Problem Solving/Termination/Replacements. Procedures will be determined at the ordering facility. 21. Minimum Qualifications of Professionals: : Physician - General and Family Practice The general and family practice physician provides examinations and medical care for the entire family. The physician is concerned with proper injury care and disease prevention, diagnosis, treatment, and recovery. The physician prescribes and administers medications, performs routine vaccinations, and provides advice regarding personal health and hygiene. The physician will request necessary tests and follow- up visits and refer patients to specialists, as necessary. Degree in medicine from an accredited school of medicine. Physicians will maintain a current, active, valid, and unrestricted medical or osteopathic medical license from a U.S. State. Physicians who have completed requirements for training and experience meeting the standards of various member boards of the American Board of Medical Specialties (ABMS) will be board certified in their respective specialties. 5

The physician is concerned with proper injury care and disease prevention, diagnosis, treatment, and recovery. The physician prescribes and administers medications, performs routine vaccinations, and provides advice regarding personal health and hygiene. The physician will request necessary tests and follow- up visits and refer patients to specialists, as necessary. Degree in medicine from an accredited school of medicine. 4 years Post Graduate Training Physicians will maintain a current, active, valid, and unrestricted medical or osteopathic medical license from a U.S. State. Physicians who have completed requirements for training and experience meeting the standards of various member boards of the American Board of Medical Specialties (ABMS) will be board certified in their respective specialties. Not Current, Valid DEA number : Physician Psychiatrist (General Adult, Geriatric) The Psychiatrist valuates patients for mental, emotional or behavioral disorders. This mental health specialist diagnoses disorders, including nature and extent, and determines the appropriate course of treatment for the patient. The Psychiatrist may prescribe medications when necessary. Degree in medicine from an accredited school of medicine. Psychiatrist will maintain a current, active, valid, and unrestricted license from a U.S. State. Possess board certification in Psychiatry by the American Board of Psychiatry and Neurology or the American Osteopathic Association. Not 6

7 : Physician - Psychologist Provides treatment to patients using talk therapy. Evaluates patients for mental, emotional or behavioral disorders. Diagnoses disorders, including nature and extent, and determines the appropriate course of treatment for the patient. Master s Degree in Psychology Psychologist will maintain a current, active, valid, and unrestricted license from a U.S. State. N/A Not : Registered Nurse General (Med Surge, OB/GYN, Geriatric, Psychiatric) As a valuable member of the healthcare team, performs comprehensive physical examinations; plans, assesses and provides specialized patient care. Records and maintains patient medical information Graduate of an accredited Registered Nurse program, BSN Preferred. Current, unrestricted license issued by any U.S. State. CEN Preferred At least 12 months of experience in past 24 months : Registered Nurse Specialty (Coronary, ICU, CCU, OR, Pediatric, Other) Plans and provides comprehensive nursing care in accordance with professional nursing standards. Uses judgment in assessing patient conditions, interprets guidelines, and modifies patient care as necessary. Recognizes and determines proper action for medical emergencies. Provides highly specialized patient care in a difficult specialty area, such as intensive care or critical care, etc. Graduate of an accredited Registered Nurse program, BSN Preferred. Current, unrestricted license issued by any U.S. State. PALS, and CCRN preferred 7

8 : Registered Nurse Specialty (Coronary, ICU, CCU, OR, Pediatric, Other) At least 12 months of experience in specialty, in past 24 months : Physician Assistant Provide healthcare services typically performed by a physician, under the supervision of a physician. Conduct complete physicals, provide treatment, and counsel patients. May, in some cases, prescribe medication Graduation from a physician assistant training program accredited by the Accreditation Review Commission on Education of the Physician Assistant (ARC- PA). Current, unrestricted license issued by any U.S. State. Current certification by the National Commission on of Physician Assistants. Not required At least 12 months of experience in specialty, in past 24 months. Current, Valid DEA number : Audiological Technician Diagnoses and treats conditions related to speech and language. Studies factors related to speech and communication. Diagnosis and evaluates speech and language skills as related to educational, medical, psychological and social factors. Plans, directs and conducts rehabilitative treatment programs. Master s Degree in Speech Pathology. Current, unrestricted license to practice as a speech pathologist in one of the 50 U.S. States. Must possess a Certificate of Clinical Competency in Speech Pathology (CCC- SLP) from the American Speech- Language- Hearing Association (ASHA). Not : Physical Therapy Physical Therapist Assess, plan, organize, and participate in rehabilitative programs that improve mobility, relieve pain, increase strength, and decrease or prevent deformity of patients suffering from disease or injury. Works 8

