from the facility. If a physician has to report to the facility and/or see patients, the negotiated hourly rate for actual worked hours will be charged with a one (1) hour minimum. See schedule for proposed rates. Call Back Rate: If the physician must report to the facility and see patients, the negotiated hourly rate applies when physically onsite for all hours worked. For Allied Healthcare Providers (SINs ) Note 1 -Weeknight/Weekend On Call: $6.00 per hour for the time the provider is on call. On call provider is to carry a phone or pager and be able to provide phone consultation. The provider must be within 30 minutes or 30 mile radius from the facility. Note 2 - SINs 053 (CRNA) and 58 (CNM): $10 per hour for the time the provider is on call. On call provider is to carry a phone or pager and be able to provide phone consultation. The provider must be within 30 minutes or 30 mile radius from the facility. Weeknight/Weekend Call Back: The normal hourly rate applies if less than 40 hours worked. In excess of 40 hours, overtime of 1.3x the normal hourly rate applies for each hour worked when called back and physically onsite at the facility. No minimum billable hours. Minimum Qualifications of Professionals: through 022 Physician Each physician will possess: A medical (MD) or Osteopathic (DO) degree or Certification by The Educational Commission for Foreign Medical Graduates (ECFMG) if appropriate. Completion of a Residency Training Program in their specialty approved by the Accreditation Council of Graduate Medical Education (ACGME). Completion of a Fellowship Training Program in their subspecialty approved by the Accreditation Council of Graduate Medical Education (ACGME), if required for certification. Board Certification or Eligibility by their specialty American or Osteopathic Board. A current, valid medical license in one of the U.S. states, territories or commonwealths. 6 certification according to requirements for specialties (i.e. BLS, ACLS, PALS, NALS, etc ). Minimum one year experience in their respective specialty Emergency Medicine Trauma I, II In addition to the Physician qualifications listed above each physician shall be a Doctor of Medicine or Osteopathy who has successfully completed an approved Emergency Medicine Residency and is Emergency Medicine Board Certified. Minimum one year experience Dentist Each dentist will possess: A DDS or DMD degree from an American Dental Association accredited program. Board Certified or Eligible by the American Board Certification. Applicable certification according to the requirements for specialties (i.e. BLS). A current valid unrestricted license in one of the U.S. states, territories or commonwealths. Minimum one year experience or applicable subspecialty training Physician Assistant A Bachelor of Science Degree and completion of an accredited Physician Assistant Program. Certification by the National Commission on Certification of Physician Assistants, Subspecialty training as appropriate. A Current, valid license in one of the U.S. states, territories or commonwealths. Must have a minimum of one year experience Radiologic Technology Computer Tomography (CT) Technologist Must have completed an accredited educational program in Computer Tomography. Must have current valid registration/certification in one of the U.S. states, territories or commonwealths. Must have a minimum of one year experience Radiologic Technology Diagnostic XRay Must have completed an accredited educational program in XRay Technology. Must have current valid registration/certification in one of the U.S. states, territories or commonwealths. Must have certification by the American Registry of Radiologic Technicians (ARRT). Must have a minimum of one year experience Dosimetrist Must have completed an accredited educational program in Dosimetry (minimum 18 mos), or have a Bachelor s degree in physical or biological sciences or greater or be a radiation therapy technologist. Certification by the Medical Dosimetrist Certification Board (MDCB). Must have current unrestricted license in one of the U.S. states, territories or commonwealths. Must have a minimum of two years experience under the supervision of a Certified Medical Dosimetrist or Medical Physicist.
