Valid license in at least one state, territory or commonwealth of the US Pharmacist/Pharm D Pharmacists will possess a Masters or Doctorial Degree from an accredited School of Pharmacy with completion of a Pharmacy Practice Residency or a Pharmacy Practice Fellowship. Must have Basic Life Support (BLS) Must maintain a current license in at least one state or territory of the U.S Physician Assistant Physician Assistants will have successfully completed a nationally accredited Physician Assistant Program. Must maintain certification and have current BLS and/or ACLS. Must maintain a current, unrestricted license in a state, territory or commonwealth of the U.S Emergency Response Professionals/EMT/Paramedics Graduate of an accredited Emergency Medical Technician and/or Paramedic Program. 13 Registered Respiratory Therapist A certificate as an RT from the National Board for Respiratory Care (NBRC) and a registry number, or a certificate from another body which the NBRC recognizes as its credentialing equivalent with registry number. Must maintain certification and have current BLS and/or ACLS. Minimum of one year experience within last three years. If required by state or contract, will maintain a current, unrestricted license in a Physical Therapist Will possess a Baccalaureate Degree in Physical Therapy from an American Physical Therapy Association approved program. Will maintain certification and have current BLS. Minimum of one year of full time experience within the last three years. Must maintain a current, unrestricted license in a state, territory or commonwealth of the U.S Athletic Trainer Will possess a Baccalaureate Degree in Athletic Training or Sport Sciences program from an accredited school Certified athletic trainers have fulfilled the requirements for certification established by the National Athletic Trainers' Association Board of Certification, Inc. (NATABOC) and will have current BLS. Minimum of one year of full time experience within the last three years. If required by state or contract, will maintain a current, unrestricted license in a Physical Therapy Assistant Providers will have successfully completed an accredited Associates Degree Program in Physical Therapy. Must have current BLS and a current OT Certification by the National Board for Certification in Occupational Therapy.