9 : Physical Therapy Physical Therapist with physicians. May supervise physical therapy technicians. Bachelor of Science Degree in Physical Therapy from an accredited Physical Therapy institution. Current, unrestricted license to practice physical therapy in one of the 50 U.S. States. As required by state and/or certifying authority. Not : Physical Therapy Physical Therapist Assistant Physical therapy Assistant performs a variety of tasks under the direction of a Physical Therapist. Associate of Science Degree in Physical Therapy from an accredited Physical Therapy program. Current, unrestricted license to practice physical therapy in one of the 50 U.S. States. As required by state and/or certifying authority. Not required : Medical Records Clerk Medical records and health information technicians assemble patients' health information including medical history, symptoms, examination results, diagnostic tests, treatment methods, and all other healthcare provider services. Technicians organize and manage health information data by ensuring its quality, accuracy, accessibility, and security. They regularly communicate with physicians and other healthcare professionals to clarify diagnoses or to obtain additional information. High School Diploma and in a Medical Assistant Program. As required by state and/or certifying authority. Not required At least 12 months of experience in past 24 months : Ophthalmic - Technologist 9

10 The Ophthalmic Technician s duties include preparing examination and treatment rooms, reviewing the appointment schedule, performing basic and routine vision screening examinations, administering eye medications, cleaning and maintaining ophthalmic instruments and assisting physicians during minor surgical and laser procedures. Minimum high school diploma, or equivalent and completion of an Ophthalmic Technician training or certification program. N/A Must possess at least one (1) of the following: Certified Ophthalmic Assistant (COA); Certified Ophthalmic Technician (COT); or Certified Ophthalmic Medical Technician (COMT). Not : Counselor - Social Work (Graduate Degree) The licensed clinical social worker provides comprehensive services through a broad range of individual, family, programs and services to sustain, restore or enhance the social well- being and functioning of individuals, families, and communities. d clinical social workers are members of the facility s healthcare team, frequently working in social work service, outpatient mental health clinics, family advocacy programs; and substance abuse treatment services; mental health services, combat stress detachments and correctional facilities. Services provided shall comply with the National Association for Social Workers (NASWs') Standards for Social Work Case Management, Standards of the Joint Commission; Standards of Practice of the Case Management Society of America; the American Accreditation Health Commission/Utilization Review Advisory Commission (URAC); other applicable credentialing and accreditation agencies and applicable provisions of law and the rules and regulations of any and all governmental authorities pertaining to: (a) Licensure and/or regulation of healthcare personnel in treatment facilities, and (b) The regulations and standards of professional practice of the treatment facility, and, (c) The bylaws of the treatment facility s professional staff. Master of Social Work (MSW) degree from a school of Social Work accredited by the Council on Social Work Education. Must possess a current, active, valid and unrestricted MSW license at any level from a U.S. State. As required by state or licensing authority. 10

11 : Counselor - Social Work (Graduate Degree) Not Minimum 2 Years post- MSW clinical social work experience : Pharmacy Technician - Certified Under the direct supervision of a registered pharmacy, receives, interprets, fills and dispenses medical prescriptions. Performs some clerical duties relating to the pharmacy department. Receives and stores incoming supplies. Verifies stock and enters data in computer to maintain inventory records. Must be a graduate from a pharmacy technician program accredited by the American Society of Health- System Pharmacists (ASHP). Not applicable Must be certified by the Pharmacy Technician Technician Board (PTCB). Not : Nurse Practitioner Assess, plan and provide comprehensive patient care independently or in autonomous collaboration with other health professionals. Will usually provide specialized care, i.e. family practice, pediatrics, etc. Must be a graduate from a Master s of Nursing program accredited by the National League for Nursing Accrediting Commission (NLNAC) or the Commission on Collegiate Nursing Education (CCNE). Current, unrestricted nurse practitioner license in one of the 50 U.S. States. Must possess a current certification in the specialty practiced as a nurse practitioner by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP). Not Current, Valid DEA number : LP/VN (d Practical/Vocational Nurse) Participates in the implementation and evaluation of patient care. Familiar with standard concepts, practices, and procedures within a 11

12 : LP/VN (d Practical/Vocational Nurse) particular field. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. Works under general guidance and typically reports to a registered nurse unless determined otherwise at facility level. A certain degree of creativity and latitude is required. High school diploma and a minimum of one- year experience within the preceding 3 years unless otherwise stated in the Task Order. Must possess maintain a valid, unrestricted license to practice as an LPN/LVN in any one of the 50 States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands Is certified as a LPN/LVN. The HCW is responsible for complying with all applicable State licensing regulations. Additionally, HCW s licensed outside of the 50 States must provide proof of having successfully passed the d Practical Nurse National Council Licensure Exam (LPNNCLEX). Not required. Minimum 2-4 years of clinical experience. and Licensure Requirements Certified Nursing Assistant Provide direct patient care and patient transport under the direction of a licensed professional. Assists patients with ADL s and assists licensed staff with patient care exclusive of medication administration. May not draw blood.
